4 Core Duties: 1. To provide a service in line with statutory requirements and departmental priorities. 2. To provide a holistic assessment service to disabled people, their families and carers within the context of the Social Model of Disability and in line with statutory requirements and departmental priorities; and subsequently arrange services to meet identified need. 3. To understand the role of Social worker and to be able to communicate this to both service users and professional colleagues. 4. To represent the team at relevant meetings/working party groups. 5. To participate in training a required in the interests of the service overall and career development. 6. To take responsibility both individual and with colleagues to consider new initiatives. 7. To understand the concept of customer care and actively promote this with both internal and external customers of the Disability Team. 8. To understand the stated priorities of the Department and how these are to be achieved. 9. To effectively liaise with relevant external agencies e.g. Primary Care Trust, Hospital Trusts, Private and Voluntary Sector, Police. 10. To be responsible for individual case planning/case management and professional decisions in line with the professional standards of the Department, and in consultation with management. 11. To actively participate with team colleagues in devising the annual Local Action Plan, which identifies the goals, the team wishes to achieve, subsequently, to work with team colleagues to actively monitor the team s progress towards achieving the intended goals. 12. To work with team colleagues in planning for and participating in events appropriate to this service user group e.g. Celebrating Learning Disabilities Week 13. To proactively identify and participate in training relevant to the interests of the service and your personal career development, in consultation with management. 14. To take responsibility for maintaining your General Social Care Council registration. 15. To actively participate in regular supervision with your manager and peer group supervisions. 16. To actively participate in team meetings including contributing to agendas, chairing and minuting these meetings.
5 17. To work in the role of Duty Social Worker to cover annual leave, training and sickness absence as required. 18. To participate in written recording, or using new technology where necessary. 19. To work with service users in a person-centred way 20. Respond to public and other agencies by telephone, correspondence and direct contact in order to provide a service within departmental priorities. 21. Liaise with relevant external agencies, e.g. Police, Health Authority etc. 22. Liaise with relevant colleagues in other Divisions and Council Departments. 23. To be responsible for individual client case planning in line with the professional standards of the Department. 24. To be active in implementing the Council s Equal Opportunity Policy in professional practice and service delivery. 25. To be responsible for professional decisions within the Department s priorities framework and in relation to specific procedural guidelines referring to consultation with management. 26. To undertake service development tasks as appropriate. 27. To familiarise themselves with Departmental policies and procedures. 28. To work in the role of Duty Social Worker to cover annual leave, training and sickness absence as required. 29. To show an awareness of and to contribute to performance information.

This enables the panel to assess your ability to meet each criteria. Use additional sheets if necessary. A. A Dip SW or equivalent qualification or experience. B. A commitment to the Council s Equal Opportunities policies and the ability to understand and implement the policies in relation to the job responsibilities. C. Demonstrate an understanding of the needs of People with Learning Difficulties and the main recommendations of the government White Paper, Valuing People (Now) D. The ability to undertake complex needs led assessments. To plan, implement and review the delivery of packages of care or other appropriate Social Services Interventions. E. Demonstrate an understanding of the legislative framework relevant to Learning Disabilities Service users and of the professional responsibilities of a Local Authority Social Worker. F. Demonstrate the ability to work in a team and with other professionals and agencies on behalf of Service Users, their relatives and carers. The ability to represent the social work perspective and task in multi-disciplinary meetings. G. The ability to maintain appropriate records in accordance with the legislative requirements relating to access to files. H. To demonstrate an understanding of the processes of loss, bereavement and change and the ability to use that understanding in a plan of intervention.
