Ensuring timely follow up post discharge, attendance at scheduled physician appointments, purchase of medications, compliance with diet, medications and wound dressing orders are just a few of the issues that can lead to a readmission if the patient or caregiver do not follow the discharge instructions. A few organizations have demonstrated success in decreasing readmission rates by as much as 50%. This is one replicable strategy that can positively impact readmissions rates.
