1 Week 1 Field Trip Permission Slip will be attending a field trip to: A Maze in Pottery in Briarcliff Manor on (date): Wednesday, July 2, Time at venue: 12:30 2:00 p.m. Special Instructions/notes: Your child s pottery piece will be available for pick-up at World Cup about a week and a half after the trip.

2 Week 2 Field Trip Permission Slip will be attending a field trip to: Bounce Trampoline Sports in Valley Cottage on (date): Friday, July 11, Time at venue: 12:00 1:30 p.m. Special Instructions/notes: Parents, you MUST sign Bounce s waiver, in addition to this permission slip, in order for your child to participate. You can do so electronically at: Please check this box to certify you have done so, and print Bounce s waiver and submit it to World Cup along with our permission slip. 3 Week 3 Field Trip Permission Slip will be attending a field trip to: Bounce U Elmsford (inflatables) on Friday, July 18, Time at venue: 12:00 2:00 p.m. Special Instructions/notes: Parents, your child MUST WEAR SOCKS in order to bounce! You may want to have your child wear clothing that covers knees and elbows. In addition, you MUST sign Bounce U s waiver in order for your child to participate, as well as this permission slip, and return both to us. Thank you!

4 Week 4 Field Trip Permission Slip will be attending a field trip to: Legoland in Yonkers on Wednesday, July 23, Time at venue: 11:00 1:00 p.m. Special Instructions/notes: We will eat lunch at the facility, consisting of a choice of ham and cheese, or turkey and cheese, or just a cheese sandwich, with chips and a drink. No outside food is allowed. Please have your child wear socks.

5 Week 5 Field Trip Permission Slip will be attending a field trip to: Tarrytown Music Hall to see a production of Hairspray Jr. on Friday, August 1, Time at venue: 10:30 12:30 p.m. Special Instructions/notes: None.

6 Week 6 Field Trip Permission Slip will be attending a field trip to: Life: The Place To Be in Ardsley on (date): Wednesday, August 6, 2014 Special Instructions/notes: Please have your child wear sneakers in order to do rock climbing. If he or she wears white, it will show up a lot when playing laser tag! We will eat lunch before this trip, so I prefer they do not bring money with them as it is difficult to keep track of!

7 Week 7 Field Trip Permission Slip will be attending a field trip to: Bounce Trampoline Sports in Valley Cottage on (date): Wednesday, August 13, Time at venue: 12:00 1:30 p.m. Special Instructions/notes: Parents, you MUST sign Bounce s waiver, in addition to this permission slip, in order for your child to participate. 8 Week 8 Field Trip Permission Slip will be attending a field trip to: The Cliffs at Valhalla to go rock climbing on Thursday, August 21, Time at venue: 10:00 12:00 p.m. Special Instructions/notes: Parents, your child must wear sneakers in order to rock climb safely. In addition, you MUST sign The Cliffs waiver electronically (no paper option) in order for your child to participate, as well as this permission slip. 9 Week 9 Field Trip Permission Slip will be attending a field trip to: Palisades Climb Adventure in West Nyack on Wednesday, August 27 th, Time at venue: 12:00 2:00 p.m. Special Instructions/notes: Closed shoes are required. Children must be 3 6 (42 ) in order to climb.
