1 KATHARINE HOUSE HOSPICE JOB DESCRIPTION Advanced Nurse Practitioner (Independent Prescriber) Post Holder: Area of Work: Responsible to: Vacant Day Therapies Director of Nursing Services Mission To offer the best care so that people in our community affected by progressive diseases can live their lives to the full Department Objectives The purpose of Day Therapies is to provide specialist palliative care to adult patients, their carers and friends within the palliative care philosophy framework identified by the World Health Organisation. Overall Aims of the Post 1. To manage the Day Therapy, Well Being and Carers Support Services 2. To manage staff and volunteers within Day Therapy Services 3. To provide clinical and operational leadership within Day Therapies 4. To be responsible for setting, implementing and monitoring standards of patient care in line with the philosophy and principles of palliative care 5. To manage resources within defined budgets 6. To contribute to the delivery of the Katharine House Hospice Mission Statement 7. To ensure the on-going professional development of all nursing and care staff, working closely with the Director of Nursing Services, Medical Director and Clinical Lead Nurse The post holder will normally work Monday to Friday and will need to be flexible in their working hours to meet the needs of the service.

Clinical 2. Managerial 3. Teaching 4. Research Within these areas specific responsibility will be as follows: 1) Clinical i) Provide clinical excellence and leadership in palliative care, acting as a role model and resource for other staff on the unit ii) Assess, diagnose, plan and implement programmes of evidence based care and treatment for patients. iii) Admit, refer and discharge patients in accordance with hospice policy and procedures. iv) Provide specialist advice and support to patients and carers. v) Provide a visible clinical profile, having both an advisory and clinical input into patient care thus providing support to the multi-disciplinary team and facilitating the clinical development of registered nurses, health care assistants, other members of the multi-professional team and students on placement vi) When registered with Stafford Hospital Pharmacy the post holder will be expected to act as an independent and supplementary prescriber. In undertaking this role the post holder will follow the Katharine House Hospice Independent Nurse Prescribing and Supplementary Prescribing policy and procedures. vii) When registered with Stafford Hospital Pharmacy as an Independent Prescriber the post holder will conduct nurse led admissions in line with hospice policy viii) Encourage practice development utilising research and evidence based findings
3 ix) Promote a multi-professional approach to care by facilitating communication and co-operation between all Katharine House Hospice departments. x) Review the outcomes of treatment and care in conjunction with the other members of the multi-disciplinary team. xi) Ensure that the service reflects the Hospice philosophy in accordance with the Mission Statement, underpinned by the Hospice policies and procedures and statutory legislation xii) Ensure safe custody and administration of drugs in accordance with good practice, Hospice policies and procedures, Nursing and Midwifery Council and regulatory and legal requirements. xiii) Identify local, demographic, cultural and environmental factors that influence the health and quality of life of patients. xiv) Ensure that relevant Hospice policies and protocols are up to date and commensurate with evidence based practice, regulatory and legal requirements. 2) Managerial i) Ensure that efficient and flexible duty rotas for Day Therapy Services are co-ordinated providing a skill mix that ensures safe practice and continuity of care at all times in keeping with established resources and patient dependency. ii) Demonstrate clear, supportive and effective leadership. iii) Ensure all staff are aware of their role and responsibilities iv) Undertake regular performance review and appraisal of staff and implementation of personal development plans v) Monitor study leave and ensure feedback is received from all training undertaken in association with the Director of Nursing Services. vi) Actively support all staff, identifying any concerns and ensuring equity of access to available support systems and advising the Director of Nursing Services if there are problems or issues which may require further intervention or additional resources vii) Deputise for other service leads where necessary. viii) Contribute to ensuring that Katharine House Hospice meets the regulations and outcomes set by the Care Quality Commission ix) Attend, and participate in, appropriate management meetings and other relevant committees as required. x) Co-ordinate the introduction of new members of staff and volunteers to the team, ensuring appropriate induction and mentoring xi) Work closely with the Director of Nursing Services and Chief Executive in the management of all relevant budgets xii) Co-ordinate collection and maintenance of information relating to the services and submit reports to the Director of Nursing Services and committees as required xiii) xiv) xv) xvi) xvii) Participate in the recruitment and selection of staff and volunteers. Investigate, record and report accidents, complaints and other untoward occurrences concerning patients, visitors, staff or volunteers that arise. Monitor the standards of cleanliness and food provision within the services. Demonstrate vision to develop the services Seek opportunities to lead and support quality initiatives

Contribute to local, regional and national clinical networks and specialty specific groups, working in partnership with other key professionals xxvii) Analyse relevant national policy, advise on impact for service, and work with other key professionals in the implementation of changes and evaluation of processes. xxviii) Access clinical supervision for own practice with agreement from Director of Nursing Services. 3) Teaching i) Create and promote a learning environment by maximising teaching and learning opportunities for all nursing and care staff ii) Promote the development of clinical supervision for all nursing and care staff and to access clinical supervision for own practice iii) In conjunction with the Director of Nursing Services, carry out annual Training Needs Analysis iv) Participate in the development of the education strategy v) Participate in education programmes within and outside the Hospice as required vi) Maintain own personal commitment to the expansion and development of professional skills and knowledge and to foster similar attitudes in other members of staff 4) Research i) Assist staff in developing their skills in the understanding and use of research to inform practice ii) Initiate and participate in research projects, complying with the Hospice s Research Policy and Procedure iii) Ensure all clinical decisions are evidence based and to assist staff in bridging the theory/practice gap iv) Take responsibility for developing audit tools, and audit programmes, to enable quality of care to be monitored

6 Confidentiality To ensure confidentiality is maintained on all matters pertaining to patients, staff and organisation in accordance with the Hospice s Confidentiality Policy Infection Prevention and Control In accordance with the Health and Social Care Act 2008, the post holder will actively participate in the prevention and control of infection within the capacity of their role. The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work. Infection prevention and control must be included in any personal development plan or appraisal. Smoking The hospice has a no smoking policy. All hospice premises are considered No Smoking Zones This post requires the individual to consent to an enhanced disclosure by the Disclosure and Barring Service
