SESSION 1: 2014 IPPS FINAL RULING: BEST PRACTICE RECOMMENDATIONS Ralph Wuebker, MD, MBA Chief Medical Officer, Audit, Compliance and Education Team Executive Health Resources, Inc. Newton Square, PA SESSION ABSTRACT: Within the last year, the Centers for Medicare and Medicaid Services (CMS) have proposed two separate rulings (1455-P and 1599) that impact inpatient status determinations and procedures for managing appeals. This session will provide insight on the final ruling and most recent updates, with a discussion of what the regulations mean in terms of potential course adjustment and how they impact patient quality of care. The speaker will address the two-midnight rule, observation v. inpatient status, certification, rebilling and documentation requirements. 1. Summarize key points of the 2014 IPPS final ruling and discuss three consequences for providers and patients 2. Discuss criteria for determining observation v. inpatient status based on the two-midnight ruling 3. Explain the importance of accurate physician documentation and a robust utilization review process to best defend against denials SESSION 2: THE VIRGINIA WAY: MEDICAID EXPANSION IN VIRGINIA Ann Harbour, MPA Associate Vice President, Government Relations InTotal Health, An Inova Health Plan Vienna, VA SESSION ABSTRACT: This presentation will detail efforts to expand Medicaid in Virginia, with a focus on the current state Medicaid program and the commonwealth s plan for development and expansion. The speaker will discuss the politics of expansion and how to strengthen the business case for Medicaid expansion. Finally, the session will review opportunities for case managers in the changing health care environment. 1. Identify barriers to Medicaid expansion 2. Articulate the business argument for expansion 3. Cite opportunities for case managers in the current health care environment SESSION 3: MEDICAL ETHICS AND Earl D. White, II, MD Medical Director, Sentara Center for Healthcare Ethics Sentara Healthcare Hampton, VA SESSION ABSTRACT: This presentation will provide an overview of ethical background and theory in health care. The speaker will utilize ethical dilemmas and case consultation in order to illustrate the challenges and key points in health care ethics. Attendees will gain practical tips on identifying and resolving ethical dilemmas and leveraging case management to improve ethics at their facilities.

SESSION 3 (CONT): 1. Describe the importance of ethical theory in ethics case consultation 2. Explain the nature of an ethical dilemma and what to expect in ethics case consultation 3. Identify opportunities for case management to assist with resolving ethical dilemmas SESSION 4: POPULATION TOOLS & PROACTIVE DISEASE MANAGEMENT Robert Fortini, PNP Vice President/Chief Clinical Officer Bon Secours Virginia Health System, Inc. Midlothian, VA SESSION ABSTRACT: Population health is a rapidly growing area of research and development that focuses on how to effectively use tools to identify populations at risk and efficiently risk-stratify that population. This discussion will cover some of the population health tools used by Bon Secours. The speaker will discuss specific registries and their usefulness, a simple prediction modeling tool and patient outcomes, as well as a fully automated outreach tool. 1. Discuss how population tools can be used in health care 2. Explain the benefits of a proactive approach to chronic disease management 3. Describe the differences between a fully automated approach and a semi-manual approach to population health SESSION 5: THE PATIENT EXPERIENCE Mark Katz, MD, MPH Chief Medical Officer/Cardiac Surgeon Specialist Bon Secours Heart and Vascular Institute Richmond, VA SESSION ABSTRACT: The patient experience is essential to Medicare reimbursement and a priority for the hospital experience. This presentation will review how hospitals can gain perspective on achieving a positive patient experience, which is based on good communication and effective patient care with positive outcomes communicated to the patient. Finally, the speaker will discuss the role of case management in the patient experience and how to identify opportunities for improvement. 1. Describe a positive patient experience 2.
