We typically think of metabolic surgery outcomes in terms of our traditional procedures, particularly the Roux-en-Y gastric bypass, but there are many new interventions and indications being trialed, said session moderator Dana A. Telem, MD. With new indications, techniques and devices on the horizon, it s important to assess what, if any, effect they are going to have on metabolic outcomes. It s important to look beyond just weight loss and assess the effect these surgeries have on the person as a whole in improving overall health. Dr. Telem is associate director of the Stony Brook Metabolic and Bariatric Weight Loss Center and assistant professor of surgery in the division of foregut, bariatric, general and advanced gastrointestinal surgery at Stony Brook University Medical Center, Stony Brook, NY. Hiroyuki Ono, MD, who invented endoscopic submucosal dissection (ESD) in the 1990s in Japan, will be faculty at a special ASGE Hands-On Workshop on Sunday. A separate registration fee is required to attend the ESD workshop, which is limited to 40 participants. It s our privilege to have him because he hasn t taught hands-on sessions at DDW in the past, said workshop course director Norio Fukami, MD, FASGE, associate professor of medicine at the University of Colorado School of Medicine, Aurora. He kindly agreed to do this for the first dedicated ESD hands-on. Drs. Ono and Fukami will be joined during Sunday s workshop by other ESD Sunday s 90-minute session, which is sponsored by the SSAT Research Committee, will begin with a review of current metabolic surgery options, including the Roux-en-Y gastric bypass, the duodenal switch and sleeve gastrectomy. The discussion will focus on short-term and long-term outcomes for metabolic syndromes such as diabetes, high blood pressure and high cholesterol. The next speaker will discuss patients who may benefit from metabolic surgery. Currently, the NIH criteria to qualify for bariatric surgery is a BMI of 35 with a comorbid condition, or a BMI of 40 without any comorbid condition, Dr. Telem said. But there are studies looking at lower-bmi patients and at different risk profiles of patients in terms of who should be offered these operations and who is going to benefit from them the most. The session s third speaker will discuss the biology behind metabolic surgery, assessing DAILY NEWS Digestive Disease Week ddwblog.org The present and future of metabolic surgery to be examined SSAT Norio Fukami, MD, FASGE pioneers and experts from Japan, Hong Kong, Europe and the U.S. During the interactive session, 10 faculty members will guide participants through the various procedures and answer attendee questions. They will review upper and lower ESD techniques with explanations about how to use ESD-specific knives and special techniques. In previous years at DDW, ASGE combined ESD with endoscopic mucosal resection (EMR) in one hands-on workshop, but Dr. Fukami said interest in ESD has grown to the point that it needs its own session. Even so, he said ESD remains vastly underutilized in the U.S. because of a lack of training, despite the the different gut hormones, how they interact, and mechanisms of action at a molecular and physiological level. Another speaker will discuss novel endoluminal devices, including the Maestro Rechargable System, manufactured by EnteroMedics, which was recently approved by the Food and Drug Administration for treating obesity. It s important to assess the data being accrued on this and other devices concerning their effect on metabolic outcomes and not just weight loss, Dr. Telem said. The final speaker, an interventional radiologist, will describe a potential future technique for weight loss involving embolization of the left gastric artery, and the procedure s effect on weight loss and weight-related medical comorbid conditions. Those populations will be a primary focus of Sunday s AASLD Clinical Symposium Hepatitis C Recommendations for The symposium will also provide a comprehensive overview of regimens to fight hepatitis C virus (HCV) treatment options that will be available for clinical use in The session will open with a review of current treatment protocols for patients across the disease spectrum, followed by three talks about the treatment of special populations. The special patient groups to be discussed are: recipients of liver transplants, patients with HIV co-infection and patients with renal failure, said Raymond T. Chung, MD, FAASLD, director of hepatology and the Liver Center at Massachusetts General Hospital, Boston. This is particularly remarkable for recipients of liver transplants for HCV liver disease, he said, because essentially all of these patients experience a recurrence of hepatitis C, and previous interferon-based regimens produced limited response rates and often-intolerable side effects. Yet, Dr. Chung noted, the advent of new oral agents does not eliminate the need to take special steps with these populations.
