JOB CONTEXT This psitin is lcated within the Melburne City Missin Disability and Palliative Care divisin and implements Melburne City Missin s cmmitment t best practice in Palliative Care. Melburne City Missin Palliative Care (MCMPC) was established in 1981 as the first cmmunity-based Palliative Care service t perate in Victria. MCMPC aims t enhance quality f life by prviding specialist interdisciplinary palliative care fr peple living with a terminal cnditin, their families and carers. MCMPC prvides a 24 hur service 7 days a week. The service is prvided t residents f the lcal gvernment areas f Darebin, Hume, Mreland and Yarra in accrdance with the Palliative Care Australia s Standards fr Prviding Quality Palliative Care fr all Australians 2005. JOB PURPOSE Wrking as a member f an interdisciplinary team and in cllabratin with external health prfessinals, the Palliative Care RN will prvide Palliative Care t MCMPC clients in accrdance with the missin, values, plicies and prcedures f MCM Palliative Care and within the philsphy and rganisatinal values f Melburne City Missin. JOB OBJECTIVES The Palliative Care Nurse will prvide initial nursing assessment and psychscial screening fr all clients admitted fr care. The utcmes f the assessment are used t crdinate and plan apprpriate nging nursing care in partnership with RDNS and ther services within Melburne City Missin Palliative Care catchment. Page 1 f 5

Client care Demnstrate high level f skill in clinical decisin making: Prblem identificatin and slutin Analysis and interpretatin f clinical data Maintenance and imprvement f clinical standards Prvide cmprehensive assessment and regular review f palliative care clients - using a hlistic and anticipatry apprach. Screen clients, carers and families fr clinical, psychscial and spiritual needs, fcusing n strengths and challenges, scial supprts, financial and legal cncerns Assess client nursing needs and the carer s ability t prvide care at hme Assist clients in evaluating symptm cntrl, setting gals and plans Prvide supprt, advice, educatin and advcacy fr clients and carers f diverse ages, cultures and backgrunds Initiate and participate in nging advance care planning with all clients and primary carers Initiate and participate in the preparatin fr end f life with all clients and primary carers Plan and crdinate services (with ther agencies r disciplines) relating t end f life care with all clients and carers Anticipate after-hurs requirements and ensure preparatins are in place fr client and carer shuld the need fr after-hurs care arise Ensure that client care meets prfessinal, rganisatinal, legal and ethical requirements at all times Deliver a high standard f care regardless f clients r clleagues differing values, beliefs, culture and scial cntext. KEY SELECTION CRITERIA Essential Registered Nurse Divisin 1 with current certificatin frm the Australian Health Practitiner Regulatin Agency (AHPRA) Minimum 4 years pst graduate wrk experience 3 years experience wrking in palliative care and/r nclgy Pstgraduate qualificatins in palliative care and/r nclgy, r wrking twards Demnstrated high level f skill in clinical decisin-making Cmpetence with electrnic client recrds and well-develped cmputer skills Excellent written and verbal cmmunicatin and negtiatin skills Sensitivity t peple living with a terminal cnditin and thse wh are dying r bereaved Ability t wrk independently with minimal supervisin Ability t wrk cllabratively within an interdisciplinary team setting Excellent time management and rganisatinal skills Current Victrian driver licence Desirable Nursing experience in a cmmunity setting Demnstrated knwledge f aged care and/r chrnic illness Page 4 f 5
