1 Position Description Central Gippsland Health Service Service: Nursing Position Title: Registered Nurse Classification level: YP2-YP11 The Service: Central Gippsland Health Service (CGHS) is the major provider of health and residential aged care services in the wellington Shire with more than 70 acute beds: 180 high and low care residential aged care beds, as well as 15 independent living units. Acute services are situated at Sale (incorporating an Operating Suite, Emergency Department, Critical Care Unit, Obstetrics, Level 2 Nursery and Paediatric services) with some acute beds located at Maffra and Heyfield. CGHS is an integrated health service providing a wide range of primary health and community support services; including Home and Community Care (HACC) throughout the Shire of wellington with community centres at Maffra, Sale, Heyfield, Rosedale and Loch Sport, and residential services at Sale, Maffra and Heyfield. Reports to: Nurse Unit Manager The structure: Director of Nursing Nurse Unit Manager Registered Nurse Grade 2 Approximately 900 staff fill the equivalent of 620 Full Time positions. Decision making / delegation This position is at operational level and will have input into planning and decision making processes as part of the service team. The position reports to the Nurse unit Manager who holds a delegated authority to the level of Department Head/Manager in accordance with CGHS Instrument of Delegation. The work context: Registered Nurses work as part of the health care team to advocate for and facilitate the involvement of individuals, their families and significant others in planning and evaluating care and progress towards health outcomes. The purpose of the position: The position scope is to provide clinical and aged care to patients across all settings within CGHS including acute inpatient, outpatient and residential aged care whilst achieving and maintaining high standards of quality care and service, reliability and safety, upholding and fulfilling the organizations vision, mission and values. This will occur within the principles of care coordination, where patients/clients/residents have access to a highly coordinated health care and community support system. Page 1 of 7
2 The key responsibilities of the role: Specific Responsibilities Provide patient-centred nursing care include recognition of normal and abnormal in assessment, intervention and evaluation of individual health and functional status. Support patient, client and community participation in decisions in all aspects of the service. Monitor the impact of nursing care and maintain ongoing communication with the nurse in charge regarding the health and functional status of individuals. Use critical and reflective thinking skills in contributing to decision making which includes reporting changes in health and functional status and individual responses to health care interventions. Provide support and comfort, assisting with activities of daily living to achieve an optimal level of independence, and providing for emotional needs of individuals. Participate in interdisciplinary assessment and service delivery planning process consistent and support care coordination across the care settings. Use information technology with regard to the application of health care technology Function in accordance with legislation, CGHS and professional policies and procedures affecting registered nursing practice. Support the development of and comply with organisational and service level policies and procedures pertinent to registered nursing practice. Identify and report breaches of law, policies and procedures related to nursing practice to line manager, nursing authorities or other appropriate authority. Maintain an effective process of care when confronted with differing values and beliefs. Liaise with others to ensure that the rights of individuals/ groups are maintained. Liaise with others to ensure that the spiritual, emotional and cultural needs of individuals/groups are met. Contribute to the provision of relevant health care information to individuals and groups. Consult with other members of the health care team to facilitate the provision of accurate information to, protect rights of, and enable informed decisions by, individuals and groups. Contribute to the review of care plans in conjunction with individuals/groups. Accurately collect and report data regarding the health and functional status of individuals and groups. Participate in evaluation of progress of individuals and groups toward expected outcomes and reformulation of care plans. Implement planned nursing care to achieve identified outcomes. Participate in team meetings and case conferences. Ensure communication, reporting and documentation are timely and accurate. Organise workload to facilitate planned nursing care for individuals and groups. Prioritise the delivery of nursing care to individuals/groups. Contribute to the health education of individuals or groups to maintain and promote health. Generic Responsibilities Work collaboratively with all members of the multidisciplinary team to facilitate integrated person-centred care leading to timely and effective client discharge. Actively participate in quality activities and accreditation processes in accordance with standard practice. Manage OH&S consistent with overall OH&S programs, policies and procedures within CGHS and also: look after your own health; look out for the health and safety of others in the workplace; follow safe work practices; report hazards and injuries; participate in agency health promotion initiatives and support healthy lifestyle choices for staff Contribute extensively to the development of an annual capability development plan designed to support you to: Page 2 of 7
