1 JOB TITLE: Clinical Nurse Specialist, Paediatric Diabetes 1. PURPOSE OF POSITION The Nurse Specialist as part of a multidisciplinary team is responsible for providing specialist education and coordination of family-centred care to children and young people aged 0-16 years with Type 1 and/or Type 2 Diabetes Mellitus. The role incorporates an inpatient based education portfolio, ambulatory case coordination and education of key groups within the outpatient and community setting, and the opportunity to become involved in clinical research projects within paediatric diabetes. It also incorporates an organisational aspect for the Paediatric Diabetes service, managing patient databases, and co-ordinating group educational activities. 2. Undertakes or leads specific service or organisational portfolio responsibilities as required. 2. Patient Care Management/Care Coordination Uses advanced nursing knowledge and skills to assess, plan, implement and evaluate patients/ clients/ family / whanau health needs. Provides direct nursing care for patient/clients in specialty focus to achieve best health outcomes. Collaborates and communicates with multidisciplinary team to co-ordinate and negotiate care working across health care settings to achieve best health outcomes. Clinical liaison with adult diabetes service. Facilitates effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs. 3 3. Education and Clinical Teaching 4. Evidence based Practice 5. Continuous Quality Improvement variance to clinical pathway, protocols or best practice guidelines. Educates and provides information to patients/clients/family/whanau to improve knowledge of disease/illness, self management, prevention of complications and promotion of recovery. Accurately documents assessments of patients/clients health status, diagnosis and decisions made regarding interventions and referrals or follow-up. Manages patient database, linking with national databases, and ensuring timely referral to services such as ophthalmology for screening of co-mordbidities. Models expert skills within the clinical practice area. Demonstrates skilled mentoring/coaching, teaching and supervision of nursing staff and other health professionals. Participates in case review and debriefing activities as required. Acts as a nursing resource providing expert advice and education to nursing staff and other health care professionals within scope of speciality practice. Identifies and supports staff education and professional development based on knowledge of service needs and TDHB strategic direction. Organises group-based education sessions with patients and community, such as school visits, Adolescent stepup day and Teacher education nights to assist with knowledge and understanding of diabetes within the community, and for patients enhanced well-being. A commitment to education surrounding paediatric diabetes within the community is key to success of this role. Fosters inquiry, critical thinking and research skill acquisition among the nursing workforce to advance nursing practice and patient/client care. Works to ensure the recommended best practice guidelines/policies are research based and relevant across TDHB. Participates in research in conjunction with Centre of Evidenced Based Nursing Aotearoa (CEBNA) as able. Contributes to and participates in TDHB and national policy development. Demonstrates commitment to quality improvements, risk management and resource utilisation. Reviews and develops clinical standards / protocols and policies and facilitates clinical audits processes. Assists in the implementation of nursing and midwifery practice and models of care appropriate to patient/client population needs. Identifies barriers and solutions to access for patient/client, family or whanau. Assists in the implementation and management of initiatives to address differential access to healthcare services for Maori. Contributes to projects that enhance the clinical implementation of evidence-based practice. Acts to identify and minimise organisational risk. 6.
