1 Job Description Date May 2014 Post Title ducation Social Worker Post No. Grade Grade 9 Section Service Area Service Grouping Responsible To Integrated Family Support Service (IFSS) Children and Family Services Social Well-being and Housing Team Around the Cluster Manager Job Purpose Within small locality areas, to work with secondary and feeder primary schools, social services and local partners, to improve children s and young people s lives by overcoming barriers to learning. Key Results Areas 1.0 Service Provision Be the initial point of contact for schools to access additional bespoke services needed to support individual pupils and families improved school attendance and wellbeing. Build and encourage a shared social care perspective within educational establishments. To be a standing member of the local Team around the family (TAF) panels. To undertake holistic assessments for vulnerable children and families Direct social work with vulnerable children and families identified within educational settings building on, and creating additionality to the services already provided by the Prevention Service and Families First as well as other community based provisions Build close links with the Social services Duty Team and be involved in decisions about eligibility for social work services and bring challenge to decisions made by the Duty Desk Where appropriate, as lead professional, co-ordinate a range of agencies and interventions for the child/young person and family Contribute to and advise on plans to help students and their families cope with personal and psychological issues that affect school performance, behaviour and socialization. Provide professional supervision/advice/guidance to unqualified staff working with vulnerable children and families nsure safeguarding is at the heart of multi-agency services to children and families Within educational settings, advise on best practice methods of addressing specific problems, and with partners and other professionals, continually keep abreast and utilise best practice. Provide information of outside support services or agencies to help students/families deal with economic, emotional or physical challenges. Contribute to the maintenance of records and reports to track student problems and progress, and to recognize patterns in behaviour.

2 And importantly, support and contribute to improving educational outcomes by working with colleagues and partners to shared common objectives Generic duties of the post are: 1.1 To perform all the functions of the post in a manner consistent with current legislation and with current Social Services and ducational policies, standards and procedures as endorsed by the Partnership Board 1.2 To take responsibility for all the functions of the post within the boundaries of delegated responsibility. 1.3 To complete requisite administrative tasks in an efficient and effective manner. 1.4 To act appropriately upon receiving referrals 1.5 To engage and empower children young people and families and their support networks and actively elicit their feedback in order to ensure person centred/family centred interventions. 1.6 To ensure that appropriate information is available for service users and carers, colleagues and partners 1.7 To carry out assessments as appropriate, within timescales and to contribute to or coordinate the construction of outcome focused intervention plans which are informed by best practice, guidance and an understanding of the organisational context. 1.8 To work in partnership with other professionals, in order to provide appropriate and effective services for service users and carers. 1.9 To provide written and verbal reports as required and to assist in assessing and monitoring the implementation of plans and outcomes for service users To assist colleagues in the development of best practice Additional duties beyond SCP To support the professional development of newly qualified social workers and undertake the workload management and practice assessment of social work students undertaking practice learning opportunities in the team To participate in the planning and commissioning of service delivery as required and monitor and review service delivery To promote the involvement of service users and carers in both care planning and service development To chair meetings as directed by the team manager. 2.0 Administration 2.1 To have an understanding of and follow Social Services administrative procedures and Processes 2.2 To provide a quality standard of report and to maintain accurate case recording using the appropriate software as required

Qualifications and xperience Registration with the CCW as a qualified social worker Qualifications in the Care Council for Wales post qualifying qualification framework for social workers ffective communication skills See competency framework document for progression beyond SCP 36 Supervisory Responsibilities No formal supervisory responsibility. xpectation to support and advise colleagues from a range of other agencies in relation to safeguarding, whole family working and collaborative practices. Supervision Received To receive supervision from the Team around the Cluster Mananger or Senior Practitioner in line with the NCC Supervision Policy Principal Contacts mployees of Newport City Council Shool staff including Faith schools and Social care staff Service users and their families Informal carers xternal social care agencies Community based organisations and agencies.
