2 Aloha, Parents! Moʻolelo O Lānaʻi Important Dates & information (Please separate this page from the registration form and keep this at home for reference.) Under the creative direction of Aunty Irene Davis and many other talented and generous leaders, we have an amazing program planned and look forward to your children experiencing it. Thank you for entrusting us with your children; we are excited to spend the summer with them. It is important that he/she comes to school after having received a good night s sleep as well as a healthy breakfast so that they are prepared to experience an active and fun day. Moʻolelo O Lānaʻi is a seven-week program beginning on Monday, June 8 th and ending on Friday, July 24 th. Each day school starts promptly at 7:45 am and ends at 5:00 pm. The program is being held on the campus of LHES. Important dates: Monday, June 8 th - First day of summer program (Drop off your child in the LHES turn-around car line any time between 7:00 am and 7:45 am.) Fieldtrips are weekly, every Wednesday 6/10, 6/17, 6/24, 7/1, 7/8, 7/15, 7/22 School holidays are Thursday, June 11 (Kamehameha Day) and Friday, July 3 (observance of the 4 th of July Holiday) Friday, July 24 th Last day of summer program with Hoʻoike celebration and performance at 4:00 pm. (Parents and family are invited and encouraged to attend. Bring your cameras and video recorders.) What your child needs for school each day: Dress in cool, comfortable play clothes and covered shoes Pre-apply sun screen, as many of the activities will be out doors Each day, he or she should bring the following items: o Extra set of clothes o Hat o Towel o Lunch (packed in a cooler or a bag with ice packs). Water and snacks will be provided throughout the day. In addition, If your child is in grades K 2, please bring a sleeping mat or blanket each day On swim days, which will be announced in advance, bring swim suit, towel and sun screen Attendance: If your child will be absent, please advise the summer program office by 7:50am that day, by calling Behavior expectations & procedures: Thank you for entrusting us with the care of your children. The Moʻolelo O Lānaʻi summer program welcomes all children, but reserves the right to dismiss, at any time, any student whose conduct is deemed inappropriate, unsafe, and/or counter to the standards or the goals of the summer program. Under no circumstances will bullying or hazing be tolerated. In the event that dismissal or withdrawal is necessary, no refund of tuition will be issued. 3 ACCIDENT, MEDICAL, FIELD TRIP, AND MEDIA RELEASE I/WE, guardian of who is (Name of Parent or Legal Guardian) (Student Name) attending the LHES Summer Enrichment Program June 9, 2014 through July 17, 2014, release all officers/directors/staff members, teachers, and helpers of LHES program and activities, and all other sponsoring agencies and/or organizations of any claim for damages, liability, injury, expense or loss on account of negligence or other wrong doing that may occur while my/our child is attending all program activities. I/We also agree to indemnify and hold harmless those persons of the stated organizations on any claim arising out the activities under this agreement. In case of accident or need for medical attention/we give permission to LHES director or other staff members to take my/our child to a doctor, dentist, and /or emergency medical facility. I/we give permission for my/our child to participate in field trips associated with the program. I/we give permission to LHES to transport my/or in a Non-school approved vehicle as they deem necessary and therefore I/we waive also the state s liability. I/We also hereby give permission to LHES to film, tape, or otherwise record my/our child s name, voice, and/or person. I/we understand that these recordings of my/our child may include news release to include photographs and other media releases to publicize LHES, and open and close broadcast, and/or cable television transmission within or outside of the State of Hawaii in perpetuity. I/we also understand that there will be no financial or other remuneration for recording my/our child, either for initial or subsequent or playback.
