Purpose of the role : DIABETES EDUCATION PATIENT Maintain community education services in the North and West Provide individual and group based education, counseling and assessment for the person with diabetes, including stabilization of blood glucose levels. DIABETES EDUCATION STAFF Act a diabetes resource person/advisor for other healthcare workers and participate in providing educational programmes as required PROFESSIONAL ACCOUNTABILITY Responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with professional development requirements Accurate documentation as per Waitemata DHB Clinical documentation Policy Dietetics, and Waitemata DHB audit protocol Prescriptions are documented as per WDHB policies and meet legal requirements Apply for special authority numbers when needed for nutritional supplements Ensures clinical effectiveness by critically appraising current research and applying it to practice Work with diabetes service across the hospital/community interface for patients who would benefit from support in improving self-management of their diabetes. Work closely with GPs and other community and hospital healthcare providers to achieve this. Coordinates the development, implementation and review of relevant dietetic policies, procedures and documentation in the diabetes service, ensuring a consistent evidence based approach to patient care Work with diabetes services towards improving health outcomes and reducing health inequalities for those with diabetes Provide appropriate and accurate information to health care workers in a variety of settings including within WDHB, Private Hospitals, Rest Homes, IHC facilities, PHOs, NGOs Provide education sessions on nutritional care to up skill and inform health care providers and the inter-professional team members Develop and sustain own knowledge, clinical skills and professional awareness in diabetes management Work autonomously within the Diabetes Team, consults and discusses care plan and patient needs with members of the Diabetes team as appropriate Completes clinical audit and research projects as required to measure patient outcomes. To assist with the training of dietetic and other health discipline students Keeps up-to-date with national/local policy development and change for the management of diabetes and disseminate appropriately within Waitemata DHB Acts as a role model in work and professional development consistently exhibits fair, dependable and ethical conduct. Practice is monitored through peer review and standards maintained through on-going diabetes study, research, journal clubs and practice discussion. Practices within codes of conduct and Waitemata DHB policies. Meet the annual continuing competency requirements for NZ registered dietitians as set out in the HPCA Act by completing the Dietitians New Zealand Continuing Competency annual requirements. Completes the Waitemata DHB Dietitians annual Practice Review requirements Contributes to the development, implementation and evaluation of clinical/cultural guidelines and standards of care in the Diabetes Service Contributes to the evaluation and achievement of key performance indicators for the service Shares knowledge with staff so that they emulate best practice. Service Development To participate in diabetes service planning and primary/secondary care initiatives around detection and prevention and management of diabetes related problems.
