4 Creating Surveys New surveys can either be created frm scratch r by cpying an existing survey. When creating a survey by cpying an existing survey, the existing survey is cpied in its entirety. All survey ptins, questin, page cnditins, etc. are cpied t the new survey. If the survey being cpied has respnses, nne f the respnses are cpied. After cpying the survey, yu can then change the name f the survey, and mdify If a user is lgged int the applicatin with Create permissin, the drpdwn bx used fr cpying an existing survey cntains a list f all surveys wned by that persn. If the user has Admin permissin, all surveys in the system are displayed in the drpdwn bx. Designing Surveys Each survey can cntain an unlimited number f questins distributed amngst any number f pages. A survey shuld cntain multiple pages if page cnditins are desired, r t limit the number f questins per page. New pages are inserted int a survey by clicking the "NEW PG" buttns in the apprpriate lcatin. Page cnditins can be inserted r updated by clicking the "INSERT PAGE CONDITION" r "EDIT PAGE CONDITION" buttns that appear at the tp f each page. Nte that these buttns are nt displayed fr the first page in a survey, since the first page must be displayed and therefre cannt have page cnditins assciated with it. The main page fr designing surveys displays visual representatins f all f the questins. Nte that because f space limitatins, the way the questins are displayed n this page is nt exactly the same as they are displayed in an actual survey. Use the Preview functinality t see hw the survey will actually appear. New items can be added t a survey either by creating them frm scratch r by cpying an existing item. When cpying an existing item, the entire item, with all respnses, if applicable, is cpied int the survey. The item can then be mdified as desired. The lk and feel f every survey is determined by the template assciated with that particular template. The template fr each survey can be set n the survey ptins page. See the sectin belw, "Templates," fr mre infrmatin n using templates. Page 4 f 38

5 Every page in the survey may ptinally include a title r an intrductin. If either the title r the intrductin is mitted fr a particular page, that page is rendered withut the title r intrductin, as specified. Templates Templates are used t fully define the lk and feel f all surveys. There is n limit t the number f templates a user may create. Each survey must be assciated t a template, and any number f surveys can be assciated t a single template. Mdifying a template changes the lk and feel f all surveys assciated with that template. Templates are used t define all f the fllwing: Lg (ptinal) Brder width and clr (ptinal) Backgrund clrs Heading and alternating clrs fr matrix questins All fnt styles HTML fr headers and fters (ptinal) Navigatin buttns r links Using templates, the lk and feel f almst all web sites can be created fr yur survey. By prviding HTML fr the header and fter, yu can have the same navigatin, header, and fter as yur wn web site. With the many ther template attributes, the entire lk and feel can be cpied. Finally, applying yur wn lg t the survey can help t create a fully branded survey. The applicatin cntains a default template called, "Default Template." This template is assciated with all new surveys. This template cannt be remved frm the system, but it can be mdified, as desired. New templates can be created either by creating a new template frm scratch, r by cpying an existing template. Templates can be deleted if they are nt assciated with any surveys. When setting the template fr a survey, nly surveys that are marked as active can be selected. Page 5 f 38

6 Templates are wned, by default, by the user wh created the template. T enable sharing f a template, the "Public User" prperty must be set t "Yes." This prperty indicates whether r nt usage f the template is limited t administratrs and the template wner, r it is available t all users f the applicatin. The lk and feel f any template can be viewed by clicking ne f the "Preview" buttns, which are lcated n the template list page, the template details page, and next t the template drpdwn n the survey details page. Custm Stylesheet: The custm stylesheet bx is where yu can paste a custm CSS (cascading style sheet) text. Directly belw the textbx is a stylesheet reference with the class names f every questin type which can be referenced in yur custm stylesheet. This allws ultimate flexibility and cntrl t manage style ver each individual questin type if yu like. Otherwise the default styles will be used. Libraries are useful because yu can create a questin nce in a library, and then reuse it ver and ver again by inserting it int yur surveys. T insert an item int a survey frm a library, click ne f the "Insert" buttns n the survey. Then, click the "Insert frm Library" buttn. Select the desired library frm the drpdwn list, then select the item, and click the "Cntinue" buttn. New items can be added t a library in tw ways, bth f which can be accessed by clicking any f the "Insert" buttns n the library page. The first way is t create a new questin frm scratch. The secnd way is t click n the "Cpy Existing Item" buttn, and cpy an item frm an existing survey. Page 6 f 38

