1 Title: Term of contract: WORCESTERSHIRE ACUTE HOSPITALS NHS TRUST JOB DESCRIPTION Pre-Registration Pharmacy Graduate 12 months fixed term Pay Band: Agenda for Change Band 5 Hours: Base: Accountable to: Reports to: Liaises with: 37.5 hours per week Pharmacy department, Worcestershire Royal Hospital or Alexandra Hospital, Redditch Chief Pharmacist Clinical Services Pre-registration Tutors, Clinical Pharmacy Team leaders, other departmental trainers all other pharmacy staff, medical and nursing staff in relation to the day to day delivery of the WAHT pharmacy services. Job Purpose: The pre-registration pharmacy graduate post is a one year fixed term training post, which will assist the post holder in meeting the requirements of the General Pharmaceutical Council (GPhC) to register as a Pharmacist. The post holder will: undertake supervised duties involved in the provision of the WAHT pharmaceutical service to patients, medical and nursing staff in WRH/AH and other hospitals with which the Trust has a Service Level Agreement (SLA) ensuring the services provided are risk managed and comply with medicines legislation, GPhC Code of Ethics, the Trust Medicines Policy, and Pharmacy Standard Operating procedures. follow the Trust s GPhC approved pre-reg training programme. be responsible for documenting evidence in order to meet the GPhC performance standards. undertake the registration examination within the time scales specified by the GPhC. Risk Assessment: The post holder may be occasionally involved in the counselling of patients and carers under difficult situations, especially around a newly diagnosed clinical condition with far reaching implications on quality of life and life expectancy. This requires occasional involvement in emotionally distressing conditions such as discussions around palliative treatment for terminally ill patients or those with poor recovery or functional prognosis. This may also include contact with patients or carers where the patient may be suffering from an adverse reaction to a prescribed medication or have suffered a drug misadventure and who may be distressed, anxious or verbally or physically aggressive as a result. The post holder will be frequently involved in the dispensing of cytotoxic agents and regimes for patients under their care and could be exposed to other noxious chemicals used within the Pharmacy. Work will often be required for a pressurised deadline. As a result, the post holder will be exposed to stressful interactions with patients, nursing and medical staff, balancing individual patient need against resources available.

2 The post holder will use a VDU for periods of time throughout the day totalling about 10% of his or her working day. Occasional requirement to lift boxes of medicines e.g. intravenous fluids weighing 6-15kg. Key Roles and Responsibilities: 1. Provision of Clinical Pharmacy Services The post holder will: 1.1. Provide clinical pharmacy services on rotation to the acute medical, surgical, and emergency units, under the supervision of Lead Specialist Pharmacists Ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines-related national and local guidelines within the Units where duties are being carried out Be accountable for his or her own professional actions guided by appropriate technical and professional Standard Operating Procedures and associated policies, national directives, standards, guidelines, senior pharmacist colleagues, and the postholder s own knowledge Plan his or her duties and prioritise workload where duties are being carried out according to the needs of patients, medical and nursing staff in order to complete the following within the time available, and all under the supervision of a clinical pharmacist: Analysing patients individual pharmaceutical care needs Identifying and correcting prescribing errors/unclear prescriptions with medical staff Advising prescribers on doses, side effects, alternative treatments Communicate complex pharmaceutical recommendations and advice concerning patients treatment effectively to medical and nursing staff and other health care professionals, as delegated and under the supervision of a clinical pharmacist Interview newly admitted patients, as assigned and under the supervision of a clinical pharmacist, to construct an accurate medication history, assess compliance/concordance issues, and: Review information against the admission details and prescription chart Identify to medical staff any potential or actual medicines-related hospital admissions or associated issues and outline any required action to resolve identified discrepancies Construct pharmaceutical care plans for patients, assess and monitor clinical progress and outcome with regard to their medication and actively intervene to ensure that patients receive optimal care with minimal side effects, as assigned and under the supervision of a clinical pharmacist Assess patients discharge information requirements as early as possible in the admission stay to be able to prepare information on medicines for them based on individual need Assist with the therapeutic blood monitoring of identified drugs within the postholder s clinical area (all under the supervision of a clinical pharmacist) including: Initial patient assessment and initiation of appropriate loading or starting doses Completion of request cards for level assaying Interpretation of blood level results and subsequent adjustment of the patient s dose of medicine and amendment of the patient s prescription in order to optimise the beneficial results of therapy whilst minimising any potential toxicity Use MS Word, Excel and Powerpoint to produce patient information, information for medical and nursing staff, audits and project reports.

