1 Northern Ireland Hospice Job Description Post: Location: Reports to: Responsible to: Hospice Dementia Nurse Specialist (3 year fixed term contract. NI Hospice Community Services (initially based at Jennymount site) Clinical Services Manager Community Director of Nursing and Patient Services Background This is a 3 year post which will concentrate on piloting the provision of a community dementia palliative care service in the North and West locality of the Belfast Health and Social Care Trust. The post holder will work in collaboration and partnership with statutory and voluntary services, people with dementia and their family and carers. Role Working as a Hospice Dementia Nurse Specialist (HDNS) the post holder will be responsible for piloting the introduction and delivery of a specialist palliative care service to patients, carers and healthcare professionals in the North and West locality of the Belfast Health and Social Care Trust. The HDNS will develop professional collaborations with existing primary and secondary care service providers and with other voluntary and community representative groups to ensure the palliative and end of life care needs of people living with dementia are addressed. The overall aim of the service is to provide a person centred community palliative care service which ensures people living with dementia in the community are supported and have access to palliative care when they are approaching the end of life. The post holder will contribute to the overall achievement of the project by being an active member of the project delivery team, working closely with the Hospice Education and Research Department and being involved in the research and evaluation of the project. Leadership and Management Actively demonstrates leadership throughout the project by being an active member of the project delivery team ensuring the objectives of the project are achieved. Be aware of, understand and use the strategic drivers to inform and develop services for people with dementia. Work in collaboration with the Day Hospice Manager in developing and delivering a comprehensive, multidisciplinary day hospice programme.
2 Work in collaboration with the Hospice at Home Coordinator and the Hospice at Home nurses. Work in collaboration with the Volunteer Coordinator in recruiting and training volunteers. Develop professional working relationships with key providers within statutory primary and secondary care service providers and with other voluntary and community representative groups Clinical Practice Identify the stage of the disease and advise, support and enable families and healthcare professionals to deliver an appropriate range of interventions to meet the multiple and often complex needs of dementia patients. Address the multiple and often complex needs of the patient and their families by providing a specialist holistic nursing assessment, discussing and planning interventions and support the evaluation of their care. Actively manage a caseload of patients in partnership with patients, carers and other healthcare professionals. Enable and support the initiation of referrals to appropriate generalist and/or specialist services. Advise, support and enable healthcare professionals to address and manage the needs of the family through a holistic assessment. Introduce, enable and support the development of Advanced Care Plans. Develop, agree and implement policies, procedures and guidelines for the delivery of an effective and efficient service for patients living with dementia. In partnership with the project steering and delivery team, design and deliver a day hospice service and a nurse led outpatient clinic (one day per week). Promote clear and appropriate communication between patients, carers, and the various health care professionals involved in the patient s care. Research and Audit Working in partnership with the Dementia Research Collaborative, The Atlantic Philanthropies and the Hospice Education and Research Department actively participate in the planning and implementation of the evaluation research methodology which captures the effectiveness and outcomes for the project. Monitors the impact of the project by leading clinical audit which demonstrates quality in clinical practice.
3 Disseminate relevant research findings in the promotion of evidence based practice. Develop and implement a programme of continuous quality improvement within palliative care dementia services. Use and promote reflective practice using case studies to learn from experiences gained throughout the project. Education and Professional Development Develop an in-depth knowledge of the aetiology and management of dementia by completing the Essentials of Dementia Care and working towards the achievement of a Post Graduate Diploma in MD Dementia Care. Deliver training and education relevant to the project in care homes and other care settings to improve the delivery of holistic care to patients and families dealing with dementia. Identify and provide formal and informal training and development opportunities for staff in all care settings, such as shadowing and placements in Hospice services. Act as a resource for all healthcare professionals in promoting best practice in dementia care. Working in partnership with statutory and voluntary sector service providers provide supportive education to those working in dementia and end of life care. Participate in and or lead conferences, seminars and workshops, to contribute to higher level educational programmes in palliative dementia care. Encourage involvement of the patient and their carers in decision making to improve the understanding of their condition and management, identifying their needs, providing education and support. Recognise own professional educational limitations and set learning objectives to develop competence in specific areas of practice. Undertake an annual personal development review after one year in post. Ensure mandatory training is up to date. Participate in clinical supervision and act as a supervisor for other staff. Network with other Dementia Nurse Specialists and services locally, regionally and nationally. 5 Northern Ireland Hospice Job Specification Post: Department: Band: Hours: Hospice Dementia Nurse Specialist NI Hospice Community Team Based on Band 7 ( 30,088-39,665 per annum) hrs per week Essential Criteria: Registered Nurse on the NMC Register, Adult Five years post registration experience, excluding post basic courses, or part time equivalent. Specialist Practice Degree in Palliative Care or a qualification at degree/post graduate level in dementia care. (Candidates must also indicate their willingness to undertake specific modules relevant to the role). Three years recent full time (or part time equivalent) experience, practising as a specialist nurse in palliative care or oncology care at Band 6 or above or 3 years recent full-time (or part-time equivalent) experience, practising as a nurse within dementia care at Band 6 or above or 3 years experience working in an environment that delivers palliative or dementia care at Band 6 or above. Experience of working in a multi-disciplinary environment with the ability to develop and foster positive working relationships. Awareness and understanding of the strategic drivers in delivering palliative care to people with dementia. Excellent communication and negotiating skills. Able to work unsupervised and demonstrate effective clinical decision making. Experience of teaching. Evidence of the ability to innovate and manage change. Proven ability to participate in audit, research and quality initiatives.
6 Computer literate. A valid driving licence with access to a car to enable the requirements of the post to be fulfilled.
