3 Keynote Address: ACMA and The Future of Your Professional Practice L. Greg Cunningham, MHA Chief Executive Officer American Association Little Rock, AR Session Abstract: The last ten years have been a time of growth and increasing importance for the role of nurses and social workers in health care. The case manager s role will continue to evolve dramatically, and ACMA has become an important and active association both locally and nationally. Establishing ACMA and as a crucial partner in health care innovation is both of strategic and outcome orientation. Using the last ten years of ACMA s national research data and the evident trends (staffing, structure, function and national policy), the CEO of our national association will share his vision concerning the changing health care/case management environment and the importance of ACMA s strategies to both advocate for and support its members. Session 2: Positioning Your Hospital for Evolution in Health Care Linda Van Allen, RN, BSN, PHN, ACM Senior Director, Performance Management and Innovation Tenet Healthcare Corporation Dallas, TX Session Abstract: We are bombarded with all of the changes going on in health care today. Not only do we need to find ways to do more, we also need to find ways to work differently in order to survive and thrive in the evolving environment. This session will review ways that hospital case management is working to address the challenges by partnering with community providers and testing new ways of doing work to reducing readmissions and better coordinate care. Attendees will come away with ideas and tools to use in their hospital to help position them for the future. 1 Understand key elements of health care reform that impact hospital case management services 2 Describe the case management models and skills needed to successfully manage the new challenges that extend beyond the traditional hospital walls 3 Evaluate next steps required to work differently going forward Session 3: Health Care 2013 in the Golden State Impact for Patricia Blaisdell, MA, MS Vice President, Post Acute Care Services California Hospital Association Sacramento, CA Session Abstract: The passage of the Affordable Care Act (ACA) in 2010 set into motion wide-ranging changes to the financing and delivery of health care services, including expanded access to care and an emphasis on care coordination, quality of care and patient outcomes. The current presentation will provide an update on the implementation of ACA with a particular focus on implications for case managers. An update of other significant state and federal issues will also be presented. 1 Understand current status of ACA and federal health care reform implementation 2 Identify and understand the components of health care reform that affect hospitals and other providers, including the post-acute care continuum 3 Discuss the implications of health care reform for case managers
4 Session 4: Understanding Latina/o Communities in Context: Bridging Cultural Gaps Between Service Providers and Community Members Miguel E. Gallardo, Psy. D. Associate Professor of Psychology Graduate School of Education and Psychology Pepperdine University Irvine, CA Session Abstract: Latinos constitute the largest and fastest growing underrepresented group in the United States. Factors identified as barriers for this patient population in the utilization of health services include high rates of uninsured persons, social and language barriers, and the complexity of the delivery system. The lack of bilingual and bicultural providers in particular has been cited as a major barrier for Latinos seeking mental health services. Accounting for clients values, culture and context is an ethical responsibility. As we continue to expand our understanding of culture and its multiple dimensions, our ability to negotiate service provision in culturally responsive ways becomes imperative. This presentation will address ways that attendees can enhance their capacity to build relationships with Latin communities within the context of health care service provision. 1 Identify their own culturally based beliefs, and understand how these beliefs impact service provision to Latin communities 2 Expand the definition of culture beyond race and ethnicity only 3 Enhance their ability to build relationships and communication with Latin communities Session 5: Panel Discussion Innovative Tools to Prevent Avoidable Readmissions Tom Greely, MD Medical Director Inpatient John Muir Health Walnut Creek, CA Zenobia Collins Johnson, RN, BSN, CLNC, CPHIT, MBA Project Director, Readmission Reduction Care Management Dignity Health San Francisco, CA Panel Discussion Part 1: The LACE scoring tool is a key predictor for a patient s readmission. LACE is an acronym for Length of Stay, Age, Co-morbidities, and Number of ED visits. The patient is scored on discharge, and the higher the LACE score, the higher the probability is for readmission. Learn how John Muir Health is using this tool to refer patients to various post-discharge programs such as Care Transitions or Outpatient. Panel Discussion Part 2: Potentially preventable rehospitalizations are frequent, costly and burdensome for patients and families and extremely frustrating to providers. We must begin to address the complexity of variables that contribute to readmissions by using a multi-focal approach. The speaker will discuss Dignity Health s Readmission reduction program including their use of the Louisiana eq solutions tool and the strategies utilized by their care providers. 1 Understand the use of the Prospective Readmission tool and its use in triage to discharge programs 2 Utilize data to identify opportunities and specific interventions to reduce unnecessary readmissions 3 Identify strategies to remove barriers to your readmission reduction efforts