3 2. Provision of Dispensary Services: The post holder will be responsible for: 2.1. Contributing to the provision of the WAHT dispensary service to inpatients and outpatients using the pharmacy computer system for stock control and labelling, under the supervision of a pharmacist or suitably qualified technician, including: Maintenance of satisfactory stock levels, storage and turnover of medicines to ensure the quality of medicines are maintained Identification and solution of actual and potential problems with reference to other staff as necessary Completion and maintenance of accurate records where appropriate Entering patient details on the Pharmacy Computer System from prescriptions of doctors and other practitioners and accurately produce medicine labels Accurately, with no margin for error, dispense/prepare medicines ensuring doses are correct and accurately weighed, measured and/or counted Issue of medicines to patients, including provision of appropriate information Check medicines that other pharmacy staff have dispensed/prepared to ensure they have been dispensed correctly Using the Pharmacy Computer System to query the availability, location and cost of medicines, and to dispense and supply medicines Using the Electronic Discharge System for processing discharge prescriptions Participation in dispensary weekend and bank holiday rotas as appropriate. 3. Provision of Medicines Information Services The post-holder will: 3.1 Participate in the enquiry answering function of the medicines information service under the direct supervision of a pharmacist including: Receipt of enquiries from healthcare professionals, patients and members of the public Use of a variety of information sources according to UKMI standards; database (electronic and manual) searching, critical appraisal and evaluation of information Provision of the response to the enquirer in a variety of formats (verbal and written) Referral of the enquirer to more appropriate sources of information where necessary Documentation of enquiries and associated audit and user surveys. 4. Provision of Aseptic Services The post-holder will: 4.1 Contribute to the work of the Aseptic production unit under the direct supervision of a pharmacist or suitably qualified technician including (where appropriate): Assisting in the provision of a cytotoxic reconstitution, parenteral nutrition, intravenous additive and radiopharmacy service Participation in cleaning schedules Assisting in monitoring the clean room environment Assembly of raw materials required Completion and maintenance of accurate records Ensuring all work completed complies with departmental standard operating procedures and current legislation and guidance.

4 5. Financial Responsibilities The post-holder will: 5.1. Help to maintain the security of medicines and supplies within the WAHT Pharmacy Department Help to ensure there is a complete financial audit trail, using the Pharmacy Computer System, for all medicines issued from the WAHT Pharmacy Department and medicines are accurately charged to wards, departments, consultants, and any other areas where medicines are supplied Handle prescription charges when necessary. 6. Education, Training and CPD The post holder will be responsible for: 6.1. Developing their own professional and pharmaceutical skills through participation in the Trust s GPhC approved pre-registration pharmacy graduate rotation programme including a minimum of 2 weeks cross sector training Completion of the GPhC performance standards by gathering a portfolio of evidence to demonstrate competence and maintain progress with quarterly appraisals with the pre-registration tutor Completion of all required competencies and performance standards to take the GPhC registration examination Attending the Regional pre-registration study days Undertaking and recording own continuing professional development (CPD) Undertaking study in own time when necessary Taking a proactive role in own personal development through objective setting, review of progress, and participation in progress and development meetings with preregistration tutors. 7. Other Responsibilities The post holder will: 7.1 Undertake an audit of a defined aspect of the pharmaceutical service. Standard Clauses: The purpose of this post should remain constant, but the duties and responsibilities may vary over time within the overall role and level of the post. The post holder may from time to time be asked to undertake other reasonable duties. Any such changes will be made in discussion with the post holder in the light of service needs. Competence The post holder is responsible for limiting his/her actions to those which s/he feels competent to undertake. If the post holder has any doubts as to his or her competence during the course of his/her duties then s/he should immediately speak to their line manager or supervisor. Codes of conduct All employees of the Trust who are required to be registered with a professional body, to enable them to practise within their profession, are required to comply with their code of conduct and requirements of their professional registration. Those staff who are not required to be registered with a professional body are required to comply with the Trust s codes of conduct. Confidentiality The post holder must maintain confidentiality, security and integrity of information relating to patients, staff and other Health Service business.

5 Records Management All employees of the Trust are legally responsible for all records that they gather, create or use as part of their work within the Trust (including patient, financial, personnel and administrative), whether paper or computer based. All such records are considered public records and all employees have a legal duty of confidence to service users. Employees should consult their manager if they have any doubt as to the correct management of records with which they work. Health and Safety Employees must be aware of the responsibilities placed on them under the Health and Safety at Work Act (1974) and the Manual Handling Operations Regulations (1992) and all relevant Trust Health and Safety Policies and Guidance. This ensures that the agreed safety procedures are carried out to maintain a safe environment for employees, patients and visitors to the Trust. Infection Control Employees must accept personal responsibility and accountability for Infection Prevention and Control practice. Employees should ensure they are familiar with, and comply with, all relevant Infection Control policies for minimising the risk of avoidable Health Care Associated Infection. All Employees must undertake annual mandatory updates in Infection Control. Risk Management It is a standard element of the role and responsibility of all employees of the Trust that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, taking appropriate actions, and reporting all incidents, near misses, and hazards promptly. It is a contractual obligation that all employees must co-operate with any investigations undertaken. Children and vulnerable adults You have a responsibility for promoting and safeguarding the welfare of the children/young people/vulnerable adults that you come into contact with or are responsible for in your job role and sphere of competence. Disclosures and Barring Service (formerly CRB) Disclosure The Trust aims to promote equality of opportunity for all, with the right mix of talent, skills and potential. Criminal records will be taken into account for recruitment purposes, only when the conviction is relevant. The Trust will undertake the relevant Standard or Enhanced DBS disclosure in accordance with 2012 Protection of Freedom Act and DBS guidance. In summary, a Standard Check will be used where an individual work is concerned with the provision of healthcare services which is of such a kind to enable the holder of that employment to have access to recipients of such services in the course of their normal duties. An Enhanced DBS Check including the DBS/ISA barred lists if they engage in what is defined as Regulated Activity which are jobs that involve caring/supervising or being in sole charge of children/ vulnerable adults. The Trust s Standing Orders require any member of staff to declare any interest, direct or indirect, with contracts involving the Trust. Staff are not allowed to further their private interests in the course of their NHS duties.
