Groundbreaking Techniques for Optimizing Patient Flow Management through Effective Discharge Planning, Enhanced Patient Care/ Satisfaction, and an Efficient Collaborative Strategy The new era of healthcare reform has greatly impacted hospitals and health systems, forcing them to implement new measures addressing patient flow management to improve and streamline hospital throughput. The influx of the newly insured entering the healthcare system presents greater challenges to hospitals and the necessity to streamline flow and capacity has become enormously vital. As this landscape continues to evolve and dramatically transform, there is a huge shift towards hospital wide collaboration and throughput in order to achieve these goals. Optimizing the ED, decreasing patient wait times, and enhancing care coordination are key components to moving the patient quickly, efficiently and safely through the hospital system. We have created a high level forum featuring knowledgeable leaders and executives from Hospitals and Health Systems who will share their perspectives, valuable insights and expertise on how to be best equipped for the rapidly evolving landscape of Patient Flow Management. Attendees will benefit from learning about best practices and strategies that have been deployed to address the challenges presented under the Affordable Care Act impacting hospitals and health systems in effectively managing patient flow. This exclusive event targets senior level executives in order to maximize educational and networking opportunities. This talk will focus on part of the strategy that was implemented as well as various models of open and block scheduling that have been utilized to manage patient flow. Chief, of Anesthesiology Professor & Chairman, of Anesthesiology Boston University School of Medicine 9:00 essential Components for Implementing Care Coordination to Optimize Patient Flow Management This presentation will share key components for implementing quality care coordination to effectively optimize patient flow management and throughput. Attendees will benefit from learning about enhanced care coordination procedures that safely facilitate patient throughput. The session will examine in depth strategies on how to effectively achieve these results. Michelle Hawkins, DNP, MBA, MSN, RN, CCM Director, Care Management & Population Health Integration Johns Hopkins HealthCare 9:45 Networking Break & Refreshments 10:15 Patient Flow Improvements at (SHC) is experiencing tremendous growth, which leads to hospital and Emergency overcrowding. It also negatively impacts the patient experience and our ability to support increased demand for services. We will describe how SHC applied lean management principles to improve the patient flow. Marlena Kane Executive Director, Performance Excellence & Medicine Services Timothy Seay-Morrison Administrative Director, Clinical Support Services 11:00 strategies to Reduce Patient Length of Stay through Efficient Care Coordination and Discharge Procedures One of the top Clinical Care priorities for most organizations is to decrease all-cause readmissions. Hospitals across the country are chasing a host of initiatives and interventions to impact readmission rates. There is no doubt that the transition from hospital to home has become more complicated for many patients as well as for the care facilities. Ensuring timely follow up post discharge, attendance at scheduled physician appointments, purchase of medications, compliance with diet, medications and wound dressing orders are just a few of the issues that can lead to a readmission if the patient or caregiver do not follow the discharge instructions. A few organizations have demonstrated success in decreasing readmission rates by as much as 50%. This is one replicable strategy that can positively impact readmissions rates. Shelly Buck, DNP, MBA, CCRN, NEA-BC Chief Operating Officer & Chief Nurse Executive Bon Secours Baltimore Health System 11:45 innovative Approaches to Improve and Prevent ED Overcrowding and Boarding ED crowding, more often than not, is by design, not by accident. Yes, you read that correctly. This session intends to demonstrate why hospitals create ED crowding as a business model. Understanding the drivers of crowding allows one to work with hospital administration to address this issue more effectively. However, sometimes the solution rests with the clinical managers in the ED. This means one must understand and visualize the conceptual differences between ED patient demand and ED bed capacity. The presentation will intersect these economic and operational challenges to fixing crowding looking at what administrators and ED managers can do to solve this chronic problem.
