RESPONSIBILITIES Bachelors Level Social Work Practice 1. Assess the psychosocial needs of patients and families in order to eliminate barriers to treatment, enhance patient s health status, increase patient/family satisfaction and improve utilization of resources. 2. Provide discharge planning assistance that may include coordination of durable medical equipment, home health services, home care services, determination of placement needs or referrals for community resources, transportation assistance, financial assistance and/or other necessary follow-up. 7. Facilitate nursing home placement and other facility transfer. 8. Advocate, mediate and negotiate in collaboration with the health care team to formulate a cohesive plan for maintaining or enhancing patient s health status, improving social supports and moving patients safely into less restrictive, less costly levels of care. 9.
