3 Supported Living Officer Out Of Hours/On Call We are looking for talented people to join our Supported Living Team to work on a rota d basis providing On Call services to our Supported Living Residents The Supported Living Team deliver a supported housing service to individuals in their own home, offering housing related support and services, advocating for them and empowering them in the context of independent living in order that they are assisted in maintaining their homes. As an On Call Supported Living Officer you will provide out of hours emergency cover for our tenants living in our Supported Living Accommodation throughout Wellingborough and the surrounding area, you will need to be able to respond quickly, have access to a car and have a calm manner. The whole team manage our Supported Housing schemes including taking responsibility for health and safety, maintenance and the cleanliness of the communal areas of the schemes. We do not provide care services. We are looking for people with previous, or current, Supported Living experience who have the ability to communicate effectively with people and are used to dealing with a wide range of situations and people. Applicants will have excellent interpersonal and customer care skills. To be considered for this post you must be able to demonstrate in your application form: Some work or voluntary experience in a supported living environment That you can offer relevant skills including but not limited to interpersonal and communication skills, and that you have experience of supporting people in their own homes without supervision. That you have some experience of interacting with the general public either face to face or by phone including dealing with people either by telephone or face to face who are distressed or angry. Your ability to commit to working on a rota d basis to cover On Call services That you are able to respond to emergencies in the Wellingborough and local area quickly and efficiently and that you have access to a car and a full uk Driving Licence Your salary will be based on salary band 5 starting at 20, ( per hour) You will receive the following payments for On Call periods: The on call rate for time spent on call between 17:00 and 09:00 (overnight) for on call periods commencing at 17:00 Monday to Thursday will be 20% of your hourly rate; The on call rate for time spent on call between 17:00 and 09:00 (overnight) for on call periods commencing at 17:00 Friday to Sunday will be 25% of your hourly rate; The on call rate for time spent on call commencing between 09:00 and 17:00 Saturday to Sunday will be 30% of your hourly rate;

4 The on call rate for time spent on call between 17:00 and 09:00 (overnight) for on call periods commencing at 17:00 on a day designated as a bank or public holiday day in England will be 30% of your hourly rate; The on call rate for time spent on call commencing between 09:00 and 17:00 on a day designated as a bank or public holiday day in England will be 35% of your hourly rate; If you are called out you will be paid your standard hourly rate, you will also be eligible for a flat rate essential car user allowance based on the emissions of your car. You are guaranteed 13 weeks Out of Hours work every 52 weeks as set by your manager on a rotated basis One week is classed as Monday to Monday, where you will be on call; Monday to Friday - Night Only All day Saturday and Sunday (Day and Night) You will be rota d to work at least 2 bank holidays nights on call per 52 weeks. There is an expectation that you will complete scheduled visits on a Saturday and Sunday lasting approximately 2 hours On call night period is between 17:00pm to 9:00am 1. Bank/public) days 28 per annum (pro rata part time and part completed year) and minimum 20 holiday (ex. Bank/public) days per annum (pro rata part time and part completed year) An Occupational Sick Pay scheme is in place, entitlement depending on length of service. Either Career Average Related Earnings (CARE) Social Housing (defined benefit) Pension Scheme OR Money purchase (defined contribution) pension scheme (employee and employer s contribution) with salary sacrifice option for additional contributions. Reimbursement of one subscription where professional membership is necessary for the performance of duties. 3* Life Assurance Cover in addition to any cover provided if you enrol in the defined contribution pension scheme. Access to occupational health and counselling services provision. Complementary membership of The Waendel and Redwell Leisure Centres in the Borough of Wellingborough. Deliver a supported service to individuals in their home and community, offering housing related support and services, advocating for them and empowering them in the context of independent living in order that they are assisted in maintaining their homes. 2. Encour age social, cultural, leisure and recreational activities in consultation with residents. 3. To support tenants with specific needs to live in the community by: - Undertaking joint assessments with other service providers - Co-ordinating support services from other agencies - Acting as advocate for tenants in accessing appropriate services. - Working collaboratively with other agencies Key Achievement Areas: 1. Assess needs of prospective clients prior to them accessing the service to ensure all needs can be appropriately supported and met. Assess and carry out support planning for individuals to include budget management, benefits advice, housing management, form filling, daily living skills, lifelines and needs assessments, referring onto other agencies or Support Wellingborough where appropriate. 2. Be aware of the individual needs of each client with special reference to differing disabilities i.e.: learning disabilities, mental health, physical and or sensory disabilities and liaise with family, GP s, domiciliary services and other agencies. 3. Respond to emergency calls for assistance and co-ordinate the appropriate services and maintain records of all emergency calls and events whilst on duty. 4. Carry out support visits to clients as defined by their support plan, and record the outcome of contact as appropriate, maintaining accurate factual records. 5. Contribute to the effective operation of the Support Services Team as it relates to the home support service in the provision of a high quality, customer orientated service, and to work with individuals to ensure that they have meaningful input into the support that they receive. 6. Be responsible for notifying the Supported Living Team Leader of deaths, unforeseen absences of tenants and cases where the tenant is no longer able to look after themselves with minimal support. 7. Provide sufficient information relating to the scheme, residents and clients to allow for an effective handover to other supported living officers. 8. Bring forward Best Practice and Best Value in support and care initiatives as it relates to Corporate Objectives and Support Service delivery. 9. To ensure compliance with the quality assessment framework. 10. Keep all resident records, e.g. next of kin, doctor, etc. and ensure the call centre records are amended as necessary in accordance with procedures. 11. Provide performance statistics and information, as required.

