MU Cre 12 Send Patient Reminders This guide will prvide instructins n hw t cnfigure the necessary setup and preferences t enable Enterprise EHR t wrk fr Cre Measure 12: Use clinically relevant infrmatin t identify patient wh shuld receive reminders fr preventive/fllw-up care and send these patients reminders, per patient preference. Final Rule Requirements Mre than 10% f all unique patients wh have had 2 r mre ffice visits with the eligible prfessinal (EP) within the 24 mnths befre the beginning f the EHR reprting perid (RP) was sent a reminder, accrding t each patient s preference, when it has bee n recrded. Cre Objective: Menu Objective: Numeratr: The number f patients in the denminatr wh were sent a reminder per patient preference when available, during the reprting perid. Denminatr: The number f unique patients wh had 2 r mre ffice visits with the EP in the 24 mnths prir t the beginning f the reprting perid. Exclusin: Prviders with n ffice visits in the 24 mnths befre the EHR reprting perid. Objective is shared acrss Eligible Prfessinals: Yes: N: Objective must be recrded during reprting perid: Yes: N: Prerequisites Requires: Patient Query (frmerly knwn as Ppulatin Health Management): Availability v11.2: Enhanced v11.4.1: Cnfiguratin Steps Preferences 1. Review Pt Cmmunicatin Reminder Editable Preference This preference can enable the user t edit r update a patient s preferred methd t receive reminders. Set t Yes if this value is nt captured and sent frm the Practice Management System. Set t N if this value is being sent by the Practice Management System. G t TWAdmin>Preferences>General

2. Review Create Overdue Reminder Task Preference (ptinal) This preference determines if an Overdue Reminder task is autmatically created fr the Planned By prvider when a health management reminder is verdue. Set t Yes if the prvider wishes t receive these tasks. This preference is ptinal fr the measure reprt. G t TWAdmin>Preferences>General 3. Perfrm the fllwing steps t generate patient reminders within the EEHR: Identify the criteria in which t create a list f patients yu want t create reminders fr. Define the cntent f the reminder letters t send t patients. Create the Text Templates that will define the cntent f the reminder letters. G t TWAdmin>Nte Admin>Manage Text Templates (v11) tab Utilize the Pt Cmmunicatin ptin within the TW Mdule drp -dwn menu: Create a Patient Query fr yur defined clinical criteria and assciate an actin set t the ppulatin query that includes bth a cmmunicatin setup using the Patient Preferred Methd, as well as reminders t be added t the patient charts. Navigate t TWAdmin > Patient Query

Click the buttn t create a new query. Enter the title f the new Query Name within the field n the bttm left f the page and then click the yellw crss icn t pen the Clinical Rules Editr.

In the Clinical Rules Editr chse the patient query criteria by clicking n the Criteria links frm the list n the left side f the screen.

Example: Demgraphics Criteria Fill ut required/apprpriate fields and click OK. When finished selecting the criteria click Save and Return -. Clse the windw and navigate back t the Clinical Rules Editr windw. T view the new query - click the Refresh All icn. This will ensure that yu will see yur newly created query. Find and highlight the query and click the Run icn and ensure the query successfully ran. Click the Return t UMP link (upper right crner f screen). Navigate back t TWAdmin > Patient Query and select the Add/Edit Patient Actin Set tab.

Create a new Actin Set, by clicking the buttn. Name the new Actin Set and prvide a descriptin.

Click n the Patient List tab and chse the Patient List Type. Select an Existing List r create a new Patient List Name. Prvide Patient List Descriptin and click the buttn.

Select the Actin Set Prperties tab and assciate the new Patient Query t the Actin Set by clicking the binculars icn. This brings up the Assciate Patient Ppulatin Queries Search. Select the new Patient Query and click the buttn. Click the buttn.

Click the Patient Cmmunicatin tab, select the Cmmunicatin Methd (Patient Preferred Methd is recmmended) & Cmmunicatin Owner, assign Task(s), select the apprpriate Text Template(s), and click the Add buttn. The Cmmunicatin Owner is the user r team that will be the wner f the dcumentatin that is generated fr cmmunicatin. This is the dcument wner that will appear in ChartViewer nce the reminder is created n the patient. Cnsider adding a statement within yur Mail Text Template t encurage patient prtal enrllment. 4. Setup HMP Reminders Actin Set (Optinal Step) Click the HMP Reminders tab, select the Reminder Type, select the Frequency Methd and click the Add buttn.

