1 JOB DESCRIPTION Job Title: Department: Location: Responsible to: Ward Sister / Charge Nurse In Patient Unit Warwick Myton Hospice TBA Date JD reviewed: October 2015 Summary of Role: To lead the nursing team in developing, delivering and maintaining high quality specialist palliative care to patients. To facilitate and maintain an atmosphere that reflects the Hospice philosophy and values and supports the Myton Strategy. To promote a culture of learning and development. To support the Deputy Director of Nursing to ensure the effective and efficient operational management of the Ward and the delivery of nursing care. The post holder will at all times practice with care, compassion, competence, good communication, courage and commitment. 2 Key Relationships for this role: Deputy Director of Nursing, Deputy Sister, Senior Staff Nurses, Staff Nurses, Team leaders, Nursing Assistants, IPU Volunteers Key Responsibilities: 1. As a specialist practitioner, facilitate a holistic approach to patient care encompassing all aspects of assessment, planning, delivery and evaluation of care. 2. To provide professional leadership through clinical expertise to all the Ward staff and ensure that practice of specialist palliative care is evidence based approach is promoted within the clinical area. 3. Promote a patient centred approach to care in collaboration with carers and all relevant health care professionals. 4. To ensure a clinical learning environment where students, junior staff and those on placement to Myton achieve their learning needs and expertise is shared with the wider health economy. 5. Ensure that all clinical documentation is completed correctly and appropriately by members of the nursing team in line with NMC Guidance. 6. Work closely with nursing and care teams and encourage teams to be proactive in facilitating discharge and ensure that standards of discharge are implemented and maintained in liaison with the discharge manager/co-ordinator. 7. To facilitate others to be proactive in preparing and delivering clinical information to promote the effectiveness of the MDT, acting as an equal member. 8. To act as a clinical resource to others and to ensure the effective coordination of services to meet patients'/relatives' needs. 9. To participate in clinical audit as required by the organisational requirements and the Clinical Governance agenda. 10. Be responsible for own continued professional development to maintain up to date knowledge across the palliative care trajectory. 11. To be aware of the political and national influences affecting clinical care, such as NICE guidance/end of Life Care strategy. 12. To abide by the policies and procedures of Myton Hospices, the guidelines and Code of Conduct of the Nursing and Midwifery Council and the regulations and standards of the Care Quality Commission. 13. Ensure the Ward environment is clean and tidy and that all staff adhere and comply to the Hospices Infection Control Policy, dress code and all other Hospice Policies and Guidance.

3 14 Ensures that documentation is completed accurately and legibly in accordance with NMC guidance and confidentiality is maintained using Caldicott Guidelines. 15. Ensure all staff work in accordance with Myton Hospice policies, guidelines and procedures. Professional Leadership / Organisational Responsibilities 1. To take responsibility for the organisation and management of the Ward including the supervision of staff and teaching of students. 2. To take responsibility for leading, coordinating and supporting the volunteers on the Ward ensuring effective communication is established and maintained. 3. To co-ordinate Ward off duty planning, study and annual leave allocation and monitor staff sickness/absence levels and bank/agency staff usage working within designated budget wherever possible. 4. Participate in budget setting and business planning as required within the Hospice. 5. Act as a positive role model for others to aspire to, leading by example. 6. Exercise strong, effective leadership skills and establish excellent communication with all professionals, patients, relatives and all disciplines within the wider multi-disciplinary team. 7. Provide leadership that promotes patients and users as partners in care and treatment. 8. Ensure the Service continually improves to meet the changing needs of the patients. 9. Ensure all nursing staff receive an Annual Appraisal and review and provide an agreed Personal Development Plan as part of the Appraisal Process to support their own professional development. 10. Lead on risk management and incident reporting a foster a 'no blame' culture within the ward environment. 11. Be proactive in the process of recruitment, induction and retention of staff and volunteers. 12. In accordance with Data Protection create and maintain accurate contemporaneous patient records and ensure all staff adheres to Myton Hospices and the Nursing and Midwifery Council guidelines for record keeping. 13. Ensure effective and efficient use of resources in conjunction with the DDN to meet budgetary requirements exercising care and economy. Through effective leadership, ensuring time resources and staff are utilised and developed to their best potential. 14. To monitor and promote a safe working environment by supporting and undertaking risk assessments in liaison with the ADN. Under the Health and Safety at work Act (1974), ensure health and safety of staff, patients and visitors by complying with appropriate standards including, decontamination, infection control, COSHH, Medical Devices and the safe administration of medicines standards. 15. Ensure the timely completion of all documentation such as clinical adverse events by all staff and ensure all clinical incidents and complaints are investigated fully and documented appropriately

4 16. Assist the DDN in facilitating/participating with audit in the clinical area and the development and implementation of recommendations or actions identified to make improvements. Other Duties: 1. To be aware of the political and national influences affecting clinical care, such as NICE guidance/end of Life Care strategy. 2. To abide by the guidelines and Code of Conduct of the Nursing and Midwifery Council and the regulations and standards of the Care Quality Commission. 3. Ensure the Ward environment is safe, clean and tidy and that all staff adhere and comply with the Hospices Infection Control Policy, dress code and all other Hospice Policies and Guidance. 4.
