2 Fall Prtectin Prgram Envirnmental Health and Safety Fall Prtectin Prgram 1) Purpse and Scpe The University is dedicated t the prtectin f emplyees ptentially expsed t the hazards f wrking frm heights. Fall prtectin is required when there is the ptential f falling fur r mre feet t a lwer level r when wrking abve hazardus equipment. 2) Standards, Regulatins and References a) CCR, Title 8, Subchapter 4. Standard Railings Sectin Design and Cnstructin f Railings. 3) Rles and Respnsibilities a) The University The University is cmmitted t and has a duty t prvide a safe and healthful wrk envirnment fr emplyees and cntractrs frm the hazards f wrking frm heights. b) Envirnmental Health and Safety Sectin Envirnmental Health and Safety will i) Establish, implement and maintain the Fall Prtectin Prgram that is designed t eliminate r minimize emplyee expsure t fall hazards when wrking frm heights. ii) Perfrm an emplyee expsure assessment t identify the emplyees wh are at risk with the cllabratin f each department s management and dcument the findings. iii) Develp and implement campus-wide training requirements and materials. Emplyee infrmatin and training are prvided at the time f initial assignment and annually thereafter. iv) Maintain a recrd f training given t emplyees. Recrds will be maintained fr three years. v) Maintain a recrd f Persnal Fall Arrest System inspectins fr three years. vi) Review and apprve the Fall Prtectin Plans as required when cnventinal methds f fall prtectin are nt feasible. Maintain a recrd f cmpleted Fall Prtectin Plans fr three years. vii) Review and update the Fall Prtectin Prgram annually. c) Department Management Each affected Department will SJSU_Fall Prtectin Prgram_ dcx 2 f 19

3 Fall Prtectin Prgram Envirnmental Health and Safety i) Cllabrate with the Envirnmental Health and Safety Sectin in the emplyee expsure assessment prcess t identify emplyees wh wrk frm heights. ii) Ensure that affected emplyees received prper training prir t beginning wrk that expses them t ptential fall hazards. iii) Develp and enfrce wrk practices and methds designed t cntrl r eliminate the risk f wrking frm heights. iv) Prvide the necessary wrk implements, such as persnal fall arrest systems, tls, glves, and persnal prtective equipment t emplyees that will enable emplyees t perfrm their wrk safely. v) Issue Fall Prtectin Plans where cnventinal fall prtectin methds are nt feasible fr wrking frm heights. vi) Crdinate with Cntractrs that perfrm wrk nsite t ensure that they are prvided SJSU Fall Prtectin prcedures. d) Emplyees Every emplyee wh is at risk f wrking frm heights will i) Receive training. ii) Be prvided with the necessary wrk implements, such as persnal fall arrest systems, tls, glves, and persnal prtective equipment, and t use them t perfrm their jb safely. iii) Fllw the prescribed wrk practices and methds designed t cntrl r eliminate fall hazards. iv) Inspect fall prtectin equipment prir t usage. v) Inspect ladder and aerial lift vehicles prir t usage. vi) Reprt unsafe cnditins and wrk practices t their supervisr. 4) Cntractr Services Cntractrs will crdinate with apprpriate SJSU department management t ensure that the cntractr is prvided with SJSU Fall Prtectin prcedures. 5) Prgram Audit Envirnmental Health and Safety will perfrm an annual prgram audit and make imprvements t the Fall Prtectin Prgram as cnditins change. 5 Fall Prtectin Prgram Envirnmental Health and Safety The Fall Prtectin Prgram The University is cmmitted t and has a duty t prvide a safe and healthful wrk envirnment fr all emplyees and cntractrs frm the hazards f wrking frm heights. 1) The Fall Prtectin Prgram. The Fall Prtectin Prgram is designed t eliminate r minimize the hazards f wrking frm heights and includes the fllwing key elements: a) Determinatin f emplyee expsure b) Implementatin f cntrl methds including: i) Use f Cnventinal Fall Prtectin Methds. ii) Fall Prtectin Plans. c) Emplyee Training. d) Recrdkeeping e) Safe Wrk Practices 2) Expsure Determinatins An expsure determinatin was made f staff psitins by the Envirnmental Health and Safety Sectin. 