2 OVERVIEW Advanced practice nurses and physician assistants are increasingly integrated into the care of patients across the spectrum of kidney diseases. The Renal Physicians Association (RPA) has designed a program which highlights the relevant areas of clinical nephrology and provides current information that applies directly to nephrology practice. Whether a nurse practitioner or physician assistant is new to nephrology and needs fundamental knowledge or is an experienced practitioner looking for additional information, there are a variety of offerings at this meeting to meet both needs. Please join us as we collaborate to share knowledge and continue to improve the quality of care provided to kidney patients. This program is jointly sponsored by the Renal Physicians Association and the Office of Continuing Medical Education of the University of Virginia School of Medicine. Target Audience This meeting is designed for physicians assistants, nurse practitioners, registered nurses and other healthcare providers working in nephrology practices.

3 GENERAL INFORMATION Hotel The 2013 RPA Advanced Practitioner Conference will be held at the Embassy Suites Portland Downtown. This elegant all-suite hotel is accessible to the city s taxfree stores and popular restaurants. The hotel is conveniently located adjacent to public transportation including the Max Light Rail System with direct connections to the Oregon Convention Center, Portland International Airport, Oregon Zoo and other popular attractions. Transportation Portland International Airport is located 9 miles or 25 minutes from the Embassy Suites Portland Downtown. 4 Educational Objectives At the conclusion of this conference, participants should achieve the following educational objectives. RPA Legislative and Regulatory Update Identify legislative and regulatory requirements for the provision of care to renal patients. Describe the regulatory and legislative requirements related to the roles and responsibilities of advanced practitioners in outpatient, inpatient and home settings. QIP and the Bundle Describe the bundled payment system and its requisite components covered by Medicare that roughly covers 80% of costs to the ESRD patient. Outline the effects of the Quality Incentive Program (QIP) on payment for delivery of services to ESRD patients. New Models of Renal Care- How does the Advanced Practitioner Fit In? Review the elements in an ACO (accountable care organization) and the roles of Advanced Practitioners in meeting the requirements for patient care delivery. Clinicopathological Session Link the clinical presentation of common kidney diseases to the underlying pathology of these conditions. Urgent Start PD Outline the process for initiating an unprepared patient to peritoneal dialysis in an urgent clinical situation. Prioritize the critical aspects of the process necessary for successful initiation of urgent start PD. Home Hemodialysis Review the options available for starting and maintaining a home hemodialysis program. Describe the approaches to fostering increased and appropriate utilization of home hemodialysis. Acute Kidney Injury Review the causes and epidemiology of acute kidney disease. Discuss the diagnosis and outcomes for patients experiencing acute kidney injury. Transplant Complications Identify the common complications associated with kidney transplant. Describe the approaches for management of post-kidney transplant complications. Transitions of Care Identify the challenges encountered by patients/families as they transition from outpatient to inpatient and home settings. Discuss strategies that can facilitate seamless transitions of care for patients/ families.

5 The Pitfalls of Social Media in Medicine Identify the current patterns of the use of social media in patient care. Review the approaches for social media use that meet current standards of care, and regulations for patient privacy. HD Rounds Describe elements that should be included in a monthly care plan that promotes high quality care for patients with kidney disease. Drug Toxicity and Renal Disease Review the medications that can cause kidney injury, and the consequences to kidney function. Describe the approaches that can be used to prevent or minimize kidney injury through medications. Monitoring of Dialysis Accesses Discuss strategies for monitoring and tracking the dialysis access sites to help ensure patient safety and delivery of care. PD Pearls Identify the primary medical challenges for patients receiving dialysis therapy. Discuss the approaches to manage and coordinate care to address the medical challenges associated with peritoneal dialysis. Primary Care in Dialysis Identify the priorities for providing primary care to patients in dialysis. Describe the approaches that can facilitate effective primary care management in coordination with dialysis care. Bone-Mineral Management in the Bundle Discuss the impact of the Medicare reimbursement regulations on the management of bone-mineral metabolism. Outline strategies for maintenance of appropriate bone-mineral metabolic status for patients on dialysis. Medico-legal Vignettes Discuss approaches for common medicolegal issues faced by Advanced Practice professionals in the management of patients with kidney disease. Cognitive Function in CKD Discuss the relationship of cognitive function to chronic kidney disease and the clinical presentation of variations in cognitive function. Explore therapeutic options available to patients with altered cognitive function associated with chronic kidney disease. Management of CKD Discuss common co-morbid conditions (e.g., hypertension, diabetes, altered lipid and uric acid levels) and their progression in chronic kidney disease. Review general medical management approaches with an emphasis on pharmacology designed to minimize the progression of chronic kidney disease.

6 Continuing Education physician assistant credit The Physician Assistant Review Panel is pleased to award a maximum of hours of AAPA Category I CME credit, with a maximum earnable by a single attendee of 17.5 hours, to the above-referenced program entitled Renal Physician Association 2013 Advanced Practitioners Conference provided by the Rector and Visitors of the University of Virginia. Nursing Credit This activity has been submitted to the Virginia Nurses Association for approval to award contact hours. The Virginia Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center s Commission on Accreditation. Pharmacology Credit VCU School of Pharmacy, Office of Continuing Education is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Upon successful completion of all learning assessment activities and submission of an evaluation form, participants will receive up to 19.5 credits.