7 All libraries are wned by and viewable nly by the wner f the library. The exceptin t this rule is that administratrs can view and manage all libraries in the applicatin. Yu may ptinally share a library with ther users by setting the "Public" prperty, lcated n the library ptins page, t "Yes." Sharing a library means that ther users can create questins by cpying items frm the library, but they cannt mdify the library itself. Only administratrs and the wner f the library can actually mdify the library. Questin Item Types The applicatin cntains ver 32 different item types. Many f the item types are questins that request a respnse frm the user, while thers are simply frmatting elements fr HTML, messages, images. etc. CHOICE - SINGLE SELECT DROPDOWN The single select drpdwn yu can set as required and set a default value. Yu can als place text replacement piping tkens in the questin text r answers. CHOICE - SINGLE SELECT OPTION BUTTONS The single select ptin buttns questin type yu can set as required and set a default value. Yu can als place text replacement piping tkens in the questin text r answers. CHOICE - MULTI CHOICE CHECKBOXES The multi chice checkbx questin allws multiple selectins and can be set as required and set a default value. Yu can als place text replacement piping tkens in the questin text r answers. CHOICE - DATABASE DROPDOWN The "Chice - Database Drpdwn" element allws a user t create a drpdwn questin that draws its data frm any database. T use this questin type, all that is required is that the user enter a valid SQL statement and the database cnnectin string. When using this questin type, be extremely careful t ensure that the SQL entered will return results, and that the database cnnetin infrmatn is valid. If either f these are incrrect, n data will be displayed. The SQL statement must cntain tw clumns called "ItemValue" and "ItemDisplay". These tw clumns are used, respectively, as the value and the display text fr each entry in the drpdwn list. The crrect way t make sure that yur SQL cntains clumns with these tw names is t use the "As" keywrd in yur SQL, as shwin the fllwing example: SELECT state_cde As ItemValue, state_name as ItemDisplay FROM tblstate ORDER BY state_name Page 7 f 38

8 If yu wish t use the same clumn fr bth the value and text, simply select that clumn twice in yur SQL statement, and alias it using the "As" keywrd, as fllws: SELECT state_cde As ItemValue, state_cde as ItemDisplay FROM tblstate ORDER BY state_name The database cnnectin string can be any valid DSN-less cnnectin, r any valid DSN. YES/NO The chice - yes n questin type allws respndents t chse a yes r n answer in hrizntal r vertical layut. Default value can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. TRUE/FALSE The true false questin type allws respndents t chse true r false answer in hrizntal r vertical layut. Default value can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. OPEN ENDED ONE LINE The pen ended ne line allws respndents t enter text answers limited t 255 characters. Default value can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. OPEN ENDED COMMENTS BOX The pen ended cmments bx allws respndents t enter unlimited text characters. Default value can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. OPEN ENDED DATE The pen ended date questin allws respndents t select a date frm the date picker, r type in a date. Default value f current date can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. OPEN ENDED NUMBER The pen ended number questin type allws respndents t enter numeric answers. Number masking frmats are available fr: dllars, eurs, decimal, general numeric, etc. This frmat als will be used in the exprt files. Default value can be set and questin can be set as required. 