3 Overview of Exhibitor Program ACHI is the premier national association for community health, community benefit and healthy communities professionals. It delivers education, professional development, peer networking and practical tools that help community health leaders expand their knowledge and enhance their performance in achieving goals. ACHI s mission is to strengthen community health through education, peer networking and the dissemination of practical tools. ACHI is a personal membership group of the American Hospital Association. There are more than 800 members in 46 states and international. Payment: Under the terms of this agreement, the exhibitor agrees to pay the total fee within 60 days of receipt of an invoice. It is understood that failure by the exhibitor to remit balance due by said date shall result in a 1.5% interest charge per month on late invoices. If payment is not received, then booth will be declared null and void so that the booth can be reassigned. 2. Cancellation Policy: Any exhibitor who cancels all or part of purchased exhibit space prior to November 30, 2014 shall receive a full refund, less a $100 administrative fee. Any exhibitor who cancels all or part of purchased exhibit space or sponsorship between December 1 and December 31, 2014 will receive a 50% refund. If cancellation in whole or part is made after December 31, 2014 the exhibitor shall be liable to ACHI, as liquidated damages, for the unpaid balance of the total value of the exhibit cancelled. Cancellation requests must be submitted in writing to the address below. 3. Authorization: Exhibitor agrees to abide by the ACHI 2015 exhibitor terms of agreement on page 9 of this application and made a part of this agreement by reference and fully incorporated herein. If this application and agreement has not been received, properly signed, and accompanied by payment in full as specified in the instructions, it may be declared null and void so that exhibit space or sponsor benefits may be reassigned.
