5 Sessions ACM Certification Review: Case Management Process and Practice Debra L. Blevins PART 1: The scope of services and standards of practice provide social worker and nurse case managers with a guide to engage in the practice of case management. Understanding the parameters that a case manager practices assures a uniformity of services delivered to patients in need and assists the case manager in performing the duties required within health care delivery systems. This session is designed to provide the practicing case manager with a review of case management practice standards in accordance with American Case Management Association guidelines and ACM certification. It will provide intensives for focused review in domains of case management practice including screening and assessment, planning and care coordination/intervention/transitions. PART 2: The scope of services and standards of practice provide social worker and nurse case managers with a guide by which they can engage in the practice of case management. This session is designed to provide the practicing case manager with a review of case management practice standards which can be applied to case scenarios. It will provide intensives for focused review of the evaluation domain of case management practice. The session will include case management scenarios presented in an interactive format allowing attendees to apply information gathering, assessment, decision-making and problem-solving skills. 1. Describe the three domains of practice utilized by the health delivery system case manager. 2. Apply the essential job functions to the role of a health delivery system case manager. 3. Describe clinical and psychosocial assessment and intervention methods used in case management. 4. Utilize analytical skills to assess and plan care for the health delivery system patient. Session 1 Inspector Gadget: The Psychology of Living with Cardiac Disease and Cardiac Devices Samuel Frazer Sears Jr., Ph. D. The patient experience involves managing multiple co-morbidities and a psychological adjustment process. This talk focuses on the psychological and patient experience of cardiac events and recovery processes. 1. Assess the inter relationship research between cardiac events, patient engagement and behavior, psychological health and outcomes. 2. 6 Session 2 LGBTQ Fundamentals: Exploring Terminology and Understanding Identities Elizabeth Kahn Education Director Mazzoni Center Philadelphia, Pennsylvania SESSION ABSTRACT: The LGBTQ fundamentals training provides you the opportunity to engage with a basic understanding of the history and development of gender and sexual identity with the goal to inspire an increased sense of self-awareness of one s own understanding of LGBTQ identities. This training will improve your competencies for assisting and treating LGBTQ client populations in order to increase patient/client safety and satisfaction and reduce healthcare inequity leading to disparities. 1. Demonstrate knowledge and consideration of LGBTQ terminology, language and identities and apply LGBT-specific culturally sensitive language in your practice. 2. Recognize your influence to create safe spaces for LGBTQ patients. 3. Reflect on current inclusivity practices and strategize an action plan to best support LGBTQ patients. Session 3 Suicide Prevention: A New Approach Kristopher Thompson, LCSW Associate Executive Director for Crisis Services Lenape Valley Foundation Doylestown, Pennsylvania SESSION ABSTRACT: In this course, you will learn about new suicide prevention developments, system approaches that have encouraging results and the use of technology to enhance suicide prevention. Current myths about suicidal behavior will also be debunked. This course will provide an overview of behavioral health challenges underlying suicidal behavior and the notion of suicidality being its own diagnostic category. In conclusion, considerations surrounding the Involuntary Commitment 302 process will be reviewed. 1. Recognize the warning signs of suicide and have a basic understanding of the best practices being utilized in prevention. 2. Acquire a basic understanding of behavioral health challenges underlying suicidal behavior. 3. Review the Involuntary Commitment 302 process in PA and when to utilize it. Session 4 Elder Law Case Studies Bryan Adler, Esq. Associate Attorney Rothkoff Law Firm Cherry Hill, New Jersey SESSION ABSTRACT: Most elder law attorneys handle a wide range of legal matters affecting an older or disabled person, including issues related to health care and long-term care planning. This discussion for an expert associate elder law attorney will center around financial, legal and social planning for families with a loved one in nursing home care. 1. Identify when public benefits are available. 2. Discuss protections available to spouses of Medicaid beneficiaries. 3. 7 Session 5 Sepsis and Addiction Julia Miller, MSW Manager of Inpatient Care Management St. Luke s University Health Network Bethlehem, Pennsylvania SESSION ABSTRACT: St. Luke s Sepsis Treatment & Addiction Recovery (STAR) Program was initiated after the recognition of service gaps for individauls with history or current use of intravenous opiods and who were in need of long-term intravenous antibiotics. Patients entering the hospital system would find that there was not community-based treatment options to receive medical care and addiction treatment simultaneously. The only option was to remain in the hospital for six to eight weeks to complete their IV antibiotic treatment. Afterwards they could enter addiction treatment post discharge. More than a year of collaboration and coordinating services with community-based agencies, hospital services, payor sources, regulatory bodies and local and state government agencies has resulted in the ability to provide these services to individuals seeking medical care and sobriety within an addiction treatment facility. 1.