9 The pen ended ne r mre lines can have multiple rws fr text entry answers. Text replacement piping tkens can be used in the text. OPEN ENDED CALCULATED The pen ended calculated questin type allws the survey designer t enter a frmula which will calculate the numeric value that displays in the textbx frm answers t ther questins. (Nte this is similiar t dynamic tken except dynamic tken is set n survey ptins page) Text replacement piping tkens can be used in the text. OPEN ENDED NUMBER SLIDING SCALE The pen ended number sliding scale is a slider that can be tggled t pick a number in a number range vertically r hrizntally specified by typing in the min/max number range acceptable. Text replacement piping tkens can be used in the text. CONSTANT SUM The cnstant sum questin allws respndent t enter numbers that wuld add up t a ttal value. The minimum, r maximum r required validatin is available. Text replacement piping tkens can be used in the text. RANKING The ranking questin allws respndents t numerically rank answer ptins. Text replacement piping tkens can be used in the text. MATRIX SINGLE SELECT OPTIONS The matrix questin has multiple clumns and rws f ptin buttns. Text replacement piping tkens can be used in the text. MATRIX MULTI SELECT CHECKBOXES The matrix multiple chice checkbx questin type has multiple rws and clumns f checkbxes. Text replacement piping tkens can be used in the text. MATRIX TEXTBOXES The matrix textbx questin cntains multiple rws and clumns f textbxes. Text replacement piping tkens can be used in the text. MATRIX TEXT DROPDOWN The matrix cmb flexible text/drpdwn questin allws yes/n, true/false, number, date, pen ended types in a multiple clumn multiple rw questin. 10 MATRIX RATING SCALE The matrix rating scale is ptin buttns fr rating in multiple clums and rws. Text replacement piping tkens can be used in the text. MATRIX - DYNAMIC ROW The dynamic rw matrix cntains a drp dwn bx where the survey respndent can chse the number f rws f infrmatin that are needed t enter. When the respndent chses the number f rws, thse rws will display fr them t fill in. The survey respndent can define the max number f rws that can be displayed, and this number shws in the drp dwn bx fr the respndent. This questin type is helpful in situatins where the respndent is asked t fill in infrmatin fr a variable number f stres, lcatins, r any data that requires multiple clumns and variable number f rws. HORIZONTAL RULE The hrizntal rule questin type adds a line divider hrizntal rule acrss the page. PRESENTATION HEADING The presentatin heading questin type allws yu t type in a heading which uses the styles in the template fr a heading. Inline HTML and piping tkens can als be placed in this questin type fr text replacement. PRESENTATION IMAGE The presentatin image questin type displays, image,,vide r audi files t the respndent. The survey designer can click the brwse buttn t select the file t uplad t the web server and als select the file type. Default value can be set and questin can be set as required. Yu can als place text replacement piping tkens in the questin text r answers. PRESENTATION MESSAGE The presentatin message questin type can be used fr infrmatin text n the survey. Text replacement piping tkens can be used in the text. PRESENTATION HTML The presentatin html questin type can be used t paste in html such as charts, graphs, vide player bjects, whenever yu need the flexibility f custm html. Text replacement piping tkens can be used in the text. PRESENTATION SOUND (ONE TIME) The presentatin sund (ne time) is fr a sund file with nly a play buttn and nt the full cntrls. Fr full cntrls, chse the Presentatin Sund (FULL) questin type. Page 10 f 38

11 PRESENTATION SOUND (FULL) The presentatin sund (full) is fr a sund file with all cntrls, such as play, rewind, pause, stp, replay. Fr a single play versin with n vide cntrls, chse the Presentatin Sund (ne time) questin type. USER REGISTRATION The user registratin questin type allws a respndent t specify and username and passwrd and be added t the user database s they can take surveys with this lgin later. They are nly assigned the lwest lgin level where they can nly take surveys. Text replacement piping tkens can be used in the text. RESPONDENT FILE UPLOAD The respndent file uplad questin type allws a respndent t uplad a file f the type and size specified by the survey designer. Text replacement piping tkens can be used in the text. TOUCH - NUMBER SLIDER The questin types that start with the wrd "TOUCH" are designed fr ipad and smart phnes. The number slider dynamically detects the brwser type and versin and will display HTML5 versin f number slider if that technlgy is supprted, therwise it shws the IE cmpatible versin f the questin type. The number slider allws respndents t pick a number by sliding a bar n a scale line. The survey designer can specifiy the minimum and maximum numbers fr the slider scale. TOUCH - CLICKABLE IMAGE MAPS The questin types that start with the wrd "TOUCH" are designed fr ipad and smart phnes. The image maps questin allws uplading images and defining tuch areas in the image and saving thse tuch area maps. The survey designer can define the tuch areas by dragging the muse n the image n the edit item page. The muse ver text fr each image map area can be defined as well as the clr f the utline r highlight fr the area. Als the actin fr clicking n the tuch area can be defined. By default, when the respndent clicks r tuches ne f the defined image map areas, the value fr that area is input int the cmments bx which is stred with the respnse in the database. The click cunt is als recrded and stred n the survey page. If the respndent clicks the same area twice, it is de-selected and autmatically remved frm the cmments bx. Scred Surveys Pint values can be assciated with questins that have predefined values. The types f questins that can have pints values assciated with them are checkbxes, ptin buttns, and drpdwn lists. 