3 Generic Job Description for a Bachelors Level Social Worker (Organization) Last Revision Date: TITLE: Bachelors Level Social Worker CODE/CLASSIFICATION INFO: DEPARTMENT: Social Work SUMMARY DESCRIPTION The social worker, in collaboration with other members of the healthcare team, is responsible for assessing the patient s psychosocial needs and for planning, implementing and coordinating care. RESPONSIBILITIES Bachelors Level Social Work Practice 1. Assess the psychosocial needs of patients and families in order to eliminate barriers to treatment, enhance patient s health status, increase patient/family satisfaction and improve utilization of resources. 2. Develop a plan of intervention in collaboration with the healthcare team. 3. Implement a plan of care and monitor patient s response. 4. Provide individual and family support as well as social work intervention under the supervision of an MSW as it relates to crisis management, healthcare decision-making, ethical/legal concerns, elder/child abuse, domestic violence, competency, determination of placement needs, financial problems, compliance issues, as well as other psychosocial barriers to maximizing health status. 5. Provide patient, family and staff with available community resource options and supply appropriate linkage to desired services. 6. Provide discharge planning assistance that may include coordination of durable medical equipment, home health services, home care services, determination of placement needs or referrals for community resources, transportation assistance, financial assistance and/or other necessary follow-up. 7. Facilitate nursing home placement and other facility transfer. 8. Advocate, mediate and negotiate in collaboration with the health care team to formulate a cohesive plan for maintaining or enhancing patient s health status, improving social supports and moving patients safely into less restrictive, less costly levels of care. 9. Provide patient and family education. 10. Participate in process improvement in evaluating patient outcomes. 11. Identify barriers in service delivery systems and report these to Social Work Department for system intervention. 12.
