1 Date: April 2015 Responsible to: Responsible for: Functional Relationships: Director of Clinical Services Inpatient and Community Team Management Internal Senior Leadership Management Team (SLMT) Interdisciplinary team (Clinical) Day Hospice Service Hospital Palliative Care Team (HPCT) Education and Research Unit (ERU) Quality Human Resource Administration Hospitality External MidCentralDHB Community Stakeholders Purpose To co-ordinate and have oversight of all patient care across the Inpatient and the Community teams within Arohanui Hospice to ensure patients and their family and or whānau receive consistently high quality care; To deliver a standard of day to day management which ensures that quality requirements are met and continuous improvement is a daily aim; To provide the required level of clinical leadership to the Inpatient and Community Nursing Teams, to ensure highly motivated and effective staff members. POSITION RESPONSIBILITIES To be an integral and effective member of the inter-disciplinary teams, maintaining an overview of the delivery of patient care by the Inpatient and Community teams Engages with the Inpatient and Community Team Leaders, providing the appropriate level of guidance and support for operational matters Engages directly with HPCT regarding the admission of current patients to the Inpatient Unit 1

2 Liaise closely where appropriate with CNS Referral and Day Service Coordinator regarding the acceptance of patients to the service and their suitability for community clinical teams or Inpatient Unit Care Work closely with the Medical team to facilitate the appropriate/timely level of input across the Inpatient and Community teams Works effectively with other clinical staff, valuing individuals with diverse skills, and using these skills to good effect Consults with and keeps interdisciplinary team members informed of changes to patient management as appropriate Attends, as appropriate, interdisciplinary/management forums both at Arohanui Hospice and with outside agencies To support the daily operational work within the Inpatient Unit and Community teams Ensures the effective co-ordination of patients and their family s/whānau while in the Inpatient Unit or receiving Community-based care Responsible for the rosters within the Inpatient and Community teams Maintains a broad overview of inpatient/family/whānau needs by communicating regularly with family/whānau and medical and nursing staff to ensure care is as seamless as possible. Promotes effective communication and teamwork within the interdisciplinary team Liaises effectively with the interdisciplinary team regarding admission, care and discharges for inpatients Acts as a link with the IPU and Community team regarding complex discharge planning Effectively manages unexpected or complex patient/family/whānau events that occur in the Inpatient unit To ensure a programme of continuous improvement in the Inpatient Unit and Community Teams Ensures hospice/palliative care philosophy is reflected in own nursing practice Provides specialist palliative care nursing support and advice to Inpatient and Community nursing staff Promotes the ongoing improvement of clinical practice by Inpatient and Community nursing staff Promotes and encourages a programme of continuous improvement within the Inpatient Unit and Community Teams Works with and supports CNS Education and Practice coordinator with orientation and mentoring of new nursing staff Contributes to the in-service education programme Works within the nurses code of professional conduct, as well as Arohanui Hospice policies Maintains medico-legal accountabilities and responsibilities 2

3 To ensure effective management of both staff and material resources for the Inpatient Unit and Community Teams Predicts patient/staff risk and responds appropriately Encourages and motivates staff Promotes and ensures high standards of clinical service delivery Communicates regularly and freely with Inpatient and Community Nurses Manages and seeks to resolve staff problems/issues Manages performance appraisals of Inpatient, Community and Nursing staff with support from the CNS Education Practice Coordinator Has responsibility for operational budgets and oversees the maintenance of supplies to the inpatient unit and the community To maintain own professional development and contribute to the education and training of others involved in palliative care Undertakes relevant education for own professional development in line with performance management process Be proficient at Expert PDRP level 4 or be prepared to work towards this Shares expertise and knowledge with others as an education resource at Arohanui Hospice as well as to outside agencies. Initiates own nursing research or support research to be carried out at Arohanui Hospice To maintain and develop quality standards within area of practice at Arohanui Hospice Contributes to the continuous quality assurance process at Arohanui Hospice Maintains relevant records and statistics as required Regularly monitors and evaluates ways of processes and procedures and seeks improvement through changes in operation To demonstrate acceptance and understanding of the Treaty of Waitangi and its principles Integrates the principles of Treaty of Waitangi into practice Is aware of the cultural protocols and practices when working with patients/whānau who are Māori Develops partnerships with patients/whānau and acknowledges individual need 3

4 To practice according to health and safety policy Role models safe practice and care in own nursing practice Is knowledgeable of the hospice s health and safety policies and procedures, including the reporting of accidents and incidents, and ensures nursing staff follow these procedures Actively contributes to a healthy environment by working alongside colleagues, patients and family/whānau to ensure a safe environment for practice and care Takes responsibility for the health and safety of your staff and self Ensures all hazards are identified and controlled Ensures that all staff, volunteers and contractors are provided with appropriate information, training and supervision Actively encourage and support local work place safety management practices Ensures accurate reporting and recording of all Health & Safety matters within Arohanui Hospice Essential PERSON SPECIFICATIONS Be a Registered Nurse with a current practicing certificate and have at least five (5) years experience in a clinical generalist/specialist setting Palliative Care experience or knowledge an advantage Understanding of Hospice philosophy Have the ability to work as part of our interdisciplinary team Demonstrated leadership skills with understanding of staff management and supervision requirements Take and enquiring approach and work around problems, rather than accept practices and assumptions as given Experience or knowledge of staff roster and acuity management A focus on quality improvement Have well developed verbal and written communications skills Have an understanding and application of the concept and the need for cultural safety Actively support and encourage teamwork Have a high standard of computer literacy, with experience in electronic patient management and MS Office suite of programmes Hold a clean and unrestricted Class 1 (Car) drivers licence. 4