12 Nt all questin types supprt pint values, and it's pssible that nt all answers in a questin that des supprt scred surveys will have a pint value assciated with it. Therefre, when calculating the scre fr a survey, any questin r answer t a questin that des nt have a pint value assciated with it is simply ignred. Data abut the scres fr any survey with pint values assciated t it can be viewed, by default, in the reprts. Page Cnditins Page cnditins are the mechanism by which pages in a multi-page survey are ptinally displayed. That is, ther than the first page in a survey, each page may be shwn t r hidden frm the respndent based n the page cnditins assciated with the page. If a page des nt cntain any page cnditins, the page is always displayed t the user. The page cnditins n a page are based n the respnses given t a questin r questins n a prir page r pages in the survey. A page cnditin can be based n any questin n any prir page, nt just the previus page. If the cnditins n a survey evaluate t "True," the page is displayed. Similarly, if any f the page cnditins evaluate t "False," the page is hidden frm the respndent. Each cnditin is created by selecting a questin frm a prir page, selecting an peratr, and entering r selecting the value t be cmpared using the selected peratr. The apprpriate peratrs and, if applicable, the predefined answers fr a questin, are displayed depending n the type f the questin selected. Each page cnditin must be part f a grup, and each page in a survey can use up t five grups. A grup is the way "AND" vs. "OR" lgic has been implemented in the applicatin. The cnditins within a single grup are evaluated using "AND" lgic, while each grup is cmbined tgether using "OR" lgic. Thus, a page is displayed if at least ne grup evaluates t "True," even if all ther grups n the page are "False." A single grup evaluates t "True" if all f the individual cnditins within that grup evaluate t "True." T better illustrate hw grups wrk, let s use an example f a page in a survey that has tw grups, each f which has tw cnditins. The page will be displayed if: Bth cnditins in the first grup evaluate t "True" (regardless f hw the secnd grup evaluates) Bth cnditins in the secnd grup evaluate t "True" (regardless f hw the first grup evaluates) Page 12 f 38

13 The page will NOT be displayed if: Each grup has at least ne cnditin that evaluates t "False" As a secnd example, a survey cntains five pages. The first page cntains a single questin which is a drpdwn bx prmpting the user t select his/her gender. If the user selects "Male," the fifth page shuld be displayed. If the user selects "Female," page tw, three, and fur (but nt five) shuld be displayed. T implement, page cnditins are added t the secnd, third, furth, and fifth pages f the survey. Each page has a single page cnditin, all f which use the questin frm the first page. On pages tw, three, and fur, the page cnditin uses an equal peratr, and the value "Female." On the fifth page, the page cnditin again uses an equal peratr, with the value "Male." In the case where "Male" is selected n the first page, the user "skips" frm the first page t the fifth page. This type f skip lgic is cmmnly used, and is a gd example f hw every page in a survey can be ptinally displayed t the user. Questin Cnditins Cnditinal questins are questins that nly appear when an assciated questin answer matches the cnditins specified by the survey designer. T insert a cnditinal questin, n the mdify survey page, click the "insert cnd. quest." buttn t the right f the questin f which answer will determine whether the cnditinal questin is shwn r nt. The questin will appear r disappear n the same page as the determining questin. Yu can add ne r mre pen ended text bx questins t any "chice" questin type t be cnditinal questins. Cnditinal questins are nly allwed n "chice" questin types, such as single chice ptin buttns, r multi chice checkbx. 360 Questin Grups/Categries 360 Questin Grups/Categries, GAMP, ISO and Practice Level are all set frm the edit item page, and frm the Questin Categries management page. These questin item prperties are used fr 360 reprts such as Free Frm Reprts, which have chart cmpnents which analyze the questin scres by grup. The chart types are described in Free Frm Reprts sectin. Piping Tkens ANSWER PIPING: Displaying the answer t ne questin in the text f a subsequent questin is called answer piping. Fr example, n the first page f yur survey, yu might have a questin that asks the user fr his/her name. On a subsequent Page 13 f 38