6 Fall Prtectin Prgram Envirnmental Health and Safety iii) Vehicle munted wrk platfrms, such as Aerial Lifts. iv) Use f Persnal Fall Arrest Systems. v) Safety Net Systems. b) Fall Prtectin Plans i) A Fall Prtectin Plan must be created when cnventinal fall prtectin is impractical r creates a greater hazard. The fall prtectin plan will dcument the reasns why the use f cnventinal fall prtectin systems is nt feasible r why their use wuld create a greater hazard. ii) The Fall Prtectin Plan will identify each lcatin where cnventinal fall prtectin methds cannt be used. These lcatins will then be classified as Cntrlled Access Znes. 4) Emplyee Training All emplyees wh wrk frm heights greater than 7 ½ feet receive initial training at the time f assignment and annual refresher training. a) The key training elements include: i) Classrm instructins and practical training which cnsists f pre-peratinal inspectins and usage f the fall arrest and fall restraint equipment. ii) Safe use f persnal fall arrest and restraint systems which include limitatins f the equipment, free fall distance, methds f use, prper anchring, and hw t dn the equipment. iii) Inspectin and strage f the fall arrest and restraint systems. iv) Rf fall prtectin prcedures. v) Ladder safety. vi) Aerial lift safety. vii) Fall preventin and safety prcedures when wrking at elevated heights. viii) Rescue prcedures. ix) The University s Fall Prtectin Prgram and prcedures. b) Training is crdinated by San Jsé State University, Envirnmental Health and Safety. 5) Recrdkeeping a) Training Recrds i) Recrds are retained fr each emplyee upn cmpletin f training. These dcuments will be kept fr three years by Envirnmental Health and Safety, Industrial Studies, Rm 134 B. ii) The training recrds include: (1) The dates f the training sessins. (2) The cntents r a summary f the training sessins. (3) The names and qualificatins f persns cnducting the training. (4) The names and jb titles f all persns attending the training sessins. Such requests shuld be addressed t San Jsé State University, Envirnmental Health and Safety. b) Fall Prtectin Plan Recrds Cmpleted SJSU Fall Prtectin Plans will be retained fr three years by Envirnmental Health and Safety. c) Persnal Fall Arrest Inspectin Recrds. Cmpleted SJSU PFAS inspectin recrds will be retained fr three years by Envirnmental Health and Safety. 6) Persnal Fall Prtectin Equipment and Safe Wrk Practices. a) Persnal Fall Arrest Systems are wrn by emplyees when wrk is perfrmed at elevatins exceeding 15 feet abve grund, water surface, r flr level belw and where temprary guardrail prtectin is impracticable. b) Persnal Fall Arrest Systems, when stpping a fall, must: i) Be rigged such that an emplyee can neither free fall mre than 6 feet, nr cntact any lwer level, and, where practicable, the anchr end f the lanyard shall be secured at a level nt lwer than the emplyee's waist; ii) Bring an emplyee t a cmplete stp and limit maximum deceleratin distance an emplyee travels t 3.5 feet. iii) Anchrages used fr attachment f Persnal Fall Arrest equipment must be independent f any anchrage being used t supprt r suspend platfrms and capable f supprting at least 5,000 punds per emplyee attached. iv) Lanyards and vertical lifelines shall have a minimum breaking strength f 5,000 punds. v) Bdy belts shall nt be used as part f a Persnal Fall Arrest System. c) Persnal Fall Restraint Systems are wrn by thse emplyees whse wrk expses them t falling in excess f 7 1/2 feet frm the perimeter f a structure, unprtected sides and edges, leading edges, thrugh shaft ways and penings, slped rf surfaces steeper than 7:12, r ther slped surfaces steeper than 40 degrees. i) Anchrage pints used fr fall restraint must be capable f supprting 4 times the intended lad. ii) If an emplyee's duties require hrizntal mvement, rigging must be prvided s that the attached lanyard will slide alng with the emplyee. d) Prmpt rescue f emplyees in the event f a fall is necessary when emplyees are nt able t rescue themselves. e) Each persnal fall arrest system will be inspected nt less than twice annually in accrdance with the manufacturer's recmmendatins. The date f each inspectin is dcumented. 