Policymakers are now beginning to focus on post acute reforms as well. Community based organizations, including home health and hospice, must understand the implications of these reforms as well as the significant opportunities for new partnerships and business models. This presentation will outline existing pilot and demonstration programs, highlight quality improvement goals in new care models, and describe opportunities to influence future policymaking on post acute care models. 9 About Our Presenters Tracey Moorhead is President and Chief Executive Officer of the Visiting Nurse Associations of America (VNAA). In this role, she spearheads VNAA s strategic vision and growth in collaboration with the VNAA Board of Directors. She effectively directs quality improvement programs, policy formulation and strategic advocacy activities and represents the non profit home health, hospice and health promotion community before the media, allied organizations and constituents, and federal policymakers. She previously served as President and CEO of the Care Continuum Alliance, an industry organization advancing population health management interventions through health and wellness promotion, disease management and care coordination. Her prior roles also include Executive Director of the Alliance to Improve Medicare (AIM), a bipartisan coalition advocating comprehensive Medicare improvements through the Medicare Modernization Act of 2003, and Vice President, Government Relations for the Healthcare Leadership Council (HLC). Marcia P. Reissig has been the Chief Executive Executive Officer of Sutter Care at Home Sutter Health s home health affiliate since Ms. and Reissig is responsible for the development administration of the system s $200 million home care program, which includes home health, hospice, home infusion pharmacy, home medical equipment and respiratory therapy, Lifeline, private services, and flu and wellness services. She has more than 30 years of management and clinical experience, holding chief executive positions at Partners Health Care in Boston and other large integrated health care systems in New York prior to joining Sutter Health. She served six years on the National Association for Home Care & Hospice (NAHC) Board of Directors. She earned her Bachelor of Science degree in Nursing and a Master of Science degree in Community Health from the University of Rochester. Jeanne M. Ryan, MA, OTR, CHCE, COS C is the Executive Director of the VNA & Hospice/Cross Continuum Services of Cooley Dickinson. She is a member of the Board of Directors of the Home Care Alliance of Massachusetts and has been published in many home care journals, including Home Healthcare Nurse, The Remington Report and Caring Magazine on such topics as: Teamwork Keeps the Pressure off: The Role of Rehabilitation in Pressure Ulcer Prevention ; The Rehabilitation Professional as Agency Administrator and Executive; and Strength in Numbers: A Community based Falls Prevention Program. She is pursuing her M.B.A. with an emphasis on entrepreneurial thinking and innovative processes. Author Andrew Zolli directs the global innovation network PopTech and has served as a fellow of the National Geographic Society. His work and ideas have appeared in a wide array of media outlets including PBS, The New York Times, National Public Radio, Fast Company and many others. Ms. Madden Baer has practiced as both a nurse and nursing leader in a variety of home health and community based settings for more than 30 years. She is board certified as a public health/community health clinical specialist, received her Doctorate of Nursing Practice and holds certifications as a Professional in Healthcare Quality, Home care and Hospice Executive, certified OASIS specialist and Population Health Care Coordinator through a postgraduate program at Duke University. Rose s responsibilities include creation of clinical operations improvement strategies and development of new evidence based programs. Ms. Margaret Meg A. Wallingford is an accomplished healthcare executive with extensive experience and proven success developed during a thirty year career with New England s most successful health service organizations (Tufts Health Plans, Blue Cross and Blue Shield of Massachusetts, Caritas Christi Health System, Capital District Physicians Health Plan).Meg was a founding member of Senior Whole Health (SWH) LLC. a multi state innovative health care plan for "dual eligibles which led the way as an affordable health solution and model for the Affordable Care Act. 10 About Our Presenters Dr. Robert Fazzi is Founder and Managing Partner of Fazzi Associates, one of the oldest, largest and most respected organizations of its kind dedicated to home and hospice. Its core services include outsource coding, consulting, benchmarking, training and national research projects. Mr. Fazzi has been involved with the home care field for over thirty years. He has acted as Director and Co Director on major national studies with partners such as 3M Corporation, Philips, the National Association for Home Care and Hospice, Blackberry, Briggs Corporation and Delta Health Technologies. He has served on numerous national committees and was named as the first Chairperson of the National Association for Home Care and Hospice s National Research Academy. Mr. Fazzi is the author of numerous articles and books on home care and hospice related issues and is the author of Management Plus, a supervisory management book published by McGraw Hill and named as a main selection by Fortune Magazine s Book of the Month Club. Singer and songwriter Judy Collins has led an impressive musical career with an extensive catalog from every decade beginning in the 1960s to the present. Judy Collins is still writing, performing, and nurturing fresh talent. She plays 80 to 100 dates a year around the country. A relentlessly creative spirit, Ms. Collins is a modern day Renaissance woman who is also an accomplished painter, filmmaker, record label leader, musical mentor, and an in demand keynote speaker for mental health and suicide prevention. She continues to create music of hope and healing that lights up the world and speaks to the heart. Barbara McCann is the Chief Industry Officer at Interim Healthcare, the nation s largest home care and hospice franchisor with over 300 offices in 44 states. In this role, she acts as the company s liaison and industry representative to Congress, CMS, MedPAC, the National Quality Forum, and the Patient Centered Primary Care Collaboration. In addition, she directs the national care transition programs partnering with hospitals, physicians and payors to reduce avoidable readmissions and effectively transition chronically ill patients and their families to home self management and safety. Ms. McCann has received many industry awards for her work and has presented worldwide on health care quality and accountability. She holds a Masters in social work from the University of New Mexico. Julia Fox Garrison, Author of the book Don t Leave Me This Way, shares her personal memoir of her struggle as a stroke victim and the importance of provider/patient relationships and dealing with the medical and insurance communities. A graduate of Champlain College in Burlington, VT, she began her career in the computer industry. Having a knack for problem solving, she rose through the ranks and became a first level manager for software support when she suffered a massive stroke ending her career in the corporate world. Susan Dentzer is one of the nation s most respected health policy thought leaders and journalists. As a Senior Policy Advisor at the Robert Wood Johnson Foundation, the nation s largest health and health care philanthropic organization, she works closely with foundation leaders to carry out a mission of improving the health and health care of all Americans.
