1 CURRENT COLLEGE STUDENT SCHOLARSHIP 2015 AWARD REGULATIONS ELIGIBILITY/LIMITATIONS FOR SCHOLARSHIP AWARD Current cllege student applicant and spnsr must meet the fllwing eligibility requirements: 1. Applicant must reside in Hawaii. 2. Applicant must be a dependent. 3. The spnsr f the applicant must meet ONE f the fllwing requirements: Active duty U.S. Military member currently assigned in Hawaii. Active duty U.S. Military member n a remte assignment frm Hawaii. Retired U.S. Military member residing in Hawaii. Full time Hawaii Natinal Guard member r U.S. Military Reserve member residing in Hawaii. Deceased U.S. Military member with applicant residing in Hawaii. The fllwing applicants are nt eligible fr an award: 1. Anyne wh has previusly received an HOSC schlarship award. 2. Anyne n a full schlarship. 3. Selectin cmmittee members r family members f selectin cmmittee members. 4. Schlarship crdinatr/c-crdinatr r family members f the schlarship crdinatr/c-crdinatr. QUALIFICATIONS The applicant must meet ALL f the fllwing qualificatins: 1. Have a high schl diplma, hme schl affiliated diplma r GED if entering his/her first year f higher educatin in academic year at a cllege r university accredited thrugh an agency recgnized by the U.S. Department f Educatin and the Cuncil fr Higher Educatin Accreditatin. OR Submit a letter f academic gd standing frm a cllege r university accredited thrugh an agency recgnized by the U.S. Department f Educatin and the Cuncil fr Higher Educatin Accreditatin which he/she is currently attending. 2. Submit a cmpleted applicatin t the Schlarship Cmmittee f the Hickam Officers Spuses Club. CRITERIA FOR AWARD Schlarship award shall be based upn the fllwing criteria: 1. Educatinal Infrmatin 2. Emplyment Infrmatin 3. Vlunteer Service and Club/Activities 4. Leadership Psitins 5. Hnrs/Awards 6. Persnal Essay 7. Letters f Recmmendatin Page 1 f 7

2 APPLICATION INSTRUCTIONS The HOSC and the Schlarship Cmmittee are nt respnsible fr lst r misdirected applicatins. Please make sure yur applicatin is cmplete. Incmplete applicatins, r thse pstmarked and mailed after the deadline will nt be cnsidered. Applicatin must be pstmarked and mailed by March 9, Applicatins must include ALL f the fllwing: 1. Cpy f spnsr s military rders/retirement paperwrk shwing applicant s dependency status 2. Prf f residency (example: cpy f utility bill with hme address) 3. Officially sealed transcript frm current cllege r last schl attended 4. 2 COPIES OF EACH - 2 Letters f recmmendatin frm teacher, cunselr, emplyer, cmmunity member r service prfessinal, etc. (family/friend references nt accepted) 5. 2 COPIES Applicant Infrmatin Sheet (Attached) 6. 2 COPIES Activity Infrmatin Sheet (Attached) 7. 2 COPIES Applicant Essay. Type a ne page essay n an 8 ½ x 11 single-sided, single spaced page with ne inch margins using Times New Rman 12 pint fnt. Essays exceeding mre than ne page will nt be accepted. BOX Hnlulu, HI NUMBER AND AMOUNT OF AWARDS The HOSC Gverning Bard will apprve the ttal number f schlarships and ttal amunt awarded. SELECTION COMMITTEE The selectin cmmittee shall be cmprised f military and cmmunity members chsen by the HOSC Schlarship Cmmittee. Decisins f the selectin cmmittee are final. Page 2 f 7

3 AWARD RECIPIENT RESPONSIBILITIES 1. The schlarship must be used by the recipient at an accredited 2-4 year cllege, university, vcatinal, r technical schl during the academic year. In additin, schlarships may be used at a schl pending accreditatin by an rganizatin apprved by the US Department f Educatin. 2. The schlarship may be used fr the payment f tuitin, fees, and/r rm and bard charged by the Finance r Business Office f the schl. 3. Recipients must submit t the Schlarship Crdinatr a Use f Awards Agreement and Statement f Intent frm by September 30, Use f Awards Agreements and Statement f Intent frms will be given t each recipient fllwing the annuncement f winners. 4. Verificatin f enrllment letter r frm the educatinal institutin registrar must be received by the Schlarship Crdinatr by September 30, Electrnic verificatin thrugh Natinal Student Clearinghuse will als be accepted. Any fee assciated with this service is the respnsibility f the recipient. 5. The HOSC Welfare Treasurer shall issue a check in the full amunt f the schlarship payable t the educatinal institutin that has verified the recipient s enrllment. The Schlarship Crdinatr shall frward this check within ne mnth f receiving all necessary infrmatin frm the recipient and the educatinal institutin. 6. Any schlarships nt claimed by September 30, 2015 will be frfeited. 7.