7) Persnal Fall Prtectin Equipment Inspectin a) Fall prtectin system cmpnents must be stred away frm crrsive materials, ils and slvents, misture, heat, r any ther substance that may cause damage. 8 Fall Prtectin Prgram Envirnmental Health and Safety b) All persnal fall prtectin equipment (fall restrain/fall arrest equipment-lifelines, lanyards, carabineers, rpe grabs, harnesses) are inspected at least semi-annually and by the user prir t usage. c) Equipment shall be examined fr signs f defect and physical damage. All hardware shall be examined fr nicks, cracks, crrsin r distrtin. d) Any equipment shwing sign f defect, deteriratin r alteratin must nt be used and must be discarded immediately. e) Only the equipment manufacturer r supplier can repair r recertify the equipment. 8) Equipment Inspectin Guidelines: a) Full Bdy Harnesses i) Inspect webbing and stitches fr sign f defect and physical damage (cuts, excess wear, tears, knts, brittle spts, hles, melted spts, mld, undue stretching, pulled rivets, and lse, brken, r cut threads). ii) Inspect rivets, grmmets, D-ring, and buckles fr damage; make sure each cnnectin cmpletely clses and lcks. iii) Lk fr the Fall Indicatr r tattle tale webbing that will expse bright red r ther clred fibers t alert the user t significant wear. iv) Harness subject t a fall must be discarded. b) Lanyards and Lifelines i) Design fr use by ne persn at a time. ii) Inspect duble lcking snap hk, impact indicatr fld and threads fr physical damage. iii) Fr retractable lanyard, test lanyard retractin and braking mechanism. (1) Pull ut several feet f lanyard and allw it t retract back int the unit; it shuld retract. (2) Grasp abve the impact indicatr and give a sharp pull; it shuld stp abruptly. (3) Lk fr the Lad Impact Indicatr. Discard the lanyard if the shck absrber r rip stitch has extended/unflded r has arrested a fall. c) Hw t Wear a Full Bdy Harness: i) Hld the harness by the back D-ring and shake t allw all the straps t fall in place. ii) Unbuckle the leg straps, if necessary. iii) Slip n the shulder straps. The D-ring shuld be lcated in the middle f the back between the shulder blades. iv) Cnnect legs straps. v) Tighten all buckles s the harness fits snugly, but allws full range f mtin. vi) The chest strap must be in the mid-chest range 6 t 8 inches belw the trachea but nt belw the sternum. vii) Check all buckles t ensure they re prperly cnnected. 9 Fall Prtectin Prgram Envirnmental Health and Safety viii) Check that all straps are snug and free f twists. ix) Hang the harness by the back D-ring when nt in use. 9) Rescue Prcedures a) A rescue plan must be implemented when using a fall arrest r fall restraint system. The plan must be specific t the wrk lcatin and jb being perfrmed. b) If utside assistance is required (i.e. lcal fire department), verificatin and/r prir arrangement must be made t ensure rescuing capability and availability f equipment. c) All necessary equipment shall be readily available and inspected fr prper peratins befre starting the wrk t ensure prmpt rescue. d) In the event f a fall, the emplyee must be rescued immediately t prevent suspensin trauma. e) Fr tasks where an emplyee must apprach within 6 feet frm an unprtected rf s edge, anther emplyee must be present at all times t ntify the prper individuals and activate the rescue plan in the event f a fall. f) Anther emplyee is nt required t be present at all times fr aerial lift peratin (articulating bm lift r cherry picker), if the task perfrmed is nt in an islated area. Hwever, in the event f a fall, the emplyee might nt be able t summn fr assistance. Anther individual must be available at all times t mnitr the emplyee s status and summn/prvide prmpt rescue if a fall ccurs. g) In the event f a fall and the individual is nt able t perfrm a self-rescue: i) If pssible and if it s SAFE t d s, lwer the individual t grund level by SLOWLY and CAUTIOUSLY lwering the aerial lift. ii) Elevate anther emplyee t rescue the falling individual. h) If utside assistant (i.e. lcal fire department) is needed, cntact University Plice Department (UPD) immediately. Dial 911 frm any campus phne r (408) if using a cell phne. 10) Aerial Lift Fall Prtectin Prcedures. Safe perating rules must be fllwed t prevent falls and the aerial lift frm tipping ver during peratin. Cmply with the fllwing prcedures at all time: a) Persnal fall arrest equipment must be used during the peratin f aerial bm lifts. b) All articulating aerial bm lift must cntain an engineered anchr pints with a minimum lad capacity f 5,000 lbs. per persn. c) Befre perating lift, cmplete a pre-peratin check, which includes the fllwing: d) If there is mre than ne emplyees wrking n the prject, ensure that all emplyees invlved have received fall prtectin and aerial lift training. e) Inspect persnal fall arrest equipment and aerial lift prir t using. 10 Fall Prtectin Prgram Envirnmental Health and Safety f) Ensure fall arrest equipment is prperly dnned and lanyard is cnnected t designated anchr pint n the lift. g) Ensure that the gate n the lift platfrm is clsed and secured. h) Ensure all peple in the immediate area are clear. i) Befre elevating the lift basket, make sure the lift is n a level surface. D nt raise the basket int the air when n a grade r slpe. j) Raise and lwer the lift smthly and with cautin. k) While in mtin, d nt place any bdy parts utside the lift s basket. Stand with bth feet firmly n the basket s flr. D nt get ut f the lift s basket while still elevated. l) Never sit, climb r stand n the basket s guardrails r use planks, ladders r ther devices t gain elevatin. m) Make a visual check f all surrundings abve and belw the lift. Drive/Raise the lift nly in the directin f clear, unbstructed view. n) Be alert t keep clear f verhead hazards such as light fixtures, electrical wires, and telephne line. ) Cnstantly mnitr activity f ther peple and equipment and maintain a safe perating distance. p) Maintain a safe distance frm bstacles, debris, drp-ffs, hles, and depressins. q) When travelling, lwer the lift basket t the travelling psitin. 11) Prtable Ladders a) Any ladder that shws sign f significant defect r damage must nt be used and be immediately tagged ut with the fllwing wrds: Dangerus-D Nt Use. Ladders that are damaged beynd repair must be discarded immediately. b) Befre using the ladder, perfrm a Pre-wrk Check by inspecting the fllwing: i) All parts and fittings n the ladder are secured. ii) Nn-slip surfaces are in place n the ladder rungs. iii) Gripping safety feet are in place and secured n the ladder. iv) The fting f the ladder is secured n a firm, level, and nn-skid surface and the tp f the ladder is placed against a slid, statinary bject. v) The ladder rungs (and emplyee s hands and shes) are nt greasy, muddy r therwise slippery. vi) Dr penings near the ladder are fastened pen, lcked r guarded t prevent the dr frm pening int the ladder. vii) Wrk area is sectined ff, if wrking near passageway r driveway. 11 Fall Prtectin Prgram Envirnmental Health and Safety c) Always use bth hands and face the ladder when ascending/descending. d) D nt ver-reach r repsitin the ladder while ccupied. e) Use the 4:1 rule when setting up straight and extensin ladders; fr every 4 feet f ladder length, the base f the ladder shuld be 1 ft away frm the base f the wall r supprting structure. f) When using ladder fr rf access, the tp f the ladder must extend at least 3 feet beynd the pint f supprt. 12) Rf Fall Prtectin Prcedures a) Access t a rf is restricted t trained and authrized persnnel. b) Guardrails shuld be installed at lcatins where there is rutine need fr emplyees t be within 6 feet frm an unprtected rf s edge (rutine need means mre than 4 times a year). The guardrails shuld extend at least 6 feet beynd the areas where the emplyees are wrking. c) Fr wrk alng the rf s perimeter, evaluate whether the tasks can be dne safely using an aerial lift. If the tasks cannt be perfrmed safely using an aerial lift, fall restraint can be used. d) Due t the ptential fr a swing impact injury against the side f the building and the difficulty f rescue, fall restraint rather than fall arrest must be used when wrking within 6 feet frm an unprtected rf s