3 PRE-CONFERENCE SESSION: RULES AND REGULATIONS OF THE FLORIDA STATE BOARD OF NURSING NEW MANDATORY CONTINUING EDUCATION REQUIREMENT FOR NURSES Charles Illian, RN, BSN Director of Clinical Education Orlando Health and Rehab Center Orlando, FL SESSION ABSTRACT: This session is being offered to Florida Chapter members who are RNs and will be required to have completed this course, starting in In keeping with the Florida guidelines for a Laws and Rules course, this module includes content on Chapters 456 and 464 of the Florida Statutes and the rules in Title 64B9 of the Florida Administrative Code. 1. Verbalize specific laws and rules related to the practice of nursing and nurse assisting, and identify the pertinent levels of nursing practice in the state 2. Discuss the general requirements for continuing licensure in the state, and differentiate between ethical and legal practices 3. Discuss the process for discipline related to nursing practice, and create a professional plan for career maintenance and development within the limits of the law SESSION 1: CARE TRANSITIONS AND CO-OPETITION IN THE ERA OF ACCOUNTABILITY Kevin W. O Neill, MD, FACP, CMD Chief Medical Officer Brookdale Senior Living Sarasota, FL SESSION ABSTRACT: This session will discuss the triple aim of the Centers for Medicare and Medicaid Services (CMS) and the need for better communication among acute and postacute care (PAC) providers to reduce avoidable readmissions. The adverse impact of ineffective care transitions will be discussed, and the evidence-based innovative care models will be presented. This session will stress the importance of collaboration between acute and PAC providers. Key steps to get leadership buy-in and build the partnership relationships will be identified. 1. Discuss the triple aim of CMS and the paradigm changes taking place in the American health system 2. Identify the evidence-based innovative care models that improve care transitions and reduce avoidable hospital readmissions 3. Identify next steps for building quality partnerships between hospitals and PAC providers and among PAC providers KEYNOTE SESSION 2: FINDING THE ROAD TO THE EMERALD CITY OF OZ Stefani Daniels, RN, BSN, MSNA, ACM President and Managing Partner PHOENIX: The Hospital Case Management Company Pompano Beach, FL SESSION ABSTRACT: The continuing evolution of hospital case management is both scary and exciting. On the one hand, assumptions previously held about traditional functional models are being challenged as the health care environment is moving from volume to value. At the same time, we are exploring models to extend contemporary hospital case management practice principles beyond the boundaries of the hospital walls and into the community. Either way, hospital case management as we know it must change to reach the Emerald City of Oz. 1. Define strategies to demonstrate the value of case management to the C-suite, the health care team and the health care system 2. Explain how to capture the gaps that lie within the health care system and their impact upon effective transition planning 3. Differentiate between the practice of case management and the function of utilization review SESSION 3: THE CONTINUED ROLE OF CASE MANAGEMENT IN HOSPITAL REIMBURSEMENT Kathy Reep, MBA Vice President, Financial Services Florida Hospital Association Orlando, FL SESSION ABSTRACT: This session will look at the regulatory and legislative changes impacting the role of the case manager value-based purchasing (VBP), including the new efficiency measure; readmissions; hospital-acquired conditions (HAC); and payment bundling and the resultant implications for hospitals. The session will also look at discharge decisions and their impact on hospital efficiency and bundled pricing.

We help everyone get to the yellow brick road and teach them how to look for their rainbow, where surely it will be better on the other side. Not stopping to consider ourselves vulnerable, we either fail to see the negative that surrounds us or ignore behaviors that can hurt us, whether at work or not. We re great, beyond compare, in taking care of others... yet repeatedly fail to take care of ourselves or each other. Do we not know how? Are we too busy to care or too frightened to try? This session is about us as case managers and individuals and how to be smarter about taking care of ourselves and of each other. 1. Explore the negative emotions found within health care and how aggression impacts the health care team 2. Demonstrate tactics to deal with some of the most common negative behaviors seen in the health care setting 3. Define steps to protect ourselves and how to take better care of each other SESSION 5A: TALKING THE TALK: COMMUNICATING EFFECTIVELY WITH PHYSICIANS Frank B. Bellamy, RN, MSN, ACM Interim Director, Outcomes Management St. Joseph Mercy Health System Pontiac, MI SESSION ABSTRACT: Many elements contribute to the success of the case manager, including effective physician communications. Both the settings and the methods used by the case manager impact the outcome and influence the benefits and risks of the communication process, including patient and family needs, hospital facility needs and physician satisfaction. From the physician s perspective, where they trained (abroad or in the U.S.), socialization with other physicians and among other caregivers, and a physician s priorities or non-priorities are extremely valuable for the case manager to understand. This informative session will provide insight into possible reasons for successes and non-successes in communicating with physicians; help identify when you are over your head; and define the do s and don ts to achieve better outcomes and improve patient safety. 1. Identify elements of the case manager s work that rely on effective communication with physicians 2. Describe how physicians are trained and socialized 3. Initiate and respond to interaction with physicians in a variety of settings SESSION 5B: ALIGNING PEDIATRIC CASE MANAGEMENT WITH THE NEW REIMBURSEMENT MODEL: A CHANGE IN CULTURE Hossain Marandi, MD, MBA, CPE, CPHQ, FAAP Director of Medical Affairs BayCare Health System Tampa, FL SESSION ABSTRACT: This session will provide an important and informative overview of recent changes to reimbursement guidelines that will have an impact on the pediatric hospital population. Of equal importance, we will discuss how these reimbursement changes are expected to affect current case management processes, and explore potential changes to case management and hospital culture expected to occur as part of the realignment process. Our speaker will also present process improvement initiatives developed specifically to align case management with new pediatric reimbursement guidelines. 1. Explain critical changes to pediatric hospital reimbursement 2. Recognize anticipated effects to current pediatric hospital culture related to case management processes 3. Define specific case management process improvement initiatives to promote positive transition to new guidelines

ACMA is also a recognized CE provider for nursing and social work in multiple other states including, but not limited to, FL, MN, KY, and TX.
