S/he will have the willingness, capacity and energy to develop themselves within the organisation. S/he will be practical, flexible and passionate about supporting our client group. Experience At least two years experience of working with mental health clients with additional support needs. Experience of recognising individuals needs and seeking creative and practical solutions to enhance their quality of life. Skills/Abilities Ability to develop effective relationships with a wide range of people and respond sensitively to cultural, gender and disability factors. Have a wide range of practical skills. Ability to train, motivate and support volunteers. The ability to travel within the borough for the purpose of visiting clients in their own homes. Ability to work as an individual, using ones initiative and managing own time, as well as functioning effectively as part of a team. Have excellent written and verbal communication skills. Have a good understanding of IT skills. Knowledge & Understanding An understanding and knowledge of the needs of people suffering severe mental distress. We support our clients to participate in vocational and recreational activities of their choice with a focus on developing health and well being groups within our day service. Job Summary Outreach/Project Workers will be expected to build enduring relationships with clients that increase confidence and trust, enabling clients to feel safe and interested in regaining old or fostering new skills and talents. The Outreach/Project Worker will respond flexibly to the clients needs, seeking creative and practical solutions to facilitate their integration with the local community and improve their quality of life. Project Workers will be expected to carry a caseload of clients some of whom they will provide outreach support for. They will also be expected to help facilitate group activities within our day service. Core Responsibilities 1. Building and maintaining an enduring relationship with clients suffering with mental health needs encouraging them to be more self-reliant and empowered in the pursuit of a better quality of life in the community. 2. To support and assist clients in accessing information or advice and encourage uptake of services where appropriate. 3. Assessing and monitoring the desires and needs of the client group, responding appropriately and flexibly to changing needs. 4. To facilitate groups. 5. To hold a caseload of clients, offering them an outreach service where appropriate. 6. Set, monitor and review action plans for individuals to vocational and recreational goals. Working with other members of staff and external providers in the carrying out of individual action plans. 3

4 6. To work with other members of staff and external providers to increase vocational and recreational opportunities for clients. 7. To work with the Project Director to recruit, train and supervise volunteers. 8. To work with the Project Director and other staff members to receive referrals and/or initiate contact with people experiencing severe mental distress. 9. To liase with referral sources, locations and project staff to identify potential clients. 10. Setting priorities and monitoring allocation of time. 11. To promote a positive image of the project. 12. Identifying areas for operational development. 13. Providing cover for other members of staff 14. Undertaking other designated duties and accepting special responsibilities relating to particular requirements of the service. 4
