1 Admissin Scheduling The Scheduler will receive an ACE task ntifying them when the referral is ready fr the Admissin Visit t be scheduled. Same Day Admissin Visits: If Intake receives a referral requiring a same day admissin visit, Intake will call the Scheduler t ntify them. The scheduler will review the patient infrmatin created by intake t determine which discipline(s) need t be scheduled. Once the visit has been scheduled, the scheduler will ntify the clinician assigned t the patient via telephne. The Clinician will perfrm a quick synchrnizatin t pull the patient recrd dwn t their device. Rutine Admissin Visits: Befre scheduling the visit, the Scheduler will need t review the patient infrmatin in Allscripts t determine the discipline(s) t schedule and the timeframe fr the visit as required by reviewing the MD Ordered SOC Date n the Admissins & Status screen and reviewing the visit frequency screen. 2 T get t the Admissins & Status Screen: Right-Click n the task in yur My Day Left click n Open Patient Click n General Click n Admissins & Status OR G t the Patient Cmpnent Open the Crrect Patient by using the yellw Search flder Click n General > Admissins & Status If there is a Physician Ordered SOC Date, this is the date that the physician specifically stated n the Referral Order fr the agency t begin Hme Health Services. If there is n Physician Ordered SOC Date entered, the agency has 48 hurs frm the referral date r 48 hurs frm the date discharged frm the hspital t cmplete the Admissin visit. Page 2 f 13

3 T get t the Visit Frequency Screen: Frm the Patient cmpnent Select the crrect patient Click n Clinical > Visit Frequency The visit frequency screen will specify which disciplines need t be scheduled fr the admissin visit. If nursing has been rdered, it will ALWAYS be the admitting discipline. Open the scheduling cmpnent (Blue Mantle Clck Icn) r Allscripts Hmecare > Schedule Frm the Staff Scheduling Mde pen the crrect pssible clinicians/therapists using the Yellw Search Flder Page 3 f 13

4 If yu need t select a clinician based upn a specific skill, allergy r need, yu will have t g int the Patient scheduling mde and use Match when yu g t select a resurce fr the visit being scheduled. Once yu click n the yellw search flder the fllwing screen appears Search fr the crrect Resurce(s) by searching by first and last name (r partial f each) When the crrect Resurce is displayed in the Fund (tp) bx duble click n the Resurce s name t add it t the Selected (bttm) bx. Once the crrect Resurce(s) are listed in the Selected bx click Ok. The schedule fr all Resurces selected will display. Page 4 f 13

5 Select the crrect day n the calendar n the left side f the screen. Duble click in the timeslt yu want t schedule the visit in fr the crrect resurce. If the resurce yu are scheduling the visit with has multiple rles yu will have t select the crrect rle frm the list available befre the New Event screen will pp up. The individual hur blcks are brken dwn int 15 minute increments with the dtted lines hwever if yu select the wrng timeslt yu can change this in the new event screen that appears. (belw) Page 5 f 13

6 Rle Cnfirm this is the crrect rle r discipline t be scheduled. This is especially imprtant if the resurce yu are scheduling has multiple rles. Patient Click n the ellipsis ( ) buttn t select the crrect patient. When yu click n the ellipsis buttn beside Patient the Select Patient windw will appear. Page 6 f 13

7 Search fr the patient by First and Last Name r partial f each. Once the crrect patient is displayed and is highlighted in Blue either duble click n the patient r click Ok. Svc Cde (Service Cde) Select H200 Admissin Visit frm the drp dwn list Page 7 f 13

8 Branch Ensure that the patient s branch is crrect. This aut ppulates frm the Admissins & Status Screen Date Ensure that the date in this field is the crrect date f the visit yu are scheduling Time Ensure that the time is this field is the crrect date f the visit yu are scheduling Prvisinal Check the Prvisinal bx if the visit is t nt be mved utside f that day If there was a Physician Ordered Start f Care date the visit will need t be marked prvisinal If the visit CANNOT be mved frm this date mark it prvisinal Duratin Ensure that there is a duratin entered (des nt need t be an exact duratin as the duratin will change nce the visit is cmpleted by the clinician) Indirect Time Leave blank Travel Time Leave Blank Ttal Time Leave as is Mileage Leave Blank Supply Leave Blank Click Ok. The Visit has nw been scheduled fr the Resurce. Yu will nw need t g back int the patient cmpnent t ensure the crrect Resurce is listed in the apprpriate Assignment area. T get t the Assignments screen: Patient > Clinical > Assignments Page 8 f 13

9 Ensure the crrect resurce yu scheduled is listed in the Admitting Clinician field If nt listed in Admitting Clinician field: Click in ID field beside Admitting Clinician and click the ellipsis buttn ( ) Once yu click the ellipsis buttn the select Admitting Clinician windw will appear. Search fr the Admitting Clinician by searching by First Name, Last Name r partial f each. Page 9 f 13

10 Once the crrect Resurce is listed n the right hand side f the search windw and highlighted blue, either duble click n the resurce r click Ok. Yu will nw need t lk thrugh all assignments t verify that they are: Crrect All assignments knwn thus far are assigned t the crrect Resurce Cder Scheduler (Self) Intake Nurse Physician Insurance Verificatin Marketer (as applicable This will almst always be needed) Case Manager Clinical Manager If any f the assignments knwn are nt listed n the assignments screen yu will need t enter thse in using the same prcess t select a Resurce as listed abve. Cmplete the Welcme Call and dcument in the cmments sectin f the ACE task. Change the Status Cde in the Admissins & Status screen. Patient>General>Admissins & Status Change the status frm P27 Ready fr Scheduling t P24 Referral Cmplete. Page 10 f 13

11 Save the screen nce changing the status t P24 Referral Cmplete G back t the ACE Task yu received ntifying yu f the Admissin Visit needing t be scheduled. My Day > Duble Click n the Task OR Allscripts Hmecare > Alerts and Tasks > My Tasks and select the apprpriate task. Page 11 f 13

12 Add a new cmment stating the utcme f scheduling. T add a new cmment click n the add rw buttn under Cmment Histry Cmment shuld cntain any pertinent infrmatin relating t scheduling the admissin visit. Change the Task status t Cmpleted Save the Screen. Page 12 f 13

13 If there are n staff available t schedule the admissin visit t, Cntact the Clinical Manager fr Assistance. If after assistance frm the Clinical Manager a resurce is able t be made available t cmplete the Admissin Visit, fllw directins abve t cmplete the scheduling f the visit. If after assistance frm the Clinical Manager a resurce is nt able t be made available t cmplete the Admissin Visit, the clinical manager will change the patient status t NTUC using the apprpriate reasn cde and create a clinical nte with the details where MD AND Referral Surce were ntified.
