1 Chabt Cllege Nursing Applicatin Prcess Advanced Standing/Transfer/LVN s/prgram Re-Admissin Step 1 A Chabt Cllege Applicatin fr Admissin¹ must be submitted t the cllege. This applicatin can be submitted electrnically by ging t and is available year rund. Step 2 Cmplete Nursing Advanced Standing Applicatin¹ n-line r dwnlad applicatin. Nursing Advanced Standing / LVNRN Applicatins are accepted March 1 st thrugh Octber 1 st f each year. Step 3 Print, sign and MAIL the Nursing Advanced Standing Applicatin (Pages 3-6) t: Chabt Cllege, Admissin and Recrds Hesperian Blvd. Hayward, CA If yu are registered fr ne f the pre-requisite natural science curses verificatin f enrllment is required with the applicatin. It is recmmended yu btain a Certificate f Mailing and Return Receipt frm the U.S. Pstal Service at the time f the mailing. Step 4 The student must request fficial transcripts frm all previusly attended schls and clleges. These must be MAILED t Admissin and Recrds, Chabt Cllege. Hand carried transcripts will nt be accepted. LVN applying fr 30 unit (nn-degree) ptin must als submit high schl transcripts. Step 5 G t n-line t click Class-Web and lgn, click Student Services, Student Recrds t verify receipt f transcripts. Due t the vlume f applicants we are nt able t ntify students individually if any infrmatin is missing. Step 6 LVN s selecting Optin 1 r 3, and Advanced Standing Students shuld make an appintment with a cunselr fr a preliminary evaluatin f yur transcripts. Step 7 After evaluatin f yur transcripts, the Advanced Standing Applicatin and transcripts will be frwarded t the Nursing Prgram Directr. Yu will then be ntified by mail regarding yur eligibility and curses required fr yu t enter and cmplete the prgram. Please allw 6-8 weeks fr this prcess t cmplete. Step 8 AFTER receipt f letter frm the Nursing Prgram Directr, students wishing t enter the prgram must cmplete all curses r testing as utlined in their individual letter. Step 9: LVNRN Transitin Curse students must attend mandatry rientatin listed n webpage. In August, LVN s will be required t enrll in Nursing 70/N70L, LVNRN Transitin Curse. This curse is a review f nursing tpics including nursing care plan, nursing prcess, cmmunicatin principles, and legal/ethical cncepts cmmn t the rle f registered nurse. It will explain the functins f the Califrnia Bard f Registered Nursing and selected psychmtr skills utilized by the registered nurse in the delivery f patient care. Step 10: Once the Advanced Standing applicatin has been submitted yu have 2 years t cmplete the ATI exams, thery classes and/r skills testing and be ready fr admissin t the full time prgram. ¹Any falsificatin f data n the applicatin(s) will result in immediate disqualificatin.

2 Advanced Standing Order f Prgram Acceptance Waitlist frm previus semester Student must have cmpleted all ATI testing, all skills testing. All nursing prgram classes nted n evaluatin letters with C r better. All students must be enrlled in N84 Clinical Skills t practice and test skills at the beginning f the semester. All students in current prgressin fr: Fall Prgram Admissin must have basic skills and all ATI testing cmpleted by April 1st; Final skills exam by May 1 st and hspital by June 30 th *. Incming students will be evaluated January f each year t set ranking and again n April 15 th f each year t determine if standards have been met and re-ranked if necessary. Spring Prgram Admissin must have basic skills and ATI testing cmpleted by Nvember 15th; Final Skills exam by December 15 th and hspital by January 10 th *. If space is limited, students will be ranked by 11/15. In December the list will be evaluated t determine if standards have been met and re-ranked if necessary. 2. 3 Advanced Standing and LVN * Registered Nursing Applicatin T be cmpleted by LVN s and Re-Admissin Chabt Students t the Chabt Nursing Prgram All applicants are advised that space availability is limited t penings that ccur after the first day f the Fall Semester f the first year f the Chabt Nursing Prgram. These penings are created nly when smene leaves the prgram r declines admissin t the prgram. Every effrt is made t tailr the applicant s educatinal needs t the available spaces as they ccur. Sme placements may take lnger than thers, depending n the individual applicant s needs. A backgrund check is required n all nursing students. We will review the fllwing n everyne accepted t the prgram: Scial Security Trace, Misdemeanr r Felny Criminal Cnvictin Search, Cmbined OIS.GSA exclude parties/debarment List Search and a Pre-placement Drug Screen. If yu have been cnvicted f a misdemeanr f felny it is pssible yu will be denied placement in ne r mre f ur hspitals that are used fr clinical experience r the Bard f Registered Nursing (BRN) may nt grant yu a license. Please cntact the BRN r make an appintment fr a cnfidential meeting with the Nursing Prgram Directr fr guidance in this area. Admissin Requirements: All students must take and pass the Test f Essential Academic Skills (TEAS ) Versin V with Adjusted Individual scre f 62.0%. Student must have been previusly enrlled in an accredited Registered Nursing Prgram within the last tw years. Student must have an verall GPA f 2.0 r higher. Licensed Vcatinal Nurse (LVN) must be currently wrking and hld a current State f Califrnia LVN License. LVN must have a minimum f ne year full time experience. Submit a letter frm emplyer stating jb title and start date f psitin. Pre-Requisite Requirements: the student must have a minimum in 4-5 Semester Units with a labratry cmpnent f each f the fllwing curses: Anatmy 1 Physilgy 1 Micrbilgy 1 If the abve curses have nt been cmpleted with a C r higher prir t submitting this applicatin, yu must prvide prf f current enrllment in the missing class at the time f submitting this applicatin. If student cmpletes all testing and space is nt available a waitlist will be established. All candidates accepted int the prgram as Advanced Standing Students will need t demnstrate theretical and clinical cmpetency thrugh written and clinical perfrmance examinatins fr Fundamentals f Nursing, Nursing Care f Children, Maternal Newbrn Nursing, Nutritin and Pharmaclgy assessments. A fee is charged fr each exam and is the respnsibility f the student. Any prspective applicant needs t understand that Chabt Clleges Nursing Prgram has a tw admissin plicy. If any student des nt received a C r better in all the curses in the nursing majr she/he will be dismissed frm the prgram and is eligible fr re-admissin ne mre time nly.
