To ensure that services and support are delivered to a high quality and within nationally determined standards set by CQC and other recognised guidance. To ensure services are delivered within national and locally determined timescales and performance targets. To manage services and staff and make decisions to effectively support vulnerable people with complex needs who present as high risk. Promote individualised options for care provision and ensure adults at risk are protected from harm. Provide strategic management to the Assessment and Support Planning 25% Service and a range of in house service provision for adults and young people with a learning disability and autism ensuring that an adequate supply of high quality cost effective services are available for individuals who meet the Council s eligibility criteria and that in-house provision is of a high quality. Take lead responsibility within the service area to ensure the safeguarding of vulnerable people assessed as high risk.provide clear guidance and take high level decisions in complex cases including cases involving concerns around hate crime, sexual exploitation and forced marriage. Work jointly with Service Manager (Assessment and Support Planning) on the delivery of care management projects and initiatives. 2. Provide management leadership and direction in the division and ensure mechanisms exist to recruit, motivate train and develop staff in order to ensure the workforce can meet its objectives. Contribute to the 20% development of policy and practice within the Department and ensure that procedures and processes are available to guide staff in both Care Management and provider services, in the performance of their duties and ensure that these are adhered to. To work with other departments and agencies to ensure recruitment and development of a competent and effective work force that meets the needs of the service. 3. Strategically work at a high level with other departments, statutory agencies, other service providers, the voluntary sector, service users and carers, to ensure that services are jointly planned and strategically driven and focus on outcomes that promote social inclusion, independence, 10%
choice and well-being and safeguard service users. Work closely with specialist learning disability clinical services, specialist in patient mental health services and the Hospital Operational Discharge Manager and other health professionals to ensure safe and effective discharge for this client group and delivery on the Transforming Care agenda. 4. Take lead role to develop and implement a range of complex projects on behalf of the Department including the delivery of day care services for adults and young people with a learning disability and autism/challenging needs and the delivery of a respite model of service for this client group. Take lead role in transitions across Children s Services and Adult Social Care ensuring an effective and seamless delivery of support. Input into the market development strategy. 5. Develop Strategic Objectives for the service area, ensure they are met and that meaningful and detailed management and performance information is generated to demonstrate they are being met. 6. Work with teams to ensure they provide high quality cost effective services on behalf of the Council, either directly or through commissioned care and allocate resources/deliver services within budget allocation. Take remedial action as required to balance the budget. Chair Resource Allocation Panel for this customer group. Take lead Responsibility for the Council on section 75 agreement between the Council and the CCG. 7. Ensure that monitoring systems are in place so that services provided to customers are of a high quality, meet assessed needs, are cost effective and delivered in such a way to encourage independence, respect and choice and ensure the safety and wellbeing of customers. Work with the Purchasing and Quality Assurance Team to achieve objectives and address provider non compliance / provider failure. 8. Lead on work with Customers and Family Carers, attend Partnership Board, Autism Board and Carers meetings to ensure the contribution of customers and carers is recognised and valued and contribute to the development of personalised services that promote social inclusion. Take lead responsibility for consultation with staff, customers, carers and other stakeholders in respect of the transformation agenda for the service. 9. Be aware of and act within the legal framework including Care Act, Children and Families legislation, Safeguarding, MCA and DOLs, CQC requirements, national and local guidance which affects the service area and the wider ASC division and keep abreast of national developments and best practice for Adult Social Care. The policy drive from central and local government promotes closer working with other agencies; the expectation is that all service developments are coordinated across the Council, Health Service, Voluntary and private sectors. The policy drive from central government is for the local authority to take lead responsibility for this customer group (including responsibility for aspects of the service previously provided by the NHS). This post covers line management of the integrated social care and health assessment and care/support planning for people with a learning disability and autism in the community and in inpatient hospital settings, including responsibilities for discharge arrangements, and for the line management of the services directly provided for this customer group by Luton Borough Council. The post holder is also responsible for the strategic planning and delivery of services to this customer group and to carers. The post holder is the department s representative in the multi agency strategic planning and budget management in this area. The post holder will take a lead on transformation projects on behalf of the Council and will work with the Joint Commissioning Team, CCG, Procurement and Quality Assurance Team, Children s Services and other internal/external parties to deliver on key objectives/projects. The post holder will need to work jointly with the Service Manager (Assessment and Support Planning) on projects and initiatives, performance and quality monitoring and safeguarding due to the care management responsibilities of the role. The post holder will be required to deputise for the Head of Service as required. It will also be necessary to keep abreast of all potential additional resource requirements to ensure services develop to meet demand and that there is sufficient
understanding of the role of other Service Managers in Adult Social Care to provide effective cover.