7 12. Assist with the promotion and development of the Associations Lifeline Service. 13. Work with colleagues across the Association to ensure a seamless service is provided to customers. 14. Work with the Housing Officer to ensure a joined up approach to the prevention of, and response to, anti-social behaviour ensuring that victims and witnesses are dealt with respect at all times. 15. Ensure a range of opportunities are available to customers to help them shape the delivery of future services to provide an innovative approach to resident involvement and participation. 16. Ensure the maintenance of effective records and deal with all correspondence relevant to areas of responsibility, generating letters, reports, and statistics as required, using computer technology. 17. Work with all residents irrespective of tenure, voluntary and statutory partners taking an inclusive approach to estate management 18. Assist in the induction and training programmes for new and existing staff increasing the awareness of support service issues and processes, taking responsibility for the training given 19. Be aware of, and ensure assistance is given to tenants who are experiencing difficulty claiming housing benefit and are falling into rent arrears. 20. Give advice and information relating to benefit law and give assistance with completing Housing Benefit forms and to accept and verify all documents relating to Housing Benefit claims, taking responsibility for ensuring safe delivery of information to the local authority and obtaining a receipt for relevant documents provided. 21. Accompany and assist the Occupational Therapist on home visits to applicants seeking priority on the transfer register via the medical panel. 22. Provide cover and assistance to other supported living officers as required to ensure that the service is delivered effectively. 23. Attend Support Services team meetings and training and development events as advised by the Supported Living Team Leader Other This profile only contains the main accountabilities relating to the job and does not describe in detail all the duties required to carry them out. The post holder may be expected to: Undertake such other duties and responsibilities as are specified by the Board, Executive Team, Director of Housing and Support Services and Head of Support & Community Services and Support Services Manager which are commensurate with the level of the role. Work outside of normal office hours especially to attend meetings and respond to out of hours emergencies. Act at all times within the organisations rule, policies, procedures, standing orders and financial regulations. Special Circumstances Duties include work at any of the Association s Supported Housing Schemes as directed by the Supported Living Team Leader. You must hold a current full driving licence and have access to a vehicle for work purposes, for which a car allowance will be paid. The post holder may from time to time be required to carry out other duties provided within the general level of responsibility of the post and the abilities of the post holder The main duties and responsibilities may be amended from time to time, with consultation, to assist personal development and meet the Association s requirements.

9 Wellingborough Homes Person Specification Supported Living Officer Qualifications Experience Skills/Knowledge Essential GCSE education or equivalent through relevant training/experience. Experience of working in a customer facing role, achieving successful outcomes in the delivery of services that meet customer need Experience in undertaking risk and needs assessments. Experience in delivering support services to the elderly and vulnerable adults within the community. Experience of lone working Experience of person centred support planning. Able to effectively communicate with customers and understand their needs, often in emotional and sensitive situations. Effective advocacy and sign posting skills. Knowledge and understanding of other services available to help customers maintain independent living. Effective time management skills. Able to work effectively under pressure, organise priorities and in a lone working environment. Commitment to delivering high quality customer services and commitment to continuous improvement. Good numerical and analytical skills and an ability to understand and interpret information. Excellent verbal and written communication and interpersonal skills. Confident, professional manor. Strong negotiating skills and ability to understand others perspective whilst developing professional and effective working relationships. Experience of dealing with initial enquires/concerns in a professional, constructive and helpful manner to help prevent escalation to complaints. Desirable Housing or Supported housing qualification or studying towards. Experience of customer involvement and consultation techniques. Empathy and a good understanding of the issues faced by elderly and vulnerable customers
