1 JOB DESCRIPTION Job title: Reports to: Department: Hours: Salary: SPC Team Leader Specialist Palliative Care Team Permanent 37.5 hours 36,230 to 38,664 (plus premium duty entitlements) JOB SUMMARY The Hospice at Home Service forms part of a Special Palliative Care Team. It is a community based palliative care service whose aim is to support patients and their carers within the community, usually in the patient's own homes. This role involves linking and collaborating with other health and social care providers in primary, secondary, independent and other voluntary care, to promote communication and seamless care and support for patients and their carers. KEY RESPONSIBILITIES Professional Leadership and Management Provide appropriate clinical support, to enable palliative care patients and their carers to receive care in their place of choice. Where appropriate enable patients to stay within their own home if that is their choice. Be proactive in developing collaborative relationships with other care providers. Work within clear protocols and guidelines to deliver seamless care. To work within operational and clinical procedures that is currently in place and to highlight any gaps or concerns to the Team Leader. Participate in the development of operational and clinical guidelines for the service.

2 To assist the Team Leader in integrating patient care services with other health, social and voluntary sector services. Delegate tasks/duties to other members of the team as appropriate. Undertake any other responsibilities that may be reasonably required as part of the role. Nursing practice To demonstrate assessment skills and be able to manage, plan, implement and evaluate patient care during the shift. To adhere and be professionally accountable to the NMC Code of Professional Conduct and be conversant with the Scope of Professional Practice and other NMC advisory papers. To act as an educator and advisor to the MDT and patients and families/carers and to provide advice, support for the carers and family according to their needs and the level of intervention required during the shift. To communicate and liaise where appropriate with the MDT to receive and provide handover and updates. To communicate with the existing palliative care providers as to allocation of resources and booking of planned and emergency visits as required. To work independently as well as alongside other team members of other professionals when required. To ensure continuity of patient care by communicating and liaising with medical and nursing colleagues, providing accurate and timely reports of any changes in the condition of the patient. Maintain accurate records in order to reduce the risk of errors. To maintain a level of knowledge, skill and competence related to current drugs/therapies in the treatment of pain and other symptoms, and administer medication according to the drug/prescription chart. To maintain awareness of professional responsibilities by reading current, relevant literature and by taking up opportunities for personal and professional development, in order to meet the requirements to maintain registration to practice.

3 Resource and Quality Management To assist the Team Leader in ensuring that appropriate evaluation mechanisms are in place to make certain that the service delivered to patients is of the highest quality. To use evidence-based practice to develop and maintain high quality patient care and ensure the continuous improvement of the Hospice at Home Service. To lead and participate in auditing the service and ensuring that recommendations from audits lead to effective changes to practice and service delivery. At all times to act as an ambassador to Jersey Hospice Care to patients and their relatives, to colleagues and members of the public. Standards and expectations In addition to the specific duties and responsibilities outlined in this job description, all Jersey Hospice Care employees should be aware of the specific responsibilities towards the following: Jersey Hospice Care operates a no-smoking policy. The post holder should either be a non-smoker or be prepared not to smoke in any charity premises, grounds or vehicles or when on Jersey Hospice Care business outside the office. Adhere to all health and safety and fire regulations and to co-operate with the Charity in maintaining good standards of health and safety. Uphold ethical and professional standards and not behave in a manner that is likely to bring the Charity into disrepute. Promote and sustain a responsibility towards equal opportunities and diversity within the Charity. 5 KEY SKILLS, KNOWLEDGE AND EDUCATION Educations and qualifications Skills and Abilities Knowledge Experience Aptitude, personal characteristics Essential RN Registration Level 1 with relevant post0registration qualification. E.g. ENB 931 or Diploma in Palliative Care or equivalent Willingness to undertake necessary Diploma level modules and / or any additional education and training as required by the role. Communicate with people in a diplomatic and tactful manner Ability to work in a complex and stressful environment and demonstrate appropriate coping mechanisms Ability to quickly identify and solve problems and take action to address issues. Ability to use own initiative. Ability to work in an unsupervised setting. Up to date awareness of nursing care for patients with palliative care needs. Up to date awareness and understanding of specific needs of palliative care patients in the terminal phase. Knowledge and ability to use Microsoft office packages and be able to record and monitor patient care activity. Experience in relevant care setting in palliative care. Experience of working in the community. Evidence of CPD Desirable Degree in Palliative/ Cancer care or equivalent

7 JERSEY HOSPICE CARE BACKGROUND Jersey Hospice Care is a charitable organisation that provides specialist palliative nursing and holistic care, free of charge. The charity is governed by a Council of Trustees and is committed to providing a professional service of the highest standard to all those entrusted to its care. Its aims are to enable patients to live the remainder of their lives to the full, to remain at home if they wish and to die with dignity where they choose. The charity covers a range of services and activities as follows: Specialist Palliative Care Team will liaise with and support generalist primary care teams across Jersey. The SPCT is made up of two parts, Clinical Nurse Specialists and the Hospice at Home service. The team will adopt a multidisciplinary approach to ensure that it provides complex symptom management and holistic care in order to provide patients with the optimum quality of life possible. Day care - patients attending our day care facilities have their nursing/medical needs assessed by the multidisciplinary team of nurses, physios and complementary therapists. It also provides the opportunity for social interaction and a welcome respite for families. In-Patient Unit (IPU) - the IPU comprises twelve single en-suite bedrooms. The IPU is staffed by qualified nurses and auxiliary nurses experienced in pain and symptom management and end of life care. Therapeutic Care - Jersey Hospice Care offers a comprehensive range of therapies and complementary therapies provided by skilled practitioners, who are available in patients homes, through the IPU and day care facilities and the Community Bereavement Service. Community Bereavement Service - the Community Bereavement Service is available to anyone in the community who has suffered a loss, regardless of the nature of the bereavement. This service aims to offer help and support to those who have experienced bereavement and are having difficulty coming to terms with the loss. The service is managed by staff with the help of trained volunteers. Retail - there are two Jersey Hospice Care shops; a shop in St Helier and a shop in St Ouen, both now incorporated into a subsidiary trading company; Jersey Hospice Care Retail Limited. The shops are important sources of fundraising, as well as providing a vital contact with the island community. Fundraising - the Fundraising team is responsible for the many and varied fundraising activities that are crucially important to the financial stability of the charity. This function has now been incorporated into a trading subsidiary of JHC; Jersey Hospice Care Fundraising Limited. Administration and support services - a range of staff are responsible for the administration, accounts management, housekeeping and catering services that are vital to the smooth running of the charity. The administration team is typically the first point of call for those contacting Clarkson House. Volunteers - Jersey Hospice Care depends on the generous support of a large body of volunteers who assist in a wide range of roles across all areas of the charity. These include helping with day care, on In Patient Unit, through the Community Bereavement Service, and with fundraising and gardening etc. Review
