1 JOB DESCRIPTION JOB TITLE: DEPARTMENT: RESPONSIBLE TO: Clinical Nurse Specialist (CNS) Single Point of Contact (SPoC) Consultant Nurse SPoC OVERALL AIM OF POST The Single Point of Contact (SPoC) is the first project in a programme of change at St Christopher s. This exciting new development will be an integral part of St Christopher s delivery of services in to the future as we work towards our 5 year Strategic plan. The SPoC will link very closely with new initiatives, including case management, stratification and key working. The SPoC team will be the hub of St Christopher s, responding to urgent referrals and managing patient flows for an in-patient beds, community or supportive care intervention. In addition SPoC will respond to urgent requests for clinical support from external stakeholders. Primary Job Purpose To independently receive, assess and make decisions on referrals to St Christopher s, this will include a telephone assessment with the patient or carer using the skills of a clinical nurse specialist in symptom control, social, emotional and spiritual care. For urgent referrals this may include a face-to-face assessment. To offer advice, support and education to our external stakeholders, including the primary health care team, hospital staff, social care and voluntary services. To work from a multi-disciplinary approach to care. To participate in quality initiatives within the department. Principal Accountabilities Clinical Practice and Leadership To utilise advanced clinical reasoning skills and evidence based knowledge in the management of patients with highly complex presentations and to be clinically competent to utilise expert skills and knowledge; Recognise when further clinical information is necessary to support decision making and liaise with external colleagues; To direct and use the information gathered including results of investigations (e.g. blood tests) to plan which service the patient should be referred to for the appropriate interventions; Develop and conduct skilled nursing intervention with patients with palliative care needs. Addressing issues such as quality of life, futility, prognosis, withdrawing and withholding treatment and end of life decision making within an ethical framework; Take and analyse a clinical history in a relevant succinct and logical manner; Clinical Nurse Specialist (CNS) SPoC April

2 Record concisely, accurately, confidentially and legibly the appropriate elements of the history, examination, results of investigations, differential diagnosis and management plan; To use patient outcome measures as part of assessment; Present cases clearly, consistently and professionally to the MP team contributing effectively in patient management; Independently assess the patient s needs. Identify their goals and work with the patient and family towards achieving them; Interpret the results of investigations and discuss and liaise with colleagues to order them appropriately; Demonstrate an in depth knowledge of the options for achieving control of symptoms and ability to advise the primary health care team; Demonstrate an ability to explore the personal resources of families/carers in order to ensure patients are cared for in the place of their choice; Support and coach carers at home in managing a relative whose condition is deteriorating; Recognise the impact of caring for dying patients on primary health care team colleagues and offer support; Demonstrate sensitivity and respect for patients and families from different religious and cultural background and sexual orientation; Undertake both female and male catheterisation in the community; Demonstrate safe mobilisation, transfer and movement of patients in the community and how and where to obtain appropriate equipment; Manage the safe administration of medication in the community; Recognise and manage palliative care emergencies; Following the successful completion of the non-medical prescribing course and approval for practice, the RN may prescribe medications within agreed St Christopher s policies and procedures. To be part of the on-call rota - divided across the CNS team. Attend daily MDT meetings Communication Manage complex communication issues with patients and families; Anticipate barriers to communication, and take action to improve the communication skills and strategies of the team; Demonstrate advanced communication skills; Write and maintain accurate notes/documentation and input computerised data, keeping all information up-to-date; Demonstrate advanced presentation skills, both oral and written. Structure an interview using appropriate questioning, avoid jargon, using familiar language, at patients own pace; Communicate both verbally and in writing to patients whose first language may not be English in a manner that they understand, using interpreters appropriately; Break bad news in steps appropriate to the understanding of the individual and be able to support distress; Demonstrate an understanding of the need to involve patients and carers in decision making, offering them informed choices, respecting their views; Manage dissatisfied patients/relatives, anticipating potential problems; Demonstrate negotiation and diplomacy skills and partnership working with external agencies; Communicate effectively with members of the PHCT; Clinical Nurse Specialist (CNS) SPoC April

3 Demonstrate competent use of Electronic Patient Records and St Christopher s group IT systems Education and Training Generate and use appropriate learning opportunities and apply own learning to the future development of practice; Articulate and reflect palliative care skills, enabling others to learn; Implement and evaluate planned teaching programmes; Be competent in lecture and discussion based teaching; Mentor new staff and clinical placements Produce PowerPoint presentations; Use relevant AV equipment; Utilise experiential learning methods; Consistently draw on research and literature to influence specialist palliative care teaching; Write articles for publication, deliver at conferences, and produce posters for presentation. Obtain evidence based literature from various sources; Apply evidence to patient care; Grief, Loss and Bereavement Demonstrate skill in working with families facing loss; Identify and manage risk factors for adverse outcomes of bereavement; Liaise with external care giving agencies; Liaise with religious organisations in the community. Management Use networking and influencing skills to ensure the voice of the speciality is heard; Chair meetings effectively; Manage change effectively; Prioritise time effectively; Confidently join in debates in meetings of senior hospice staff; Make relationships with external health professionals; Work alone, prioritising work and flexibly responding to changing situations Assess risk and minimise threat to personal safety; Manage equipment resource responsibly; Develop new initiatives and look for opportunities and challenges that will extend current skill level; Process complaints according to policy Together with the consultant nurse take responsibility for building a cohesive MP team; Work within the guidance policy for the protection of vulnerable adults; Deputise for the consultant nurse; Mentor new team members Quality (including audit and research) Assist with the development of policies and procedures; Participate in forums for discussion on research and audit programmes; Ensure that audit results are presented at local and national forums as appropriate in order to Influence practice; Lead and support quality initiatives in local area. 4 Participate in clinical governance initiatives e.g. clinical review Engage in clinical supervision and self-evaluation and use this to improve care and practice; Professional Behaviour: Code of Conduct The post holder must comply with their relevant professional code of conduct at all times, where this exists. 5 PERSON SPECIFICATION: CLINICAL NURSE SPECIALIST (CNS) Qualifications: Essential Desirable NMC registration ENB 931 or Diploma/Degree in Palliative Care Possession of a valid/full UK driving licence Master s degree Non-medical prescribing course Advanced assessment skills course Proven teaching skills and community experience Community qualification Knowledge and Experience: Essential Desirable Proven palliative care/oncology with relevant qualifications and experience of working at Band 6/equivalent or above Sound knowledge of Symptom Control Counselling skills Ability to manage stress A thorough grasp of the fair access issues in Palliative Care An ability to translate equal opportunities/fair access values into practice Skills and Competencies: Essential Desirable Ability to work in a disciplinary team Excellent written communication skills Excellent oral and presentation communication skills Clinical Nurse Specialist (CNS) SPoC April
