2 New Bridge Group Learning Together, Learning for All, Learning for Life Job Description Job Title: Pay Grade / Scale / Range: Benefits & Perks: Working hours: Location: Key Worker 23,166-24,717 per annum. NJC Scale 6. Scp Flexible working, occupational pension scheme 36 hours 40 minutes per week. Full year. Some evening and the occasional weekend working. Initially based at New Bridge Horizons Medtia Square. Employees could be required to work within any site of the New Bridge Group Special circumstances: Flexible activity centred rotas Staff responsible to: Head of Site Staff responsible for: None Accountable to: Probationary period: Executive Principal 26 weeks for successful candidates new to the organisation Our organisation is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and post holders to share this commitment. For child protection purposes an enhanced disclosure will be required for this post. Job Purpose To support disabled young people in accessing social and community activities through a diverse setting. To be responsible for implementing care plans, organising multi-agency meetings and individual reviews.

3 General Tasks Support for the Young People 1. Establish rapport and respectful, trusting relationships with the young people, acting as a role model and setting high expectations. 2. Promote inclusion and acceptance of all. 3. Assess the needs of young people and use detailed knowledge of personal care needs to support them. 4. Ensure safety, welfare and personal hygiene. (Basic First aid to be included where appropriate). 5. Promote self-esteem and independence and employ strategies to recognise and reward achievement of self-reliance. 6. Provide specific feedback in discussion with young people on their progress and achievement, in line with school policy. Support for the Organisation 1. Comply with and assist with the development of policies and procedures relating to child protection, health & safety, confidentiality and data protection, reporting all concerns to an appropriate person. 2. Contribute to the overall work and ethos of the organisation. 3. Establish constructive relationships and communicate with other agencies and professionals, in liaison with staff, to support the achievement and progress of young people. 4. Contribute to the development of appropriate multi-agency approaches to supporting young people. 5. Attend and participate in meetings as required. 6. Improve one s own practice through training, observation, evaluation and discussion with colleagues. 7. Recognise one s own strengths and areas of expertise and use these to support others. 8. Contribute to the identification and execution of appropriate out-of-centre activities, which consolidate and extend the centre s activities. Tasks Specific to the Role 1. Undertake personal care tasks in a dignified and respectful manner and to a high standard. 2. Maintain contact with carers, families and professionals where appropriate. Liaise sensitively and effectively with parents and carers as agreed with management within your role/responsibilities and participate in feedback sessions/meetings with parents under direction. 3. Assist the young people as their identified Key Worker to maximize their potential and achieve their individual agreed targets, promoting their self-esteem and enabling them to become as independent as possible, and setting high expectations. 4. To observe confidentiality with regards to the young people. 5. To be aware of and comply with the policies and procedures of New Bridge Horizons. 6. Establish rapport and respectful, trusting relationships with young people, being aware of and responding appropriately to individual needs. 7. Support the young people consistently whilst recognising and responding to their individual needs as set out in their personal care plan, through 1:1 work and regular reviews. Monitor and evaluate young people s responses to activities through observation and planned recording of achievement against pre-determined objectives in their care plan. 8. To make accurate handwritten reports and use the centre s electronic database to enter information and keep records up to date.

4 9. Promote inclusion and acceptance of all young people by encouraging them to interact with each other and to engage in activities. 10. Ensure that medication is administered and recorded as instructed and in accordance with the organisation s guidelines after training. 11. Ensure the safety, welfare and personal hygiene of the young people. (Basic First Aid to be included where appropriate) 12. Promote self-esteem and independence and employ strategies to recognise and reward achievement of self-reliance. 13. Provide specific feedback in discussion with young people, families and multi agencies on their progress and achievement, in line with New Bridge Horizons policy. 14. Promote positive values and attitudes. Deal promptly with conflicts and incidents whilst encouraging young people to take responsibility for their own behaviour, in line with the centre s established policies. 15. Work under guidance to plan and deliver activities, evaluating and adjusting work /plans as appropriate to meet pre-determined objectives, and completing related risk assessments. 16. Provide cover for activities already planned within the centre as required. 17. Implement agreed activities/ programmes with advice from management, adjusting activities according to young people s learning styles and individual needs. 18. Support the effective use of ICT in activities and develop young people s competence and independence in its use. 19. Determine the need for, prepare and maintain general and specialist equipment and resources including filing, record keeping, collecting money, checking of deliveries, placing goods in stock and maintaining records of stock. 20. Work as a member of the team and on own initiative and contribute to the development of the service, in line with the ethics of New Bridge Horizons. 21. Improve one s own practice through training, observation, evaluation and discussion with colleagues. Recognise one s own strengths and areas of expertise and use these to advise and support others. 22. Organise/lead visits, trips and activities in and around the community as required, including completing risk assessments. Other Duties This job description is not intended to be all-inclusive and the job holder may perform other related duties as directed by the Executive Principal to meet the needs of the organisation. Equipment, Tools, Physical or Special Requirements 1. Ability to maintain regular, punctual attendance consistent with the organisation s expectations and policies. 2. Cross site travel may be required. 3. Adoption of professional standards of behaviour and appearance at all times in line with the protocols of the organisation.

Experience of arranging and participating in meetings/reviews General Skills and Abilities 1. Able to work with various ICT packages such as Microsoft Office 2.
