Faculty Biographies Johann Chanin, MSN, RN Johann Chanin is Director in Product Development and was a member of the team that developed the Physician Practice Connections Patient-Centered Medical Home (PPC -PCMH ) standards, policies and procedures and associated materials. She led the Physician Recognition Program tasks associated with a contracted CMS Medicare Medical Home Demonstration project which involves oversight of the application and evaluation process of 400 practices interested in qualifying as medical homes. Currently, Johann is leading the development of s Specialty Recognition Program. During her tenure at, Ms. Chanin has worked on in Physician Recognition Program operations assisting practices applying for PPC Recognition, reviewing submitted surveys and associated documentation and conducting on-site audits. Ms. Chanin has also worked with the research team, specifically to identify, develop and test potential new content areas for PPC-PCMH. She has broad experience working with physicians and practices interested in participating in a medical home demonstration across the country. Her background as a clinical nurse specialist and health care researcher provide her with a comprehensive understanding of the challenges and measurement issues associated with creating a patient-centered medical home.
