3 SESSION 1 (KEYNOTE ADDRESS): ACMA AND CASE MANAGEMENT - THE FUTURE OF YOUR PROFESSIONAL PRACTICE L. Greg Cunningham, MHA CEO American Case Management Association Little Rock, AR SESSION ABSTRACT: The last ten years have been a time of growth and increasing importance for the role of nurses and social workers in health care. The case manager s role will continue to evolve dramatically, and ACMA has become an important and active association both locally and nationally. Establishing ACMA and case management as a crucial partner in health care innovation is both of strategic and outcome orientation. Using the last ten years of ACMA s national research data and the evident trends (staffing, structure, function and national policy), the CEO of our national association will share his vision concerning the changing health care/case management environment and the importance of ACMA s strategies to both advocate for and support its members. 1. Understand the history of the case management practice and ACMA 2. Describe national research based trends regarding historical and future case management practice 3. Correlate and compare individual and organizational preparation for future practice issues SESSION 2: CURRENT HOT TOPICS IN CASE MANAGEMENT Ronald L. Hirsch, MD, FACP, CHCQM Vice President, Regulations and Education Accretive PAS Chicago, IL SESSION ABSTRACT: This session will occur two weeks before the 2015 IPPS Rule is set to take effect. The 2014 IPPS Rule marked the introduction of the Two-Midnight Rule, which caused much frustration and confusion for case managers. In this session, the speaker will review the important aspects of the 2015 Rule, the lessons learned from the MAC Two-Midnight probe and explore other current topics that affect the daily work of case management professionals. 1. Understand the 2015 IPPS Rule 2. Discuss key preparations for the resumption of medical necessity audits 3. Identify new developments in case management SESSION 3A: PALLIATIVE CARE: WHY IT IS NEEDED AND WHY WE WILL NEED MORE Michael A. Salvatore, MD, FACP Physician Advisor Case Management Beebe Health Care Lewes, DE SESSION ABSTRACT: Attendees will discuss the ethics that require physicians to provide palliative care and the process to establish ethical guidelines that can help families make difficult decisions. Through this, the session will explore the role of palliative care in not only reducing patient suffering, but improving family satisfaction. 1. Explain the use of ethics to guide physician behavior 2. Understand the use of ethics to support acceptance of palliative care by families 3. Explore the use of MOLST form in palliative care SESSION 3B: ACHIEVING SUCCESS UNDER MARYLAND S NEW DEMONSTRATION WAIVER Michael B. Robbins, MBA, FHFMA Senior Vice President, Financial Policy & Advocacy Maryland Hospital Association Elkridge, MD SESSION ABSTRACT: This session will provide an overview of Maryland s new Medicare hospital waiver and the key metrics to be monitored to ensure continuation of the waiver. The program will include an overview of the key quality-based programs that are part of the new demonstration, and how improved care management within the hospital, as well as with community-based providers, will ensure success in meeting the three-part aim requirements of the new waiver. 1. Understand the key performance metrics under Maryland s new waiver 2. Identify what role case managers can play in achieving their organization s success under the new waiver 3. Share best practices with peers on programs that are already under way in Maryland s hospitals to improve care management and care coordination and achieve success under the new waiver

4 SESSION 4A: ETHICAL AND CULTURAL ISSUES IN A HEALTHCARE SETTING FOR SOCIAL WORKERS Veronica E. Cruz, MSW, LCSW-C Forensic Social Worker Office of the Public Defender Rockville, MD SESSION ABSTRACT: This workshop will explore the numerous ethical and cultural issues that can arise in a health care setting for social workers. The diverse and complex role of social workers within a health care setting will be explored. An emphasis will be placed on professional collaborations and maintaining authenticity when working within a treatment team. There will be an in-depth discussion of the Social Work Code of Ethics and some of the major principles of the Nursing Code of Ethics. Participants will be able to identify issues that can affect treatment delivery. Various issues will be explored including cultural differences, ethical dilemmas and moral dilemmas. An emphasis will be placed on best practice techniques for ethical decision-making, allowing participants to engage in a variety of different intervention techniques. This is an interactive workshop where case vignettes will be presented and participants will work in a group setting. 1. Articulate the Social Work Code of Ethics and how it is used in a health care setting 2. Increase the understanding of various ethical decision making models that can be used in a health care setting 3. Participants will be able to identify best practice techniques and relate it to one s own clinical setting 4. Explore the connection between cultural competency and effective treatment delivery SESSION 4B: ENGAGING PHYSICIANS IN UTILIZATION MANAGEMENT Kevin Smothers, MD Chief Medical Officer Shady Grove Adventist Hospital Rockville, MD SESSION ABSTRACT: Case managers and physicians partnering together can help patients and hospital systems negotiate the tremendous changes occurring in health care today. This session will discuss ways case management staff can engage physicians in the utilization management process, certifying the patient s admission status, reducing readmissions, and increasing patient, physician and staff satisfaction. 1. Describe at least one effective method to engage physicians in the care management process 2. Describe recent changes in health care that affect physician practice and engagement with care management staff 3. Describe ways care management staff can partner with physicians to reduce readmissions SESSION 5: ETHICAL AND CULTURAL ISSUES IN A HEALTHCARE SETTING FOR PROFESSIONALS IN THE CASE MANAGEMENT FIELD Veronica E. It will build on the information from Session 4A, but does not require attendance of that session as a pre-requisite. The diverse and complex role of a nurse case manager and social work case manager within a health care setting will be explored. An emphasis will be placed on the importance of professional collaborations. There will be a discussion on the code of ethics for the field of social work and nursing, exploring the similarities and differences among the two professions while encompassing a larger picture of the ethical and cultural issues both professions face and work on together. Participants will be able to identify issues that can affect treatment modalities, services, and client/practitioner differences. Issues explored will be: cultural differences, use of translators, ethical dilemmas, moral dilemmas and various ethical decision making models. 1. Articulate the similarities and difference between the Social Work and Nursing Code of Ethics 2. Demonstrate an understanding of various ethical decision making models that can be used in a health care setting 3. Explore the connection between cultural competency and effective treatment delivery 4. Participants will be able to identify essential clinical skills needed to enhance patient care
