On behalf of The Florida End Stage Renal Disease (ESRD) Network 7, it is our pleasure to invite you to Kissimmee, Florida to attend the 2014 Annual Forum STEP UP TO THE PLATE - Hit a Home Run in ESRD Care! which will be held from November 19-21, In partnership with practitioners, healthcare providers, and ESRD stakeholders, the Network continues to assess and improve the quality of care and the quality of life of ESRD patients in Florida. We achieve this by supporting continuous improvement in patient care outcomes and the safety and satisfaction of ESRD patients and providers through patient advocacy, collaboration, education, monitoring, and mentoring. The 2014 Annual Forum program will focus on key areas of interest to members of the renal community, including: physicians, administrators, PAs, ARNPs, social workers, nurses, dietitians, transplant personnel, and technicians. We anticipate captivating presentations by local and nationally recognized experts in ESRD and are excited about the variety of topics that will motivate you with new ideas and action steps to implement within your organization. 4 Game Highlights Discuss methods for implementing quality strategies pertaining to patient-centered care, teamwork, quality, safety, and strategies for shared decision-making. Enhance participants use and understanding of the National Healthcare Safety Network (NHSN) data reporting, NHSN analysis feature, and data trending. Describe strategies to improve the patient experience for the ICH CAHPS survey, identify ways to trend survey results, and target focus areas for improvement. Examine strategies to improve ESRD patient outcomes, including the importance of fluid management, weight loss for dialysis/ckd patients, vascular and transplant updates in addition to new approaches in peritoneal dialysis, use of the Hero graft, and respite care for home hemodialysis. Offer an in-depth review and discussion of integrative techniques for stress management, staff support, and addressing mental health issues in the dialysis facility.
