1 Cnnecticut State Department f Educatin Schl Health Services Infrmatin Survey General Directins fr Cmpletin by Schl Nurse Crdinatr/Supervisr This Schl Health Services Infrmatin Survey was designed by the Cnnecticut State Department f Educatin (CSDE) and Dr. Mhra Lrentsn and Kevin Glass frm EDUCATION CONNECTION, with cnsultatin frm the State Health Recrds Cmmittee. It represents an exciting pprtunity t cllect and reprt data n the health needs f students, the health services prvided by schls, and the health staff prviding leadership and services t students and schls in Cnnecticut. This infrmatin will be used t infrm plicy makers, administratrs, schl health staff, and the cmmunity n health trends and services, and t prvide recmmendatins fr the expansin f services as needed. The survey is cnducted yearly. Results are cmpiled in an annual health service reprt. The annual reprt will dcument state and reginal data by District Reference Grup (DRG). Each district will receive a cpy f the annual health service reprt. Only the lead schl health crdinatr (i.e. schl nurse supervisr r ther designated schl nurse cntact) will cmplete the survey based n district-wide infrmatin. The survey is nly available n-line. T ensure cmplete representatin fr the state, it is the CSDE s expectatin that every schl district will cmplete and return the survey. Nw that this is the twelfth year f data cllectin, the CSDE hpes that the majrity f this infrmatin is readily available within the districts. If yur district des nt have access t this infrmatin, please prvide yur best estimate r d nt cmplete the questin (except where required). Preparatin fr Survey Cmpletin: G t the EDUCATION CONNECTION Web site described belw and dwnlad the sample Schl Level Data Cllectin Frm r use yur wn district data cllectin frm. Prvide this frm r yur district s frm t ne nurse in each schl fr cmpletin at least ne mnth prir t cmpletin f the Health Services Survey. Prir t cmpletin f the Schl Health Services Infrmatin Survey, the Schl Nurse Crdinatr/Supervisr will tabulate results t prvide district ttals. Please nte that all questins n the Schl Health Services Infrmatin Survey d nt require schl level input. Only questins fr which schl level input is required are included n the Schl Level Data Cllectin Frm. In sme cases, schl respnses will be used t estimate a district respnse (frequencies and cmments). In ther cases, each schl will prvide specific numbers f staff r students. In these cases, numbers will be ttaled t prvide a ttal number fr the district. Cmpleting the Survey: As in previus years, we will be utilizing SurveyMnkey fr data cllectin. All infrmatin needed t cmplete the survey may be fund n the EDUCATION CONNECTION Web site at Near the tp f the website, yu will see a list f links. The last link in the rw is titled Schl Services. Please click that link. On the left side f the next page yu will see anther list f links. Click n the bttm link in the list Center fr Prgram Research and Evaluatin. On the next page, the bttm link n the left is the State Department f Educatin Health Services Survey. Please click n that link t enter the SDE Schl Health Services Web page. On that Web page yu will see the Survey/Questinnaire, a frm fr data cllectin by schl nurses at the building level that yu, as the crdinatr/supervisr, may find helpful, the letters f intrductin t the schl nursing supervisr and the reprts frm the previus years f data cllectin. 2 als find a summary reprt tracking trends ver time. The Web site als includes a list f all schl districts in Cnnecticut and the schl cde. Please cmplete each page f the survey. At the bttm f each page, yu will see buttns saying NEXT (t g t the next page) and SAVE (if yu want t save the partially cmpleted survey). T SAVE the survey and cme back t it later, click the SAVE buttn. Yu will be prmpted t put the page int yur favrites r bkmarks. T recall the partially cmpleted survey, g t yur favrites r bkmarks and select the survey. When yu are ready t submit the cmpleted survey, click FINISH n the last page. At this pint, the survey is submitted and is n lnger available t yu thrugh the bkmark. If yu are interested in keeping a cpy f yur respnses, please print each page f the cmpleted survey frm the Web befre yu submit it. Directins fr Specific Sectins: In general, yu will prvide the numbers f an item, percentages that represent yur data, r the range f an item