1 JOB DESCRIPTION SOUTHEND ON SEA BOROUGH COUNCIL DEPARTMENT OF INTERMEDIATE CARE SERVICES RE-ABLEMENT SERVICES JOB PROFILE Job Title Reports To Structure chart SENIOR HEALTH & SOCIAL CARE WORKER REGISTERED MANAGER Please include a structure chart for the team. Level LEVEL 7 Main Purpose of the Job Key Responsibilities To be a member of a Multi-disciplinary re-ablement team, providing a short term, high quality service to adults in a community environment. To assist the registered professionals and lead the Health & Social Care Workers, encouraging development. Tasks include re-ablement, nursing and social care duties to promote safe and timely hospital discharge and prevent hospital, inpatient or residential admission. To provide an efficient and cost effective service to adults, their families and carers in accordance with agreed eligibility criteria using appropriate resources of Southend On Sea Borough Council Community Services. To ensure that the needs of adults and their carers are professionally assessed and to promote service user centred care following an individual plan of care established by the multi-disciplinary team ( nurses, social workers therapists) ensuring service user consent and participation in their care. To evidence leadership to health & Social Care Workers as a member of the multi-disciplinary integrated team, working with agencies across community, primary and secondary health care trusts To develop practice that takes a holistic view of the service user having full regard to their social, recreational, physical, religious, cultural, and emotional and health needs, involving family members, and care support as indicated. To ensure that all appropriate information is gathered, evaluated and analysed to enable an appropriate plan of care to be formulated for ongoing provision of care in line with Southend on Sea Borough Council criteria. To ensure that all practice is in line with NHS and Community Care Act, The National Minimum Standards for Domiciliary Care, and other associated regulations and guidance. To assume responsibility in relation to assessing for provision of care with appropriate assessments, advocating for and empowering service users and in partnership with Statutory, voluntary and private agencies. To ensure that all work undertaken is in partnership with the service user and that they are supported to identify and promote their re-ablement needs. To implement care plans by directly providing physical, social and emotional support to enhance the quality of life for Service users and their carers in a multi-disciplinary way while promoting independence. To supervise, support and mentor Health & Social Care Workers to ensure that a highest standard of care are maintained in service delivery at all times. To ensure that work programs are available on a daily basis for health and social care staff taking into account, sickness, holidays and family crisis.
2 To complete an induction program and re-ablement and competency program to enable competency to carry out tasks required. To support senior staff in the completion of the established re-ablement competency framework through the provision of cascade training to all staff as assigned, ensuring standards are met and maintained. To participate in regular supervision/appraisal in line with Southend on Sea Borough Council policy. To work to mandatory legislation and Southend on Sea Borough Council Health and Safety procedures, ensuring good practice and to promote high standards of health and safety in the workplace. To ensure any changes to health and safety is reported to senior staff without delay. To ensure that Southend on Sea Borough Council s Incident Reporting Procedure is completed in line with health and safety executive requirements. To manage the assessment of medication in line with the delegation of tasks policy, under the direction of qualified nursing staff and in line with Southend On Sea Borough Council medication policy. To work alongside other professionals to co-ordinate a program of care for individuals in the community in times of crisis. To observe, record and report any changes in individual conditions and circumstances and ensure that they are reported to senior staff without delay. To achieve standards as set by the Registered Team Manager ensuring that the wishes and needs of the individual are considered at all times. To enable individuals to achieve their optimum level of independence by providing a re-ablement program as detailed in their plans of care in a way which positively promotes their independence. To work alongside therapy staff who will set individual goals to facilitate and promote individuals independence. To undertake a Risk assessment within 48 hours of admission to the team in line with current legislation. To undertake risk assessments before re-ablement to ensure that individual is able to continue with goals set and to ensure that any hazards or risks are identified and reported without delay. To be aware of emergency procedures required for working in the community and procedures used for dealing with these. These procedures are for individuals and your own safety. To apply knowledge and skills gained from training and competency to ensure a high quality of care to individuals. To promote and maintain feedback on a daily basis regarding the progress of individuals in regards to their rehabilitation goals. Ensure that Health & Social Care Workers are aware of the team expectation regarding continued feedback on service user progress and oversee the standards regarding documentation. To develop and maintain links with other professionals in the community to promote integrated working. To ensure that own practice is of a high standard at all times. To ensure that carers needs and views are listened to and reported to senior management without delay if there are any issues that are not being met To work alongside families and carers to encourage access to other services statutory, voluntary or private. To attend weekly Multi-disciplinary meetings to discuss individual care programs and provide information to enable re-ablement goals to be set. To attend all team meetings set and promote a culture where staff are respected and involved at all times in their views. To attend all mandatory training and development dates set and to identify and promote your own development needs for progression. To ensure that individuals are treated with respect and dignity at all times To ensure that individuals, their families and carers are fully involved in their reablement at all times
3 To respect the personal, religious and cultural beliefs of individuals and their carers at all times. To respect individuals property at all times. To maintain confidentiality at all times in accordance with policies and procedures of Southend on Sea Borough Council. To be aware that any information received while on duty is dealt with appropriately. Failure to comply with the Department s guidelines on confidentiality will be subject to disciplinary action. To be aware of and implement the Comments, compliments and complaints policy and procedure and act as an advocate to ensure those individuals and their carers are encouraged to use this at all times. To contribute to a culture within the Department that values individuals for their positive contributions towards work and encourages and supports high standards of interpersonal behaviour and professional performance at all times. To maintain paperwork and records concerning hours worked and mileage claims on a daily basis. To ensure that Personal Protective Clothing as supplied by the Department is worn at all times in line with universal infection control procedures and The Health and Safety at Work Act legislation. To maintain awareness of development in professional practice and be proactive in defining ones own developmental and training needs. To contribute towards identifying service development needs, to continually evaluate appropriateness of service to ensure that it meets the needs of service users in the community to enable operational changes and development of the team. To participate in staff training both as a mentor and a learner. To ensure at all times your own actions promote the philosophy of the team. To work with statutory, private and voluntary agencies to improve standards and contribute towards pilot schemes in the team in a way that promotes inter agency development. Responsibility for Resources To enter full and accurate information onto computer based information systems, to ensure the security of such systems and the confidentiality of service User group. To ensure that all records and case files of users and others for which the post holder is responsible are properly maintained, with full regard to the need to record the views of users and third parties. To understand and comply with the Department s administrative practices and procedures. To prepare and submit reports as requested on time and to ensure that all correspondence and communication is in accordance with agreed practices and procedures. Following a comprehensive assessment, recommend expenditure within Southend on Sea Community Services agreed criteria and policy. Other duties: To undertake any other duties commensurate with the post as described above. As the Southend on Sea Collaborative Care Team evolves it may be necessary to review the duties and responsibilities. Any changes to the post will be discussed with the post holder. You have a responsibility to discuss any post changes with the Registered Manager at the time the change occurs, to discuss and agree any substantial post changes. IT literate QUALIFICATIONS Hold or be prepared and able to study for NVQ level 3/4. SPECIAL CIRCUMSTANCES Must be a car driver. To work to a rota system, including out of hours, weekends and bank holiday working. Valid driving licence and car available for business use. It is a Council requirement that applicants for this post obtain a Standard/Enhanced Disclosure from the Criminal Records Bureau. A copy of the Council s Policy Statement on the Recruitment of Ex-offenders and Safekeeping of Disclosure information is enclosed for information.
