1 Position Description Title Directorate Eastern Metro Region (EMR) Working Together Maternal & Child Health Nurse Community Development Department Family Youth & Recreation Location Classification Boroondara Municipal Offices Eastern Division Child Protection EMR Working Together MCH Nurse Position code Position purpose To strengthen the relationship between Maternal & Child Health and Child Protection and to develop and establish a communication framework between Child Protection, Out of Home Care and Eastern Metropolitan Region Maternal & Child Health. 1 Review date: March 2017

2 To develop through extensive consultation with practitioners from both areas a referral framework where information can be shared, fostering two way communications relevant to clients known to Child Protection and Out of Home Care. To embed the referral pathway in EMR practice and set up a sustainable approach to professional interactions between Maternal & Child Health, Out of Home Care and Child Protection. 3 Develop and implement the communication framework between Child Protection and EMR Maternal & Child Health. Develop and Pilot a two way information exchange and referral pathways. Document a communication framework. Produce a midpoint and final report for all partners. Adhere to the timelines and project outcomes as outlined in EMR Working Together Grant document. Work with Child Protection and Early Childhood Services to streamline processes and procedures relating to requirements of the Early Childhood Agreement. Work with Child Protection and EMR Maternal & Child Health Coordinators to facilitate the development of processes for the efficient exchange of information in relation to High risk infants. Provide secondary consultation to Intake and Child protection Areas regarding High Risk Infants. Participate in the High Risk Infant panel. Undertake joint home visits with Child Protection practitioners to selected clients. Promote a positive image of Maternal & Child Health and Child Protection to co-workers and the community through a commitment to professional standards and a high level of client service standards. Participate in and provide feedback to the EMR Maternal & Child Health team meetings to ensure all nurses are aware of the innovation project. Maintain and encourage constructive dialogue between all members of the MCH teams. This is an organisation wide project requiring commitment from all employees to ensure a successful transition from our current customer service model to a customer-centric model in a digital environment. 3 Review date: March 2017

4 Staff will be involved in the Pi (process improvement) process that requires an examination of work processes in their teams to identify those that are operating efficiently, those which are obsolete and those which could be improved through change. Document management Document management is a shared responsibility between staff, managers, corporate information and system administrators. The successful applicant will have a responsibility to: Display a willingness to work towards continuous improvement as defined by both City of Boroondara and East Division Child Protection. Follow all organisational procedures for both City of Boroondara and Child Protection. Maintain accurate records as required by both City of Boroondara and East Division Child Protection. Provide reports and framework as required in the EMR Working Together Project Outputs. OHS and Risk management Follow established safe working practices, procedures and instructions. Take reasonable care for their own OHS and that of their colleagues. Seek assistance when unsure of practices and procedures to perform a task. Report all hazards, incidents, injuries, near misses and potential risks as soon as practicable to their supervisor. Implement preventative OHS initiatives and maintain a healthy and safe work environment. Discuss OHS and Risk Management as a part of daily decision making processes, operations and team meetings. Discuss OHS and Risk Management daily decision making processes, operations and Department meetings. Conduct and document system reviews and inspections of employees, volunteers and contractors with assistance from the OHS and Risk Management Teams, team leader(s), HSRs and/or personnel. 4 Review date: March 2017

5 Ensure appropriate training and instruction is given to personnel allowing them to complete jobs safely and without risks to health. Take appropriate action to alleviate any hazardous situation, unsafe act or omission that is observed or brought to their attention and provide appropriate feedback. Ensure that hazards, incidents and near misses are recorded, promptly investigated and that, where appropriate, preventative (in alignment with the hierarchy of controls) and corrective action is taken. Assist with the implementation of Return to Work Plans. Encourage, recognize and reward OHS and Risk Management achievements. Ensure that relevant OHS legislation is complied with. Judgment and decision making Exercise professional and independent judgment, free of bias or political consideration. At all times, judgment is aimed at achieving the best outcome for vulnerable families by adhering to Council and Child Protection s policies, service plans, goals and objectives. The incumbent should make decisions appropriate to their duties in accordance with Maternal & Child Health Service Practice Guidelines 2009, applying professional judgment directed towards Council s objectives and not outside guidelines issued by the Coordinator Maternal & Child Health. Specialist skills and knowledge Ability to define goals, set priorities and ensure the outcomes identified in the EMR Working Together Innovation Grant are met. Ablilty to work independently and apply theory and evidence based approaches to practice relevant to vulnerable families. Ability to produce the Midpoint Review and Report. Ability to provide training to EMR MCH and Child Protection in new protocols and pathways. Ability to produce an evaluated Final report of the agreed framework. Be able to demonstrate an understanding of legislative requirements relevant to Mandatory Reporting, the Health Act and Information Privacy. 5 Review date: March 2017

6 Management skills Demonstrate Skills in managing time, setting priorities and planning to achieve maximum health outcomes for client families known to Child Protection and Out of Home Care. Demonstrate an Understanding of and ability to implement personnel practices, including equal employment opportunity and Risk Management. Ability to respond to future changes in service delivery with flexibility and innovation. Ability to achieve objectives within a timetable. Ability to maintain comprehensive and confidential records of client consultations. Ability to initiate, develop and produce a framework which would be valued and utilized by Maternal & Child Health and Child Protection. Interpersonal skills High level oral interpersonal skills with an ability to establish and maintain open, effective communication processes. Demonstrated ability to produce written material and the ability to initiate and document procedures. Ability to discuss and resolve problems. Ability to lead and motivate other staff in a constructive and co-operative manner in a team environment. Qualifications and experience Demonstrated robust experience as a Maternal & Child Health Nurse and the ability to respond to families with complex needs. Post Graduate Diploma in Child and Family Health Nursing, or Post Graduate Diploma, Masters or Doctorate in Child, Family and Community Nursing. Current registration as both a registered nurse and midwife on the Australian Health Practitioner Regulation Agency register (AHPRA). Computer skills in a range of software applications. 6 Review date: March 2017

7 Well developed interpersonal and communications skills. Selection criteria Current registration as a both a registered Division 1 Nurse and Midwife with AHPRA. Post Graduate Diploma in Child & Family Health Nursing, or Post Graduate Diploma, Masters or Doctorate of Nursing Science in Child, Family and Community Nursing. Extensive experience as a Maternal & Child Health Nurse, with demonstrated ability to respond to families with complex risk factors. High level communication and interpersonal skills. Ability to develop and document a communication framework in line with the EMR Working Together Maternal & Child Health Innovation Grant application. Demonstrated knowledge and application of the requirements of the Department of Education and Early Childhood Development Maternal & Child Health Program Highly developed organisational skills and ability to think strategically, manage projects, establish work priorities and manage time effectively. 7 Review date: March 2017