that best represents the district data. Data may be entered either by clicking n the apprpriate respnses with yur muse, selecting the apprpriate respnse frm a drp-dwn bx, r typing the apprpriate respnse int the bx prvided. Sectin I: Services Prvided Sectin A. In this sectin, each schl in yur district shuld prvide yu with the number f screenings cnducted and the number f referrals as a result f the screenings cnducted in PUBLIC SCHOOLS within yur district. DO NOT INCLUDE NUMBERS FROM PRIVATE, NON-PROFIT SCHOOLS WITHIN SECTION A. The number f screenings and referrals shuld be based n screenings cnducted frm September f the current schl year until cmpletin f the survey. Based n the individual schl infrmatin, the lead schl health crdinatr will give the ttal fr the entire district. Fr the mandated screenings, nly the number f referrals will be recrded since all schls are required t prvide screenings in mandated grades. Yu will als be asked t prvide the ttal number f students in the public schl district. PLEASE NOTE THAT THIS QUESTION MUST BE COMPLETED. THE SOFTWARE WILL NOT ALLOW YOU TO MOVE TO THE NEXT PAGE IN THE SURVEY UNLESS A RESPONSE TO THIS QUESTION IS PROVIDED. When entering data int the shrt answer sectin, please make sure t include summaries f schl level respnses as well as yur wn respnse. In this sectin, each schl in yur district shuld prvide yu with the number f screenings cnducted and the number f referrals as a result f the screenings cnducted in PRIVATE, NON- PROFIT SCHOOLS within yur district. DO NOT INCLUDE NUMBERS FROM PUBLIC SCHOOLS WITHIN SECTION B. The prcess and questins are identical t thse described abve. All numbers shuld refer ONLY t private nn-prfit schls. PLEASE NOTE THAT QUESTION 21 MUST BE COMPLETED BEFORE THE SOFTWARE WILL ALLOW YOU TO MOVE TO THE NEXT PAGE IN THE SURVEY. IF YOU DO NOT HAVE STUDENTS IN THE PRIVATE, NON-PROFIT SCHOOLS, PLEASE PUT A 0 IN THE BOX FOR QUESTION 21. Sectins II and III: Health Service Staffing and Staff Qualificatins There is n schl level data required fr this sectin. In this sectin, the lead schl health crdinatr will prvide the number f nurses (measured by fulltime equivalent r FTE) in this categry that are emplyed in the public and private, nn-prfit schls. N schl level data is required in this sectin. The lead schl health crdinatr will als prvide the numbers f nursing supprt staff. In this sectin, the fllwing definitins apply: Nurse Leader: The schl nurse designated as the schl nursing supervisr, head nurse, r nursing crdinatr fr the schl district. Schl Nurses: RNs emplyed in schls that meet the qualificatins f a schl nurse pursuant t Sectin Nurse Practitiner: a nurse emplyed by the schl district that has an advanced degree (APRN) r is licensed as a nurse practitiner. Permanent Flat Nurse: a nurse emplyed by the schl district and is available t flat between schls as needed as a substitute nurse r t assist in ther buildings as needed. One t One Nurse: a nurse assigned t a specific student n a regular basis. Cntracted Nursing staff: Nurses wrking in schl buildings (nt ne-t-ne nurses wrking with specific students). Hwever, these nurses are nt emplyed by the lcal schl system but are cntracted by the schl frm private nursing agencies. Other questins in this sectin are self-explanatry. District Level Infrmatin needed t cmplete this sectin: Educatinal achievement f Registered Nurses (Highest level f educatinal attainment) Number f Registered Nurses that hld Natinal Certificatin in schl nursing. Number f Registered Nurses hlding Natinal Certificatin in any ther nursing field. Qualificatins f Nurse Leaders Specialists emplyed by the district such as a Nutritinist, Mental Health Cnsultant, Psychiatrist, Assistive Technlgy Specialist. Page 3f 5