14 page, yu wuld like t begin a questin using the respndent's name. This is accmplished by piping the answer frm the first questin int the desired lcatin n the secnd page. T set up answer piping, simply insert a pipe tken int the desired lcatin in either the questin text r subtext. The frmat fr the tken is ###xxx###, where xxx represents the ID f the item whse answer shuld be inserted in that lcatin. T make this prcess as easy as pssible, n the main page fr mdifying surveys (MdifySurvey.asp), there is a small "PIPE" buttn next t each questin. Clicking n this buttn brings up a small windw that cntains the text f the tken t use t pipe the answer t that questin. Simply cpy the tken text, and then paste it int the desired lcatin n any subsequent questin. It is imprtant t nte that piped answers must always riginate frm an earlier page in the survey. T pipe answers int questin answers paste in the tken text same as described abve, int the "rw" r "clumn" text field. This can be dne with the fllwing questin types: all matrix ptins/checkbx/textbx, single select ptins/checkbxes/drpdwn. Fr a cmplete list f tkens fr yur survey click the "TOKEN REFERENCE" link in the left blue navigatin n the mdify survey page. DYNAMIC TOKEN PIPING: ###DT=DynamicTkenID click the "cnfigure dynamic tkens" buttn n the survey ptins page t add dynamic tkens, then type in r use the menu t place the tken where ever yu want the text replacement t ccur. USER DATA PIPING: ###UD=FirstName, + all user data fields, click the User data tken menu fr all available user data tkens t autmatically add, r type in the tken where ever yu want the text replacement t ccur. New piping ptins allws piping int matrix clumns and rws. New replacement text allws placing User Data tkens, list data tkens, r Hidden field tkens int the survey questins, rws, clumns r answers. On the mdify survey page in the left clumn margin area there is a list f all tkens that yu can insert int a survey which will autmatically be replaced with the tken's value when the respdent is taking the survey. This includes piping, hidden fields, user data and list data that can be pre-ppulated thrughut the survey. T add a tken t a survey just cpy and paste the tken int the survey wherever yu want the text replacement t ccur. Advanced Validatin Besides the standard built in questin validatin which sets a questin as required, r sets the minimum r maximum value, there is als ptins fr adding advanced custm validatin. Client side validatin is when a client brwser actin triggers a javascript t check the answer input, and if the input des nt match the requirements, a pp up dialg bx with an alert message is shwn. Primary Validatin: Primary Validatin is advanced custm javascript that can be cnfigured n an individual survey questin answer r sub item answer. T use primary validatin, n the "edit item" page, click the "Advanced Validatin" buttn that is in the "Validatin Optins" sectin f the item. On the primary validatin page yu can select a functin that has already been created, and then type in yur custm alert text, then click "Add", then "Save". T remve primary validatin click the "remve" buttn next t the validatin under the "current saved validatins" bttm prtin f the page. Secndary Validatin: Secndary Validatin is when yu want t validate the answer f ne questin against anther questin in the survey. T create secndary validatin, click the "Advanced Validatin" buttn n the edit item page. Then click "Secndary validatin" link at the tp right. Then select the first questin t validate against, and the answer cnditinal lgic, then the secnd questin. Then click save. T remve secndary validatin click the delete buttn next t the validatin in the list. Previewing Surveys All surveys with ne r mre items can be "previewed" by actually taking the survey. After taking the survey, simply click the "Clear" link n the main list f surveys t delete the respnse, and yu may cntinue mdifying the survey as desired. Survey testing and previewing is dne by taking the actual survey t ensure that yu are testing the exact same survey that yur end users will be taking. Page 15 f 38