5. Setup Creating Tasks Actin Sets (Optinal) Click the Tasks tab, select a User/Team t assign the Task and Task Type, enter Task Cmments, define Activate and Overdue Days, create Ntificatin Task (ptinal) and click the Add buttn. Click the Actin Set Prperties tab, review the Actin Set Details and click the Save Actin Set buttn.

Wrkflw Cnsideratins Create internal prcess t capture each patient s preferred cmmunicatin methd fr reminders. The applicatin uses Mail by default if the patient is nt enrlled in Patient Prtal. Establish wrkflws fr calling the patients. Identify which staff is ging t wrk the reminders task t call the patients. Create a new task view t capture these tasks r add them t existing task views fr the apprpriate staff. Task type is Call Patient Cmmunicatin. Establish wrkflws fr mailing letters t patients. Identify which staff is ging t wrk the reminder tasks t mail letters t patients. Create a new task view t capture these tasks r add them t existing task views fr the apprpriate staff. Task type is Mail Patient Cmmunicatin. Cnsider using the Batch Printing Menu fr staff wh will print the reminder letters. Assign Batch Printing Menu t any users that will be printing the reminder letters (this is nt required, but is the suggested wrkflw). The menu dcument is DcsBatchPrint. Create internal prcesses t rute prvider and staff requests fr specific reminder letters and the patient ppulatin wh will receive the reminder letters. Assign Patient Cmmunicatin Dcument Types t ChartViewer Sectin. G t TWAdmin > Chart Admin > Manage Sectins > Select Dcuments > Patient Cmmunicatin Mail and Patient Cmmunicatin Prtal.

Available in v11.4.1, yu can map rderable items t rder reminders frm Ppulatin Health. This is nt required fr MU reprting, but is recmmended t help yu identify which rders can satisfy a particular rder reminder. Assign the Patient Prfile Edit security t users that shuld be able t update the Patient Cmmunicatin Reminder preference fr the patient. Assign the patient security frm TWAdmin > Security Admin > Security Classificatin. Hw EEHR Calculates the MU2 Reprt Numeratr: Fr each unique patient, the system will cunt nce any patient in the denminatr wh has a reminder cmmunicatin prduced during the chsen reprting perid. Reminder ptins include: Print Mail Send t Patient Prtal If a patient s preferred cmmunicatin reminder preference is set t Decline and n reminder is sent, the patient is excluded frm the numeratr. The numeratr will nt include any reminder cmmunicatin flagged as Exclude frm MU Reprting. Denminatr: Fr each unique patient, the system will cunt nce any patient wh has had 2 r mre ffice visits with the EP in the 24 mnths prir t the beginning f the reprting perid. The system excludes frm the denminatr, appintments flagged as Exclude frm MU Reprting in the Appintment Type dictinary. N N Yes N Yes Excluded frm Measure Measure Nt Met Measure Met General Cnsideratins Final Rule cnsideratin: The Final Rule states We believe that reminders shuld be limited t new actins that need t be taken nt f actins that are already taken. Fr example, a reminder t schedule yur next mammgram is a reminder t take actin, while a reminder that yur next mammgram is scheduled fr next week is a reminder f actin already taken. If we were t allw fr reminders f existing scheduled appintments then every prvider culd meet this bjective and measure withut any patient ever learning new infrmatin. S we clarify that reminders fr preventive/fllw-up care shuld be fr care that the patient is nt already scheduled t receive. Reminders are nt necessarily just t fllw up with the reminding EP. Reminders fr referrals r t engage in certain activities are als included in this bjective and measure. Prviders wh d nt knw a patient's preferred cmmunicatin preference may chse a preference fr the patient. Patients wh decline t receive reminders are cunted in the denminatr, but nt in the numeratr. Yu can send reminders thrugh Patient Prtal, print and mail reminders t patients, r call patients with reminders. CMS des nt dictate hw reminders are t be sent. Reprting cnsideratins: Panel queries are nt supprted fr creating reminder letters. The reprt will nly cunt reminder letters created frm the Patient Query patient cmmunicatin actin sets.

Reminders generated ut f the Practice Management system are nt cnsidered, because they are nt part f the Certified EHR Technlgy. If the cmmunicatin methd is verridden in Patient Query, the reminder is still included in the numeratr.