4 Sectin IV: Student Health Fr questins 1-31, each schl nurse will need t prvide the lead schl health crdinatr with the number f students in each categry fr their public r private, nn-prfit schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be recrded separately. The lead schl health crdinatr will then prvide the ttal number f public schl students with each health care need in the left clumn and the ttal number f students within the private, nn-prfit schls in the right clumn. Fr questins 32-63, each schl is asked t prvide their respnses t these questins n the Schl Level Data Cllectin Frm. The Schl Nurse Crdinatr/Supervisr will use their respnses t prvide a district level respnse. Please nte that schl level respnses are intended as GUIDES fr the Schl Nurse Crdinatr/Supervisr. The Schl Nurse Crdinatr/Supervisr will use schl respnses t infrm the district level respnse. Descriptins f the interventins are prvided n the individual schl wrksheet. Fr questin 53, each schl nurse will need t prvide the lead schl health crdinatr with the number f students with life threatening fd allergies requiring the administratin f epinephrine in their public r private, nn-prfit schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be ttaled tgether. The lead schl health crdinatr will then prvide the ttal number f public AND private nn-prfit students requiring the administratin f epinephrine within the district. Fr questins 64-76, each schl nurse will need t prvide the lead schl health crdinatr with the types f health care prcedures perfrmed in each public r private nn-prfit schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be recrded separately. The lead schl health crdinatr will then prvide the types f prcedures perfrmed within the public schls in the left clumn and the types f prcedures perfrmed in the private, nn-prfit schls in the right clumn. Fr questin 77, each schl is asked t prvide their respnses t this questin n the Schl Level Data Cllectin Frm. The Schl Nurse Crdinatr/Supervisr will use their respnses t prvide a district level respnse. Please nte that schl level respnses are intended as GUIDES fr the Schl Nurse Crdinatr/Supervisr. The Schl Nurse Crdinatr/Supervisr will use schl respnses t infrm the district level respnse. Fr questins 78-80, each schl nurse will need t prvide the lead schl health crdinatr with the percent f students dismissed in each public r private nn-prfit schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be recrded separately. The Schl Nurse Supervisr/Crdinatr will use their respnses t prvide a district level respnse fr public schls in the left clumn and fr private, nn-prfit schls in the right clumn. Fr questins 81-83, each schl nurse will need t prvide the lead schl health crdinatr with the percent f students dismissed t each lcatin in each public r private nn-prfit schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be recrded separately. The Schl Nurse Supervisr/Crdinatr will use their respnses t prvide a district level respnse fr public schls in the left clumn and fr private, nn-prfit schls in the right clumn. Fr questins 84a - 84d, each schl nurse will need t prvide the lead schl health crdinatr with the number f 911 calls made fr students AND fr adults in their public r private, nn-prfit Page 4f 5

5 schl building as f February 1, Numbers fr public r private, nn-prfit buildings shuld be recrded separately. Questins 85a and 85b are ranking questins. Each schl will need t prvide a respnse frm 1-4 in each clumn crrespnding t the MOST frequent reasn fr a 911 call, with (1) being the MOST frequent reasn fr a 911 call, and (4) being the LEAST frequent reasn. Questins 86a - 86f are mandatry and new t the survey fr the schl year and address the ttal number f diagnsed cncussins, frequency and lcatin f ccurrences, resulting accmmdatins and number f affected students missing schl days. Each schl is asked t respnd t these questins n the Schl Level Data Cllectin Frm. Please nte that schl level respnses are intended as GUIDES fr the Schl Nurse Crdinatr/Supervisr wh will use them t infrm the district level respnse. Fr Questin 87, the Lead Schl Health Crdinatr will prvide the ttal number f public schl students in the district withut health insurance in the first clumn, and the ttal number f students withut health insurance frm private, nn-prfit schls in the fllwing clumn. The final questins in this sectin are self-explanatry. Sectin V: Health Crdinatin and Educatin The Schl Level Data Cllectin Frm requests infrmatin frm each schl in several f these categries. This infrmatin is intended t serve as a GUIDE fr the Schl Nurse Crdinatr/Supervisr. The Schl Nurse Crdinatr/Supervisr will use schl respnses t infrm the district level respnse. The majrities f questins in this categry cntains check bxes r yes/n questins and are selfexplanatry. The final sectin is demgraphics. The district infrmatin and cde must be filled in fr tracking and verificatin purpses and t prevent duplicatin f data. Please remember that published reprts will nly reflect data by state and by DRG.
