The Senir AOD Clinician wrks in crdinatin with all AOD service streams (Intake and Assessment, Cunselling, Care Crdinatin, Nn Residential and Residential Withdrawal, Residential Rehabilitatin and Yuth services) and with related sectrs, t prvide an integrated service. This purpse f this psitin is t facilitate seamless and integrated treatment pathways fr cmplex clients. The Care and Recvery Clinician (C&RC) plays a critical rle in clients transitins thrugh the AOD service phases, frm intake and assessment thrugh t engagement with treatment and lnger term recvery services. In rder t achieve this, the C&RC wrks clsely with internal and external service prviders t ensure a smth transitin and assist clients t remain n their treatment plan. The C&RC wrks with cmplex clients t develp hlistic, lng term recvery plans fcusing n recvery frm prblematic substance abuse and prmting health and wellbeing and scial cnnectedness. They wrk t help clients achieve utcmes that will sustain psitive, nging behaviur change and imprve quality f life. KEY RESPONSIBILITIES PROVISION OF TREATMENT SERVICES Prvide high quality engagement, assessment, treatment and supprt services t individuals and their families, using best practice principles including Family Inclusive Practice and Dual Diagnsis principles. Case manage cmplex clients, facilitate, crdinate and supprt clients thrughut the life cycle f their treatment plan. Prvide high quality pre and pst care crdinatin services t clients and their families, using best practice principles including Family Inclusive Practice and Dual Diagnsis Principles. Prvide pre-care supprt fr clients until their engagement with services cmmences. Prvide flexible crdinatin and supprt and targeted interventins t clients and family members, t enhance the client s engagement and retentin in treatment and facilitate re-entry fr clients wh have disengaged r relapsed fllwing treatment. These interventins may include: Cnducting assessments f clients AOD treatment and recvery needs and assisting clients t develp hlistic treatment plans t meet these needs Prviding practical assistance, such as transprt t key appintments and facilitating access t relevant services Opprtunistic mtivatinal interviewing and supprtive cunselling with the aim f retaining clients in treatment Prvisin f harm reductin infrmatin and educatin Page 1 f 5

Develp individual lng term recvery plans by assisting clients t identify achievable and relevant gals Utilise the Drug & Alchl Outcme Star and/r ther utcme measure t identify gals, track and mnitr prgress acrss a range f recvery dmains and prvide clients with tangible evidence f the effectiveness f their treatments. Wrk with clients and their families t develp hlistic discharge plans that supprt meaningful scial and ecnmic participatin and ther practical aspects f recvery. Prvide pst care supprt including assisting clients and their families t negtiate their treatment jurney beynd the AOD sectr as required. Fllw up with clients at 3 and 12 mnths pst treatment t track prgress f recvery and supprt reengagement with AOD treatment r ther supprts where apprpriate. Participate in daily clinical reviews t ensure apprpriate treatment planning and care crdinatin is in place and reviewed regularly. Identify the needs f vulnerable children, wrking with families t develp safety plans and facilitating referrals t partner Child First and Family service agents. Prvide clinical expertise and secndary cnsultatin t ther health and welfare services including the prvisin f ffsite assessments t facilitate clients access t AOD treatment. PROFESSIONAL LIAISON AND CONSULTATION Develp and maintain cllabrative and prductive wrking relatinships with a range f stakehlders including GPs, mental health services, hspitals, family services, husing wrkers and hmeless supprt, child prtectin, debt/financial cunselling, emplyment service and ther stakehlders. Develp strng cmmunity and sectr cnnectins t supprt imprved crdinatin f catchment service planning, delivery and review. Develp relatinships within the sectr, identifying new partnerships and enhancing existing nes t imprve interagency cllabratin and recvery pathways fr clients. Establish and maintain strng cnnectins with all AOD services acrss the catchments including intake and assessment, withdrawal, cunselling, residential rehabilitatin and yuth services and ther AOD family and mental health service types. Wrk clsely with ther relevant services t crdinate service planning and avid duplicatin f recvery supprts. Participate in team planning, steering grups, reference grups and wrking parties inside and utside the agency as apprpriate. Maintain regular cmmunicatin and feedback with the Clinical Cnsultant and the wider rganizatin, regarding clinical issues and service crdinatin. Page 2 f 5

3 SUPERVISION AND PROFESSIONAL DEVELOPMENT Actively participate in nging prfessinal develpment via the annual perfrmance reviews, individual and grup supervisin and by attending relevant training as required. Prvide clinical supervisin t less experienced clinicians. Prvide clinical leadership and cnsultatin t staff. Supprt the student placement prgram and supervise students as requested. ADMINISTRATION Maintain up t date prgress ntes, case files and data entry in a cnfidential manner. Actively participate in the revisin and develpment f plicies, clinical guidelines and treatment prgrams.
