The new 400 bed Austin Hospital Tower opened mid 2005 and further redevelopment is underway. LOCAL WORK ENVIRONMENT This position is located within the Clinical Service Unit (CSU) of Medical & Emergency. The CSU contains the Department of Nephrology, General Medicine, Emergency, Endocrinology, Rheumatology, Dermatology, Infectious Disease, Paediatric Medicine, Podiatry and Clinical Pharmacology and Toxicology. The Diabetes Clinical Nurse Consultant employed in this position will be based at the Repat Campus of Austin Health and provides an education service to inpatients and outpatients. This position will work across all three hospitals within Austin Health and is associated with the Diabetes Hospital Admission Risk Program (HARP). The 3 campuses of Austin Health are Royal Talbot, Repat Campus and Austin Campus). DIABETES EDUCATION SERVICE Position Objectives To plan, implement and evaluate education for both inpatients and outpatients with diabetes and their families. To identify patients with diabetes early in their admission and to facilitate discharge planning. To coordinate and monitor the diabetes nursing care of patients in both inpatient and outpatient settings. To liaise with Nursing, Medical and Allied Health staff in planning and implementing care for patients with Diabetes To act as a resource person within the hospital setting in matters relating to the education and nursing management of patients with diabetes. To provide education on the management of patients with diabetes to nurses and other health professional staff. To ensure ongoing self-education in the education, care and management of people with diabetes so as to be able to provide quality, up-to-date care. 3 Position requirements 4. POSITION REQUIREMENTS Outpatients To attend Endocrine Unit and Diabetes Education Clinics as required. To provide assessments and education to outpatients with diabetes. To encourage goal setting and self management for people with diabetes. To refer patients to the HARP Diabetes Complication and Assessment Service, as required. To act as a resource for Outpatient Clinics. To provide education and resource material to staff at Outpatient Clinics. To collect, record and evaluate data within the Diabetes Education Services. Inpatients To provide education to patients with diabetes and their families in a timely manner. To provide resource material for patients and families. To provide support for discharge planning of patients with diabetes, including referrals to HARP Diabetes Complication and Assessment Service or external diabetes services as required. To attend (as able) and contribute to Endocrine ward rounds. Education, Research, Quality To contribute to updating departmental policies and procedures as required. To contribute to collecting and recording and evaluating of data within the Diabetes Education Services. To assist in developing educational material for staff and patients with diabetes. To participate in quality and research activities within the Diabetes Education Services and the Endocrinology Department. To provide staff in-service to areas including involvement in the 2 day diabetes training program for nursing and allied health staff within Austin Health. To provide education to other community groups as required and within resource constraints. Utilise clinical expertise in diabetes to research and assist in planning activities for the improvement of the Diabetes Service. To manage and undertake portfolios relevant to the service as required. Professional Development To attend meetings relevant to the area of Diabetes Education. To maintain and update own knowledge and skills, by attendance at seminars, workshops and conferences. Develop and update educational tools. Professional Development To attend meetings relevant to the area of Diabetes Education. To maintain and update own knowledge and skills, by attendance at seminars, workshops and conferences. To participate in the Austin Health performance appraisal with the Diabetes Education Services Manager. Key Selection Criteria Essential Essential Division 1 Registered Nurse Diabetes Educator s Certificate or working towards completion of same Member of ADEA Demonstrated diabetes knowledge and skills Eligible for membership of relevant professional association. Evidence of having participated in research and quality activities related to specialty area. Demonstrated ability to work as a sole practitioner or working towards this High level of communication skills both written and verbal Good computer skills Excellent problem solving & interpersonal skills Ability to prepare and deliver education to patients, families and staff.
