HILLIARD THARP SIXTH GRADE SCHOOL EXTRACURRICULAR ACTIVITIES PROGRAM FALL SESSION 2, 2016 This fall, Hilliard Tharp will provide sixth grade students the opportunity to participate in an after school extracurricular activity. A variety of courses will be offered on different days and at different times. This program will be conducted by Hilliard Tharp staff members. Please read the requirements below carefully as you consider participation in any of the activities. There are two necessary requirements that parents need to keep in mind before registering a student. First, it is absolutely imperative that transportation is arranged by you ahead of time so that your child can return home safely after each session. Secondly, it will be necessary for instructors to work free of disciplinary problems. Should any student present a persistent problem during the activity period, he/she will be excluded from the remaining activity dates and will forfeit the fee. Please note the following guidelines when registering: All outstanding user fees (including user fees from elementary schools) must be paid in full prior to registering for and participating in an extracurricular activity. You can pay online by using MyPaymentsPlus.com If you pay by cash or check, or on-line a separate registration form must be completed for each course your child chooses to participate in (there are 3 forms on the next page that can be cut into individual slips). On-line, in the exact amount for each course, is the preferred form of payment. All checks must be made payable Hilliard City Schools. Any returned check fees are the parent/guardian s responsibility and need to be reimbursed in cash to the District. Activities are filled on a first-come, first-served basis. If a course with a maximum enrollment becomes full, no other checks will be accepted. The check or money will be returned. If a course is canceled due to low enrollment, your check or money will be returned. Payment must be received with the registration form to guarantee enrollment. Students will NOT be notified of enrollment. ALL students who have paid should attend the first class scheduled unless informed otherwise. We always attempt to accommodate all registrants. Understanding there may be a potential risk of injury in some of the offered activities; the signed registration to participate releases the instructors of the course, the school, and the school district from liability. It is our hope that many students will be able to take advantage of these activities.