16 There are tw ways a user may "preview" a survey. The first is n the main list f surveys, click the title f the survey, which will display the survey in a new windw, just as yur users will see it. The secnd way is t click n the "Deply" link n the main list f surveys, which will give several ptins fr hw t deply the surveys. Yu may use any f these links t test the survey. After previewing a survey, the respnse is written t the database. When attempting t mdify a survey with respnses, a message is displayed at the tp f the page fr mdifying surveys indicating that the survey has at least ne respnse, and therefre is subject t nly limited mdificatins. Alng with this message is a link that yu can use t clear the respnses t this survey, which will then allw full mdificatins. Managing Surveys The main survey page displays a list f all surveys. Fr a user with Create permissin, this list represents all surveys that they wn. Fr a user with Admin permissin, this page displays all surveys in the system. All surveys are always in ne f three states: Design, Open, r Clsed. When a survey is first created, it is in Design mde while the wner creates the survey. Once a survey is ready t launch, it is launched either by clicking n the Launch buttn n the main survey design page, r by clicking the small "OPEN" buttn t the left f the survey in the survey list page. Once a survey is launched, the status f the survey changes t Open. Nte that a survey can be launched nly if it cntains at least ne answerable questin. A survey can be tggled between Open and Clsed by using the small buttns t the left f each survey n the main list f surveys. Once a survey is launched, it can be fully edited nly when the survey des nt have any respnses. Once a single respnse is recrded fr the survey, nly limited mdificatins can be made t the survey. Editing the entire survey is n lnger pssible because changing the structure f a survey that has existing respnses wuld cause the reprting t fail. On the main list f surveys, a user can manage each survey using the buttns t the left and right f each survey in the list. Frm this page, the user can manage survey ptins, preview the survey, set the status f the survey, analyze the survey results, deply a survey, and delete a survey. Page 16 f 38

When clicking n the survey link, the user is directed t a page t enter the randm cde, if there is n cde they simply click the checkbx that says "I d nt have a cde", and a cde is generated autmatically fr them fr updating the respnse later. In additin t the pt ut feature, there is als the "survey decline" link, which when clicked recrds in the database that this respndent declined t take the particular survey, but are nt put n the glbal pt ut list. Aut Lgin fr 3rd party Lgin Required surveys The Aut Lgin page is used nly fr 3rd party systems which send a username, passwrd and survey ID t the page AutLgin.aspx t autmatically lgin a user t take a survey. If the user des nt exist, it creates ne with the username and passwrd passed in, at the lwest user rle level (can take surveys nly). If the user des exist, it uses that user ID frm the database t recrd the respnse under. This aut lgin page is used t bypass the nrmal lgin page frm anther applicatin. The ther applicatin has t send the paramaters "un", "pw" and Page 27 f 38

28 "sid" t the AutLgin.aspx page in the query string. The "un" must equal the username, the "pw" must equal the passwrd, and teh "sid" must equal the survey ID (encrypted). Once the user is autmatically lgged in, they are redirected immediately int the survey. Reprt Filters Reprt filters prvide the ability t limit the set f respnses in the reprts based n custm filter criteria. Fr example, if a survey cntains a questin asking users if they have a driver's license, a reprt filter culd be created t nly display the respnses where the user answered "Yes" t this questin. There are three types f reprt filters: Date, Questin, and User Attribute. Date filters limit the respnses t nly the dates within the specified range. Questin filters limit the respnses based n answers t questins in the survey. User attribute filters limit the respnses based n attributes f the user wh tk the survey, such as cmpany, lcatin, name, custm data, r any ther user field. Questin filters vary depending n the type f questin selected. Answers can be set t be equal t a respnse, cntain a respnse, r, in the case f number and date questins, be greater than r less than a certain a value. One r mre reprt filters may be applied t a survey. All f the filters are cmbined using "AND" lgic, meaning that all filters must evaluate t "True" in rder fr a respnse t be included in the results. If a reprt has filters, the filters are applied t the verview reprt, the individual reprt, the text reprt, and the data exprt functinality. When wrking with a reprt with many questins, yu may want t run multiple reprts with varius cmbinatins f filters. Rather than deleting filters, yu can set any filter t active r inactive n the page that displays the main list f reprt filters. Results Sharing Results sharing prvides the ability t share yur survey results with thers. Recipients f reprt shares d nt have t lg int the applicatin, and can be granted very granular access t survey reprts and functinality. Page 28 f 38
