1 Position Title: Social Worker Grade 2 Permanent Position up to 40 hours pw Division: Community Services Reports To: Allied Health Manager Primary Objective: Direct Reports: Grade 1 Social Worker Students To work as an effective interdisciplinary team member providing best practice Social Work Services and interventions within the scope of s integrated services. The Social Worker provides a range services that include psycho-social assessment, discharge planning, counselling, education, referral, consultation and liaison to patients, clients, family members, staff and management of Position Dimension & Decision Making Authority: Key Communication Contacts: Without referral to manager Patient care. 2 Key Accountabilities Key Result Area Major Activities Performance Measures: 1. Quality Care Plan, implement and evaluate Social Work interventions for allocated client caseload. Undertake comprehensive psychosocial assessment in a variety of settings. Coordinate CAH s Restorative Care Program Provide care coordination, counselling, advocacy, mediation, referral and support for individuals, families and groups in the inpatient, outpatient and community setting. To contribute to case conferences and clinical discussions, ensuring that psychosocial factors are considered in treatment, rehabilitation and discharge planning. In conjunction with s Palliative Care Service ensure a grief and bereavement service is provided to clients and families Provide consultation to CAH staff in relation to aged care. Coordinate with the Barwon Regional Aged Care Assessment Service in Acute services. Participate and contribute to the multidisciplinary function of the multidisciplinary teams (Acute, Palliative care and Community) Tasks are at least 75% patient related and Client treatment goals achieved. File audits reflect adherence to documented standards. Evaluation results documented. Achievement of monthly Social Work targets PJB audits reflect appropriate time utilization. 3 2. Responsibility and Leadership. 25% non-patient related, as per the National Consortium of Allied Health. Deliver best practice Social Work intervention to clients in accordance with the Australian Social Work Code of Ethics Provide resources, information and coordinate referrals for staff of the. Participate in the development and training of a CAH critical incident response team and provide debriefing as a member of the team where appropriate Participate in the orientation of new Colac Area Health staff. Participate in advanced care planning with clients Demonstrate flexibility/responsiveness within professional practice/duties/roles in order to meet the organisational goals/priorities. Responsible for clinical area as agreed with discipline coordinator Responsible for coordinating social work student placements at C.A.H. Participate in the education of students from other disciplines. Identifies issues, considers solutions and presents these to manager. Participates in problem solving and decision making with Documented guidelines adhered to. Student exit evaluations indicate student satisfaction and leaning goals met. Information Management Client documentation completed to adhere to documented guidelines. Statistics are recorded as per guidelines and within required timeframes. Retrospective file audits reflect compliance with documented protocols. PJB audits reflect requirements are met. 4. Communication and Relationship Management Communication is effective and appropriate in regard to sharing information and forming relationships with clients. Ability to impart knowledge e.g. students, clients, families and community. Modelling of effective interdisciplinary practice. 5. Organisational Quality Improvement Participate in relevant organisational quality accreditation processes. Participation in quality improvement activities Identify and document benchmarked clinical best practice for area of service provision. To develop and evaluate models of care appropriate to CAH Complies with all CAH mandatory training requirements Demonstrates effective communication which reflects CAH core values. Demonstrated participation in accreditation processes Evaluate outcomes for clients in allocated caseload. Outcome measure evaluation results documented, and indicated changes implemented. Adherence to documented clinical guidelines which indicate clinical best practice. Qualifications, Experience and Personal Attributes Bachelor of Social Work degree and eligibility for AASW membership Previous experience in a hospital/health care setting Proficient clinical skills in psychosocial assessment including risk assessment, complex care planning, discharge planning, Social Work interventions and communication Knowledge and understanding of the Social Work role in acute care, rehabilitation, palliative care, aged care, community health and outreach services Excellent IT skills in word processing and presentations Ability to work both autonomously and within multidisciplinary teams Demonstrated understanding of theoretical frameworks that inform Social Work practice. Demonstrated awareness and understanding of community resources and systems.
