During the due diligence prcess it was nted that CHF patients accunted fr the mst hspital readmissins within 30 days f discharge. A team was selected t utilize the hspital s HIT t crral the high CHF readmissin rates (the team included HIM prfessinals). Prject/Grant Case Scenari Thrugh literature research and team cnversatins it was decided t use the hspital s EHR patient prtal t help facilitate clinician/patient interactin prir t admissin.