edge. e) Fr tasks where an emplyee must apprach within 6 feet frm an unprtected rf s edge, anther emplyee must be present at all times t ntify the prper individuals in the event f a fall. f) If wrks must be perfrmed during nighttime r when visibility is pr (i.e. fg), sufficient lighting must be prvided. g) Due t the ptential fall hazard when wrking alng the rf s edge/perimeter, an assessment shall be made t determine whether a safety mnitr is needed. A safety mnitr can be used in cnjunctin with a fall restraint system. The safety mnitr will: i) Be knwledgeable in setting up the fall restraint system. ii) Warn emplyee when it appears that the emplyee is unaware f a hazard r acting in an unsafe manner. iii) Be n the same walking/wrking surface and within visual sighting distance as the mnitred emplyee. iv) Be clse enugh t rally cmmunicate with the mnitred emplyee. v) Nt have ther respnsibilities which culd take their attentin frm the mnitring functin. h) Fall prtectin is required when appraching within 6 feet f a skylight. Fall prtectin can be prvided by ne f the fllwing methds: skylight screens, guardrails, cvers, r persnal fall SJSU_Fall Prtectin Prgram_ dcx 11 f 19

Exceptins: the manufacturer r a structural engineer has certified that the skylights will supprt anticipated lads r when the wrk is f shrt duratin and limited expsure such as measuring and rf inspectin, prvided that adequate risk cntrl is recgnized and maintained. An emplyee shuld nt stand n a skylight. i) Fall prtectin is required when wrking n rf with slpe greater than 7:12. 13) Flr and Rf Openings The fllwing safeguards must be in place whenever pssible t avid situatins where persnal fall prtectin equipment is necessary: a) Wrk a minimum f 6 feet frm the rf s edge. b) Never leave unprtected flr/rf penings unattended. c) Cver any flr r rf penings greater than 12 inches in the least hrizntal dimensin with a prtective cver. i) Prtective cvers will be able t withstand a weight f 400 punds r twice the ptential weight f the emplyees, equipment, r material impsed upn the cver. ii) Prtective cvers must nt prject mre than 1 inch abve the flr level and all edges are chamfered t an angle with the hrizntal f nt mre than 30 degrees. iii) All hinges, handles, blts, r ther parts are set flush with the flr r cvered surface. iv) Prtective cvers shall be marked in 1 inch letters r larger in height with the fllwing wrds: Cautin: Flr Opening - D Nt Remve Cver d) Flr and rf penings can alternatively be prtected by a standard guardrail system with a te bard. e) In absence f a standard guardrail, hatchway cver must be kept clsed except when ascending/descending. 14) Cntractrs a) The prject manager will ensure the cntractrs cmply with applicable standards and regulatins pertaining t fall prtectin. Cntractrs failing t adhere t the prvisins f Cal OSHA standards will be asked t terminate their wrk until their wrk practices are brught int cmpliance. b) Prir t starting n wrk requiring fall prtectin, the cntractr will submit a cpy f their Fall Prtectin Plan t the prject manager. The prject manager will review the plan, and cnsult with EHS if needed, fr required prvisins. c) An effective rescue plan must be develped and implemented specific t the wrk lcatin and jb being perfrmed prir t the start f wrk. 13 Fall Prtectin Prgram Envirnmental Health and Safety d) Where persnal fall prtectin equipment is utilized, anther emplyee shuld be present at all times t activate the rescue plan r summn fr assistance in the event f a fall. e) The prject manager will assess and infrm the cntractr f all knwn hazards relating t the prject, including ptential fall hazards. f) The cntractr shall assess hazards and cntrl measures, prvide training and equipment fr their emplyees wrking at height. 16 Fall Prtectin Prgram Envirnmental Health and Safety Appendix B Persnal Fall Arrest Prtectin Equipment Inspectin Criteria ANSI Z User must inspect befre each use. Cmpetent persn (ther than the user) must inspect at least semi annually. Devices with defects, damage, inadequate maintenance shall be remved frm service r underg adequate crrective maintenance.
