2 It s been more than a year since the Centers for Medicare & Medicaid Services (CMS) approved Maryland s historic All-Payer Model Agreement in January As newfound partners across the health care continuum collaborate to improve care and reduce costs, what can Maryland learn from the national lab? What ideas and programs have been successful in other states? Can we take what s being done elsewhere, sprinkle some Old Bay on it and make it our own? To answer these questions and others, LifeSpan and MHA (Maryland Hospital Association) are planning a summit that will convene top national leaders from hospitals and post-acute care providers to highlight specific examples of how interdependent, cross-organizational partnerships can improve patient outcomes, avoid unnecessary utilization and help reduce health care costs. This summit is intended to carefully capture the essence of hospital and post-acute business relationships and describe the precise care delivery and support strategies needed to support care in non-hospital settings. In this first joint Leadership Summit between LifeSpan and MHA, participants will discuss immediate and long-term strategies that hold the promise to deliver on the Triple Aim of health care: lower per capita costs, healthier communities and better patient experiences. Speakers will demonstrate how to effectively coordinate care across settings. Attendees will hear about sharing risks and rewards while improving quality and reducing costs. They ll learn how to think of the emergency department in a new way. Through speakers presentation and robust discussion, participants will come to understand how to build successful cross-continuum partnerships through shared goals and expectations. Success in our new era of health care means that today s innovators must respond quickly to a rapidly evolving environment. This summit provides tools to help them make critical decisions that help provide the people we serve with the type of care they expect and deserve. Summit Facilitator Amy E. Boutwell, MD, MPP Amy Boutwell, MD, MPP, founded Collaborative Healthcare Strategies to pursue work aligned with the healthcare delivery redesign policies of the Affordable Care Act to lower the costs of case through delivering better care. Dr. Boutwell is a noted expert in identifying and designing approaches to improve care across settings taking the best from what is known to be effective, practical and efficient in improving care transitions. At the beginning of the present-day efforts to reduce readmissions, Dr. Boutwell co-designed the STAAR (State Action on Avoidable Rehospitalizations) Initiative of the Institute for Healthcare Improvement (IHI). The STAAR initiative was unique in its strategic approach to reduce readmissions by engaging multiple provider types to collaborate at the state and local market levels as opposed to an isolated focus on just hospital-based improvements. Currently, Dr. Boutwell s portfolio of advisement includes service as the strategic and technical advisor to the Massachusetts Health Policy Commission s $120M CHART Investment Program, strategic and technical advisor to the Maryland Hospital Association, faculty for the CMS ACO and Bundled Payments Learning System, and co-principal investigator of an AHRQ funded effort to focus on readmissions in the Medicaid popululation. Dr. Boutwell is a graduate of Stanford University, Brown University School of Medicine and the Harvard Kennedy School of Government, where she received a master s degree in public policy and the Robert F. Kennedy Award for Excellence in Public Service. Dr. Boutwell is a hospital medicine physician at Newton-Wellesley Hospital, attends on the medicine teaching service at Massachusetts General Hospital and is an Instructor in Medicine at Harvard Medical School.

4 Presenters Karim A. Habibi, FHFMA, MPH, MS Karim is the Senior Vice President & Chief of Managed Care and Payment Reform Initiatives at NYU Langone Medical Center where he is responsible for developing and implementing managed care strategies including pay for performance contracting, contract review, negotiation, implementation, and compliance for the Hospitals and the Faculty Group Practices. Mr. Habibi is also responsible for providing management services for an IPA of approximately 1,900 physicians. He also directs the efforts of negotiating contracts on behalf of the NYUPN Clinically Integrated Network. The contracts encompass the CMS Bundled Payments for Care Initiative, Commercial ACO like arrangements and other alternative Laurie Herndon, MSN, GNP-BC Laurie Herndon is the Director of Clinical Quality at the Massachusetts Senior Care Foundation. In this role she works with nursing home owners, frontline providers, and colleagues from all sectors of health care to improve quality, innovation, and research in MA nursing facilities. Laurie is also Gerontological Nurse Practitioner with 20 years of clinical experience in skilled nursing facilities. Laurie has worked as a project consultant for a number of New England Quality payment structures. He has over 29 years of healthcare experience, including positions as Director of Managed Care and Assistant Vice President Financial Operations at Maimonides Medical Center, Brooklyn, NY. Karim holds a Master of Science in Health Services Management from New York University and a Master of Public Health in Health Administration from the University of Oklahoma. He is a Fellow of the Healthcare Financial Management Association and is certified in the areas of managed care and patient accounts. Mr. Habibi coauthored articles on managed care, bundled payments, surgical implant cost containment strategies and spoke at national conferences on managed care contracting strategies and cost effective clinical practices. She has also consulted with the Massachusetts Board of Registration in Nursing on both practice and regulatoryrelated issues. Laurie has served as the Senior Project Coordinator on several INTERACT projects and in this role has trained facilities from across the country on how to implement INTERACT.

5 Presenters Donna Lichti, RN Ms. Lichti is currently the Senior Vice President of Market Development for Visiting Nurse Service of New York (VNSNY), the country s largest not-forprofit organization providing in-home nursing care, therapy and hospice, palliative services and managed health plan services to New Yorkers. Ms Lichti leads the growth of business by working across the entire organization to promote VNSNY as the optimum solution to address the needs of business-to-business customers, innovating care delivery and supporting the needs of integrated health systems, physician organizations, nursing facilities and community based organizations. Business development and intelligence, strategic clinical initiatives and managed contracting are within her portfolio of responsibilities. Ms. Lichti s prior experiences range from software development, communications and marketing to development of specialty healthcare business models. Ms. Lichti owned and operated home health care businesses in the North East and founded one of the first infusion based nursing services in the country. Ms. Lichti recently became a board member of the newly formed Healthix, a health information exchange serving greater New York. Ms. Lichti is a graduate of Pace University Leinhard School of Nursing, the State University of NY and is a Registered Nurse. She completed a Global Health Leadership certificate through UCLA Berkeley School of Public Health and her Executive MBA Certificate from Notre Dame, Medozza College of Business. Steven P. Sbardella, M.D., FACEP, FABC Dr. Steven P. Sbardella currently serves as Vice President of Medical Affairs and Chief Medical Officer at Hallmark Health System, located in Massachusetts, just northwest of Boston. Prior to his current role Steven served as Chief Quality Officer as well as Chairman of Emergency Medicine, a position he held for approximately 7 years. During those years Steven completed a Fellowship at the Advisory Board Company in Washington, DC. He has practiced Emergency Medicine for approximately 30 years, 25 of which he served in an administrative capacity as Chairman of a Department. Dr. Sbardella s undergraduate degree, in Philosophy, was completed at the University of Vermont. He obtained his medical degree from the University of Rome, Rome, Italy.
