1 Position Definition Position: Agreement: Classification: Grade 3 Position reports to: Effective date: February 2015 Approved by: Health Services Union Health Professionals Plenty Valley Community Health Workplace Determination 2009 ( the Agreement ) HACC / Community Services Manager Director Community Services Organisational Context Plenty Valley Community Health (PVCH) provides primary healthcare, social and psychological support services to people of all ages, focusing on the growing outer northern metropolitan area of Melbourne. Our services have a strong focus on treatment, prevention, early identification, rehabilitation and maximisation of independent living skills. PVCH is a major provider of primary healthcare in our region and is committed to establishing a well-integrated primary healthcare service that is accessible to people in the City of Whittlesea and neighbouring metropolitan and rural municipalities. PVCH partners with General Practitioners, Hospitals, the three levels of government, local agencies and community members to achieve an effective universal primary healthcare system. PVCH is a registered Community Health Centre and a Company Limited by Guarantee. It is funded by government grants and other self-managed revenue activities. It is governed by a Board of Directors elected and appointed from the community. Our expectation is that staff will operate as a co-operative team. Our staff will be flexible, multiskilled and able to work with a diverse range of individuals, multidisciplinary groups and community agencies. Our staff will continually update their knowledge to take on new responsibilities and challenges.
2 Purpose and Scope This newly created position will assist in the development of an Adult Community based Speech Pathology service at Plenty Valley Community Health to complement the existing Allied Health services within the organisation. Working in conjunction with the HACC/Community Services Coordinator and Manager the position will identify keys areas of service plan development to provide a clinical service addressing management of adult clients with conditions including Stroke, Acquired brain injury and progressive neurological conditions. Once the service plan is developed and approved the position will commence a clinical service delivery that is high quality and evidence based alongside working in collaboration with HACC/Community Service team members across the PVCH service sites. The position is fixed term until June 2016 with the expectation and desire of becoming permanent as the service needs/demand are fully understood and funding allocations are confirmed. The HACC/Community services team consists of HACC Allied Health, Women s Health and Chronic and Complex care services. The Chronic and complex care service has a strong role in early intervention, treatment and the management of chronic conditions by providing programs that target, in particular, Type 2 diabetes and cardiovascular and respiratory conditions through individual consultation and group based work. The Chronic and Complex Care Team works closely with General Practitioners, specialists, acute and sub-acute services in providing holistic care and chronic disease management. The Chronic and Complex Care Team also provides secondary consultation for individuals identified at risk of developing chronic conditions and will focus on preventing unnecessary hospital presentations and admissions. The team consists of Physiotherapists, Dieticians, Allied Health Assistants, Diabetes Educators, Cardio respiratory Nursing, Project Co ordination, Exercise Physiologists and Endocrinologists. The HACC component of the team consists of Allied Health staff servicing all eligible HACC clients with Services including: Allied Health Assistance, Occupational Therapists, Physiotherapists, Dieticians, Exercise Physiologists, Podiatrists and Planned Activity Groups. The Women s Health service provides preventative screening and education services to the community and consists of a team of Nurses. Services from the team are provided at multiple PVCH sites within the City of Whittlesea catchment. The Speech Pathologist will work collaboratively with other speech pathologists providing adult services in the catchment including Northern Health to ensure referrals, client management process and professional support is developed and maintained. 2 Private & Confidential Plenty Valley Community Health
3 Key Responsibilities and Duties Service delivery: To meet, and strive to exceed, personal and workgroup targets for Key Performance Indicators as set from time to time by PVCH and/or funding bodies in areas such as Safety, Efficiency and Effectiveness, amongst others. Develop Adult Speech Pathology service plan in conjunction with Team Coordinator & Manager Develop appropriate clinical practice procedures in relation to adult speech pathology activities Develop effective referral and service relationships, both formal and informal, with general practitioners, health and welfare agencies, discharge planners from acute hospitals and other relevant speech pathology services. Provide appropriate speech pathology intervention for adult patients with conditions including strokes, acquired brain injury and progressive neurological disease with a client centred focus. Develop Allied Health assistance Speech Pathology work components Support and assist patients with voice therapy/vocal fluency issues and be able to perform language based cognitive assessments. Conduct comprehensive clinical assessments and develop personalised care plans. Records computer based client service delivery and statistical data in timely manner Knowledge and utilisation of appropriate Speech Pathology outcome measures. Link and liaise with Tertiary institutions for Speech Pathology clinical placement development Completes a high standard of written reports to relevant outside organisations and agencies as required Ensure equipment and resources are utilised within the service according to organisation guide and protocols Develop appropriate education, health promotion resources relating to speech pathology services for staff, patients and students. Promote collaboration between clinicians across PVCH service areas and support the further development of inter-professional and across sector partnerships to improve the quality, safety and integration of services. Work collaboratively with the PVCH Senior management team regards service delivery, development and promotion

6 Key Selection Criteria Mandatory: Successful applicants are required to provide certified copies of the mandatory qualifications listed in this section. Tertiary qualification in Speech Pathology and demonstrated sound clinical skills. Eligibility to obtain full membership with Speech Pathology Australia Demonstrated experience in outpatient Speech Pathology rehabilitation particularly with neurologically impaired adult clients Experience supervising staff and students Flexible and adaptable approach to service delivery Current Drivers Licence Well developed written communication skills Demonstrated highly developed interpersonal and advocacy skills Demonstrated effective organisational skills and the capacity to work flexibly. Desirable: Experience in a Community Health or similar relevant setting. Experience of new service development and set up. Conditions of Employment Remuneration: The employee will be paid in accordance with the Award/Agreement. Salary Packaging is available for permanent and fixed term employees, in accordance with prevailing legislation and PVCH Policy. Novated Car Leasing is available for all permanent employees. Location: The employee will be expected to work, either temporarily or permanently, at any site operated by PVCH. Hours of duty: The employee will be required to work daily up to 7.6 hours necessary to meet the Key Responsibilities and duties of this position. 7 Conditions of Employment Pre-employment checks: A Police Check is mandatory. In accordance with PVCH Policy, relevant convictions will be taken into consideration when considering applicants for this position. All employees of PVCH must be either an Australian Citizen, Permanent Resident or have an appropriate Australian visa that will legally enable them to fulfil the obligations of the contract of employment. A Working With Children card (WWC) may be required for this position. If required, failure to obtain and retain a WWC card will make the employee automatically ineligible for employment in this position. Probationary period: Confirmation of new employees to PVCH is subject to a 6 month probationary period, during which time the employee must demonstrate satisfactory capability to perform the Key Responsibilities and Duties. This probationary period is regarded as an extension of the interview process.
