3 RUN FOR HOPE is a 5K timed run and walk benefitting and produced by Albany Area Primary Health Care, Inc., a community-based health organization, which provides high quality, comprehensive services to those who face barriers to accessing care.

4 Dance for HOPE Charity Ball is considered AAPHC s premier philanthropic celebration. This elegant, adult-only affair, features wonderful cuisine, special guests and spectacular entertainment. We are proud to acknowledge that Dance for HOPE is hosted by the Consumer Advisory Board of AAPHC s Rural Model Clinic, a body of extraordinary patients dedicated to furthering the mission of AAPHC: to provide quality health care for everyone. All proceeds will support HIV/AIDS research, ongoing community outreach and education and direct medical services at AAPHC s Rural Model Clinics. Through these special events, we strive to not only educate the communuity but to honor those affected by the condition while serving as a viable tool in the fight against HIV/AIDS.

5 Dear Community Partner: For over 35 years Albany Area Primary Health Care has helped shape our community by providing quality health care for everyone. As a non-profit Community Health Center, often our resources do not allow us to expand services in areas of great need. Without the assistance of community-minded individuals like you, we would not be able to continue the growth of our organization and expand the services we provide. 6 Status of HIV ingeorgia Health District 8-2 (Albany/Dougherty) ranks 10th for citizens living with HIV in the state of Georgia The number of newly diagnosed AIDS cases decreased 7% from 2011 to 2013 Over 75% of persons living with HIV in Georgia are male African-Americans continue to be disproportionately impacted by HIV. In GA 55% percent of new diagnoses of HIV infection in Georgia are among Black/Non-Hispanics. Source: Georgia Department of Public Health (2012)

Provides comprehensive outpatient medical services, including dental care Offers at-cost HIV-related medications through an on-site pharmacy that fills thousands of prescriptions each year Offers treatment adherence services, insurance navigation and public benefits assistance to more than 7,000 clients Improves mental health through behavioral health services Administers hundreds of free HIV tests each year Partners with cutting-edge scientific research and service organizations on HIV and STD treatment and prevention
